Abstract
Bu araştırma perkütan endoskopik gastrostomi yerleştirilen hastalarda uygulanan hemşirelik bakımının komplikasyonlara etkisinin incelenmesi amacıyla yapıldı. Randomize kontrollü nitelikteki araştırmanın evrenini İstanbul ili Anadolu yakasında bulunan bir üniversite hastanesinde 3 Ocak 2022–16 Aralık 2022 tarihleri arasında endoskopi ünitesinde perkütan endoskopik gastrostomi uygulanan tüm hastalar, örneklemini ise bu tarihler arasında araştırmaya dahil olma kriterlerini kapsayan 60 hasta oluşturdu. Araştırmanın verileri Sosyodemografik Veri Formu, Bakım Verici Bilgi Formu, İşlem Öncesi Hazırlık-Öncesi- Sırası İzlem ve Takip Veri Formu, Perkütan Endoskopik Gastrostomi Sonrası Yaşam Bulguları, Stoma ve Tüp Takip Formu ve Taburculuk Sonrası Evde Bakım Değerlendirme Formu ile toplandı. Araştırma sonucunda elde edilen verilerin istatistiksel analizleri için IBM SPSS V22 programı kullanıldı. Normal dağılıma uygunluk Shapiro-Wilk Testi ile incelendi. Gruplara göre normal dağılan değişkenlerin karşılaştırılmasında Bağımsız Örnekler t Testi, normal dağılıma uymayan değişken karşılaştırılmasında Mann Whitney U Testi kullanıldı. Gruplara göre kategorik değişkenlerin karşılaştırılmasında Yates Düzeltmesi, Fisher's Exact Testi ve Pearson Ki-Kare Testi kullanıldı ve çoklu karşılaştırmalar Bonferroni Düzeltmeli Z Testi ile yapıldı. Analiz sonuçları nicel değişkenler için ortalama ± standart sapma ve ortanca (minimum – maksimum) şeklinde, kategorik değişkenler için frekans (yüzde) şeklinde sunuldu. Önem düzeyi p<0,050 olarak alındı. Araştırmaya katılan kontrol grubu ve müdahale grubundaki hastaların ve bakım vericilerin sosyodemografik özelliklerine göre gruplar arasında istatistiksel olarak anlamlı fark saptanmadı (p>0,05). Perkütan endoskopik gastrostomi sonrası müdahale grubunun yaşam bulguları ölçümü ile kontrol grubunun ölçümleri arasında anlamlı fark bulunmadı (p>0,05). İşlem öncesi ağız bakımı yapılma durumuna göre hastanın ateşi olma durumunun dağılımları arasında istatistiksel olarak anlamlı bir farklılık bulundu (p=0,049). Müdahale ve kontrol gruplarına göre PEG kanülü bakımı uygulandığı durumların dağılımları arasında istatistiksel olarak anlamlı bir farklılık bulundu (p<0,001). Gruplara göre bakım ve beslenme öncesi hijyen işlemlerden sonra ellerini yıkama durumların dağılımları arasında istatistiksel olarak anlamlı bir farklılık saptandı (p<0,001). Gruplara göre hastayı beslemeden önce su verme durumların dağılımları arasında istatistiksel olarak anlamlı bir farklılık bulundu (p=0,001). Gruplara göre tüpü bakım sırasında çevirme durumlarının dağılımları arasında istatistiksel olarak anlamlı bir farklılık bulundu (p<0,001). Gruplara göre beslenme tüpünde tıkanma olma durumlarının dağılımları arasında istatistiksel olarak anlamlı bir farklılık saptandı (p=0,004). Gruplara göre beslenme tüpünde sızdırma olma durumlarının dağılımları arasında istatistiksel olarak anlamlı bir farklılık bulundu (p=0,001). Perkütan endoskopik gastrostomi işlemi sonrasında hemşirelik bakımının komplikasyonları önlemede etkin olduğu sonucuna ulaşıldı. Bu araştırmada hemşirelik bakımının hastalarda komplikasyon gelişimini engellemede etkisi olduğu sonucuna ulaşıldı. Araştırmanın daha büyük gruplarda, eğitim tekrarı yapılarak ve ağız bakımına 48 saat önce başlanarak tekrar edilmesi önerilmektedir. This study was carried out to examine the effects of nursing care applied to complications in patients who underwent percutaneous endoscopic gastrostomy. The population of the randomized controlled study consisted of all patients who underwent percutaneous endoscopic gastrostomy in the endoscopy unit between 3 January 2022 and 16 December 2022 in a university hospital located on the Asian side of the Istanbul city, and the sample consisted of 60 patients who met the criteria for inclusion in the trial between those dates. The data of the study were collected with Sociodemographic Data Form, Caregiver Information Form, Follow-up Data Form Before and During Preparation for Procedure, Vital Signs After Percutaneous Endoscopic Gastrostomy, Stoma and Tube Follow-up Form, and Post-Discharge Home Care Evaluation Form. IBM SPSS V22 program was used for statistical analysis of the data obtained as a result of the research. Conformance with normal distribution was reviewed with the Shapiro-Wilk Test. To compare variables distributed normally by groups, the Independent Samples t Test was used; and to compare variables not distributed normally, Mann-Whitney U Test was used. To compare categorical variables by groups, Yates Correction, Fisher's Exact Test and Pearson Chi-Square Test were used; and multiple comparisons were done with the Bonferroni Correction Z Test. Analytical results were presented as mean ± standard deviation and median (minimum – maximum) for quantitative variables, and as frequency (percentage) for categorical variables. Significance level was taken as p<0.050. There was no statistically significant difference between the groups based on the sociodemographic characteristics of the patients and caregivers in the control group and intervention group participating in the study (p>0.05). Following percutaneous endoscopic gastrostomy, there was no significant difference between the measurements of the vital signs of the intervention group and of the control group (p>0.05). A statistically significant difference was found between the distribution of the patient's fever status regarding oral care before the procedure (p=0.049). A statistically significant difference was found between the distribution of PEG cannula care status regarding the intervention and control groups (p<0.001). A statistically significant difference was found between the distribution of iv handwashing status regarding the intervention and control groups (p<0.001). A statistically significant difference was found between the distribution of pre-feeding hydration of patient status regarding the groups (p=0.001). A statistically significant difference was found between the distribution of tube rotation status during care regarding the intervention and control groups (p<0.001). A statistically significant difference was found between the distribution of tube clogging status regarding the intervention and control groups (p=0.004). A statistically significant difference was found between the distribution of tube leaking status regarding the intervention and control groups (p=0.001). It was concluded that nursing care after PEG procedure is effective in preventing complications. In this study, it was concluded that nursing care has an effect on prevention of complications. It is recommended that the study be repeated in larger groups by repeating the training and by starting oral care 48 hours earlier.
