Göğüs tüpü çıkarılmasında soğuk uygulamanın ağrı düzeyine etkisi

Abstract

Bu araştırma tüp torakostomi uygulanan hastalarda göğüs tüpü çıkarılmadan önce soğuk uygulamasının, tüp çıkarıldıktan sonra ağrı düzeyleri üzerine etkisinin belirlenmesi amacıyla yarı deneysel araştırma olarak yapıldı. Araştırmanın evrenini İstanbul ilinde yer alan T.C. Sağlık Bakanlığı İstanbul Bakırköy Sadi Konuk Eğitim ve Araştırma Hastanesi Göğüs cerrahisi servisinde yatmakta olan hastalar, örneklemini ise göğüs tüpü takılı olan ve çalışmaya katılmaya gönüllü 60 hasta oluşturdu. Araştırmanın verilerinin elde edilmesinde araştırmacılar tarafından literatüre göre hazırlanmış 'Hasta Tanıtım Formu', 'Sayısal Derecelendirme Ölçeği' kullanıldı. Araştırmanın verilerinin değerlendirilmesi SPSS23 programı paket programı ile yapıldı. Kategorik değişkenler için frekans, yüzde değerleri verildi. Sürekli değişkenler için ortalama, standart sapma, medyan, minimum ve maksimum değerleri verildi. Sürekli değişkenlerin normal dağılım sınaması Kolmogorov Smirnov testi ile yapıldı. Kategorik değişkenler arası ilişkiler için Ki-kare analizi ile yapıldı. Uygun olan durumlarda kategorik değişkenler Fisher exact ve Fisher Freeman Halton Testi ile değerlendirildi. Normal dağılım sergileyen sürekli bağımsız değişkenlerde iki grup karşılaştırması için Bağımsız örneklem t testi kullanıldı. Normal dağılım varsayımını gerçekleştirmeyen bağımsız iki grup karşılaştırmasında Mann Whitney U testi değişkenlerde kullanıldı. Normal dağılım sergileyen bağımlı değişkenler için iki grup karşılaştırmalarında Bağımlı örneklem t testi kullanıldı. Normal dağılım sergilemeyen Bağımlı değişkenlerde ise, iki grup arası karşılaştırmalar için Wilcoxon'un işaret sıralar testi kullanıldı. p<0,05 istatistiksel olarak anlamlı kabul edildi. Araştırma sonucunda soğuk uygulama yapılan deney grubunda göğüs tüpü çıkarılması öncesi ve sonrası ağrı düzeyi arasındaki fark istatistiksel olarak anlamlı bulunmuştur (p=0,005<0,05). Deney grubundaki hastaların göğüs tüpü çıkarılmasında ağrı düzeyi öncesi ortalaması 3,84±2,53 ve sonraki ağrı düzeyi ortalaması 2,48±2,31 olarak saptandı. Soğuk uygulama sonrasında ağrı düzeyinin anlamlı düzeyde düştüğü görüldü. Kontrol grubunda ise göğüs tüpü çıkarılması öncesi ve sonrası ağrı düzeyi arasında anlamlı fark bulunmadı (p=0,536>0,05). Kontrol grubundaki hastaların göğüs tüpü çıkarılmasından ağrı düzeyi öncesi ortalaması 3,03±2,81 ve sonraki ağrı düzeyi ortalaması (3,17±2,25) olarak bulundu. Bu sonuçlara göre göğüs tüpü çıkarılan hastalara uygulanan soğuk uygulamanın ağrı düzeyini azalttığı saptandı. Anahtar Kelimeler: Soğuk Uygulama, Göğüs Tüpü, Göğüs Cerrahi.THE EFFECT OF COLD APPLICATION ON PAIN LEVEL IN CHEST TUBE REMOVAL This study was carried out as a semi-experimental study in patients undergoing tube thoracostomy, to determine the effect of cold application before tube removal, on pain levels after tube removal. This study was carried out on patients with tube thoracostomy who interned in Thorax Surgery Clinic and conduct as a semi-experimental study to determine the effect of cold application on pain levels in chest tube removal. The universe of the study consisted of patients who were hospitalized in the Thoracic Surgery Department of T.C.İstanbul Bakırkoy Dr. Sadi Konuk Training and Research Hospital, İstanbul and the sampling of the study is 60 volunteer patients with tube thoracostmy. 'Patients İnformation Form' and 'Numeric Rating Scale Form' which were prepared according to the literature were used by the researchers to gather the data of this study. The data of the study were evaluated with SPSS-23 package program. Frequency, percentage values were given for categorical variables. Mean, standart deviation, median, minimum and maximum values were given for continuous variables. Normal distribution test of continuous variables was performed with Kolmogorov Smirnov test. Chi-square analysis was performed for categorical variables. Where appropriate, categorical variables were evaluated by Fisher exact and Fisher Freeman Halton Test. İndependent sample t-test was used for comparison of two groups in continuous independent variables with normal distribution. The Mann-Whitney U test was used compare two groups that did not perform the normal distribution assumption. Dependent sample t-test was used for the two groups for the dependent variables that exhibited normal distribution. İn the dependent variables that did not show normal distribution, Wilcoxon's sign rank test was used for comparisons between the two groups. Statistically significant.p<0,05 was considered. The analyzes were performed with SPSS-23 program(Statistical Package For The Social Sciences). As a result of this study, the difference between the pain levels before and after the removal of chest tube was found statistically significant in the experimental group(p=0,005<0,05). The mean pain level of the experimental group before the removal of the chest tube was 3,84±2,53 and the mean pain level after chest removal was 2,48±2,31. After the cold application, the level of pain decreased significantly. İn the control group, no significant difference was found between the pain level before and after the removal of the chest tube(p=0.536>0,05). The mean pain level of the control group before chest tube removal was 3,03±2,81 and the mean pain level after tube removal was found to be 3,17±2,25. According to these results, it was found that cold application to the patients who had chest tube removed decreased the pain level. Keywords: Cold Application, Chest Tube, Thoracic Surgery.
