Abstract
Bu araştırma kolonoskopi yapılacak bireylerin konstipasyon risk skorunun belirlenmesi ve bağırsak temizliği yeterliliği ile arasındaki ilişkiyi değerlendirmek amacıyla tanımlayıcı tipte yapılmıştır. Araştırmanın evrenini; İstanbul ilinde bir Tıp Fakültesinin Genel Cerrahi Anabilim Dalı Endoskopi Ünitesine kolonoskopi için başvuran hastalar, örneklemini ise; 14.06.2021-12.10.2021 tarihleri arasında, 18 yaşından büyük, okuma yazma bilen, bilinci açık, işitme ve konuşma engeli olmayan, hastanede günübirlik yatışı olan, kolorektal cerrahi öyküsü olmayan ve araştırmaya katılmaya gönüllü 185 hasta oluşturmuştur. Araştırmanın verileri; Kişisel Bilgi Formu, Kabızlık Risk Değerlendirme Ölçeği ve Boston Bağırsak Hazırlığı Sınıflaması kullanılarak elde edilmiştir. Araştırma verileri değerlendirilirken tanımlayıcı istatistiksel yöntemlerin (sayı, yüzde, ortalama, ortanca, standart sapma vb.) yanı sıra, Kolmogorov-Smirnov, Mann-Whitney U Testi, Kruskal Wallis-H Testi, Pearson Ki-Kare testi, Continuity Correction testi, Fisher's Exact testi, Spearman korelasyon testi, Bonferroni düzeltmesi ve Çoklu Linear ve Lojistik Regresyon analizlerinden yararlanıldı. Araştırmaya katılan hastaların yaş ortalamalarının 51,97±13,64 yıl olduğu, %50,8'inin kadın, %81,6'sının evli, %38,4'ünün ilkokul mezunu, %51,9'unun ev hanımı, %46,5'inin kilolu olduğu ve %63,2'sinin sürekli bir ilaç kullandığı saptandı. Hastaların tamamına bağırsak temizliği için oral bağırsak hazırlık ilacı, %96,2'sine ise lavman uygulandığı, %7,6'sında bağırsak temizliği yetersizliği nedeniyle kolonoskopi işleminde çekuma ulaşılamadığı ve bu hastalara kolonoskopi işleminin tekrar uygulandığı belirlendi. Araştırma kapsamındaki hastaların %74,1'inin Kabızlık Risk Değerlendirme Ölçeğine göre düşük risk grubunda yer aldığı, Boston Bağırsak Hazırlığı Sınıflamasına göre de %65,4'ünün bağırsak hazırlığının yeterli olduğu saptandı. Kabızlık Risk Değerlendirme Ölçeği toplam puanı ile Boston Bağırsak Hazırlığı Sınıflaması puanı arasında istatistiksel olarak anlamlı ve negatif yönlü bir ilişki varlığı bulundu (r=-0,150; p=0,041). Araştırma sonucunda hastaların konstipasyon risk düzeylerinin düşük, bağırsak temizliğinin çoğunlukla yeterli olduğu, konstipasyon risk düzeyi azaldıkça bağırsak hazırlığı yeterlilik düzeyinin de arttığı bulundu. Araştırmanın daha fazla değişken ile daha büyük örneklemde tekrar edilmesi önerilmektedir. Anahtar Kelimeler: Bağırsak Hazırlığı, Hemşirelik, Kolonoskopi, Konstipasyon
The aim of this study is to determine the constipation risk score of individuals who will undergo colonoscopy and to evaluate the relationship between bowel cleansing adequacy in a descriptive type. The universe of the research is patients who applied to the Endoscopy Unit of the General Surgery Department of a Faculty of Medicine in Istanbul for colonoscopy between 14.06.2021 and 12.10.2021. 185 patients who were older than 18 years old, literate, conscious, had no hearing and speech impairment, were hospitalized for a day, had no history of colorectal surgery, and volunteered to participate in the study was included. The data of the research was obtained using Personal Information Form, Constipation Risk Assessment Scale and Boston Bowel Preparation Classification. While evaluating the research data, in addition to descriptive statistical methods (number, percentage, mean, median, standard deviation, etc.), Kolmogorov-Smirnov, Mann-Whitney U Test, Kruskal Wallis-H Test, Pearson Chi-Square test, Continuity Correction test, Fisher's Exact test, Spearman correlation test, Bonferroni correction and Multiple Linear and Logistic Regression analyzes were used. The mean age of the patients was 51.97±13.64 years, 50.8% were women, 81.6% were married, 38.4% were primary school graduates and 51.9% were housewives. 46.5% of the patients were overweight and 63.2% of them were using medication. Oral bowel preparation was applied to all patients for bowel cleansing, and enema was applied to 96.2% of the patients. It was found that 74.1% of the patients were in the low risk group according to the Constipation Risk Assessment Scale, and 65.4% of them had adequate bowel preparation according to the Boston Bowel Preparation Classification. A statistically significant and negative correlation was found between the Constipation Risk Assessment Scale total score and the Boston Bowel Preparation Classification score (r=-0.150; p=0.041). As a result of the study, it was found that the constipation risk levels of the patients were low, the bowel cleansing was mostly sufficient, and the level of bowel preparation adequacy increased as the constipation risk level decreased.
