Abstract
Bu retrospektif klinik çalışma, pratik, tek basamaklı bir hibrit HYRAX genişletme apareyi tasarımının klinik etkinliğini değerlendirmeyi, ve bu aygıtla yapılan hızlı üst çene genişletmesinin dişsel ve iskeletsel etkilerini incelemeyi amaçlamaktadır. Standart bir HYRAX vidası ve tekrar kullanılabilir mini vida manşonlarından meydana gelen, maliyet ve zaman açısından verimli, tek basamaklı bir hibrit HYRAX apareyi, bir süredir İstanbul Okan Üniversitesi Ortodonti Doktora Kliniğinde kullanılmaktadır. Bu araştırma, aynı klinikte 2019-2022 yılları arasında tedavi görmüş, yaşları 12-18 arasında değişen 19 hastanın kayıtları üzerinde yapılmıştır. Maksiller genişletme öncesi (T0) ve sonrasında (T1) alınan frontal sefalometrik radyografiler ve dijital ortodontik modeller üzerindeki iskeletsel ve dental ölçümler IBM SPSS Statistics 22 programı kullanılarak istatistiksel olarak karşılaştırılmıştır. Tüm parametrelerin normal dağılıma uygun olduğu saptanmıştır (Shapiro Wilks testi). T0-T1 karşılaştırmalarında eşleştirilmiş t-testi kullanılmıştır. (p<0.05). Bu yeni tasarımın, klinik pratiğinde, hibrit genişletme apareyinin daha kolay ve daha sık kullanılmasını mümkün kıldığı görülmüş, maksiller transversal yetmezlik bu aygıt ile etkili bir şekilde tedavi edilmiştir. Tüm hastalarda yeterli iskeletsel genişleme sağlanmış; maksiller kemik genişliği ve dental ark genişliği arttırılmıştır (-5,21±3,77mm, p:0.002; p<0.05). Anterior maksiller genişliğin posterior maksiller genişlikten (-1,94±7,86mm, p:0.296; p>0.05) daha fazla arttığı (-4,81±5,65mm, p:0.003; p<0.05) ve sağ ve sol maksiller kemikler arasındaki açının arttığı gözlenmiştir (3,27±2,65°, p:0.001; p<0.05). Bu çalışma, uygun maliyetli ve üretimi kolay olan bu tasarımın, diğer herhangi bir hibrit genişletici gibi, daha az dental yan etki ve daha fazla iskeletsel etki oluşturarak, maksillayı genişletmeyi sağlayabildiğini göstermektedir.
This retrospective clinical study aims to evaluate the efficiency of a practical one-step hybrid HYRAX expander design in clinical routine, and to examine the dental and skeletal effects of rapid maxillary expansion with this device. This cost- and time efficient one-step hybrid HYRAX, consisting of a standard HYRAX screw and reusable mini screw adaptors, had been designed and was being used for some time at İstanbul Okan University Postgraduate Orthodontic Clinic. This retrospective study was conducted on the records of 19 patients aged between 12-18, treated in this clinic between the years 2019-2022. Skeletal and dental measurements on frontal cephalometric radiographs and digital orthodontic models taken before (T0) and after (T1) maxillary expansion were statistically compared using IBM SPSS Statistics 22. All parameters conformed to normal distribution (Shapiro Wilks test). The paired sample t-test was used for T0-T1 comparisons (p<0.05). Clinically, it was observed that the new design made easier and more frequent use of hybrid expansion appliance possible. Maxillary transversal insufficiency was effectively treated with this appliance. Adequate skeletal widening has been achieved for all patients; maxillary bony width, as well as dental arch width increased (-5,21±3,77mm, p:0.002; p<0.05). Anterior maxillary width inceased (-4,81±5,65mm, p:0.003; p<0.05) more than posterior maxillary width (-1,94±7,86mm, p:0.296; p>0.05), and an increase in the angle between the right and left maxillary bones was observed (3,27±2,65°, p:0.001; p<0.05). This study shows that this cost effective and easy-to-produce design is capable of expanding the maxilla, with decreased dental side effects, and increased skeletal effectiveness, like any other hybrid expander.
