Açık kalp ameliyatı uygulanacak olan hastaların umut düzeyi ve spiritüel iyi oluşlarının belirlenmesi

Abstract
Bu çalışma, açık kalp ameliyatı uygulanacak olan hastaların umut düzeyi ve spiritüel iyi oluşlarının belirlenmesi amacıyla yapılmıştır. Tanımlayıcı ve kesitsel nitelikteki çalışmanın örneklemini Sağlık Bilimleri Üniversitesi İstanbul Kartal Koşuyolu Yüksek İhtisas Eğitim ve Araştırma Hastanesi'nin kalp-damar cerrahisi servislerine Şubat-Haziran 2019 tarihleri arasında açık kalp cerrahisi için yatışı yapılan 166 hasta oluşturdu. Araştırma verileri, tanımlayıcı veri toplama formu, Herth Umut İndexi ve Spiritüel İyi Oluş Ölçeği ile toplandı. Verilerin değerlendirilmesinde,sayı-yüzdelik hesaplamaları, Kruskal Wallis testi, Pearson korelasyon analizi, ANOVA testi ve t testi kullanılmıştır.Çalışmaya katılan hastaların %69,3'ü erkek, %72,3'ü 51 yaş ve üzeri yaş grubunda, %45,3'ü ilkokul mezunu ve %84,9'u evlidir. Hastaların % 51,2'si by-pass ameliyatı geçirdiğini, %84,3'ü daha önce kalp ameliyatı geçirmediğini, %41'i 6 gündür hastanede kaldığını ve %62'si manevi destek almadığını belirtti. Hastaların Spiritüel İyi Oluş Ölçeği puan ortalaması 115,93±10 ve Herth Umut İndexi puan ortalaması 39,11±5,40 olarak bulundu. Hastaların spiritüel iyi oluşlarının ve umut düzeylerinin yüksek olduğu belirlendi.Hastaların cinsiyeti, yaş grubu, eğitim durumu, medeni durumları, hastaların ameliyat türü, daha önce kalp ameliyatı geçirme durumu, hastanede kalış süresi ve manevi destek alma durumu ile Spiritüel İyi Oluş Ölçeği puan ortalaması arasında istatistiksel olarak anlamlı farklılık saptanmadı (p>0,05). Hastaların eğitim durumu, daha önce kalp ameliyatı geçirme durumu ve hastanede kalış süresi ile Herth Umut İndexi puan ortalaması arasında istatistiksel olarak anlamlı farklılık saptanırken (p<0,05); cinsiyetleri, yaş grubu, medeni durumları, ameliyat türü ve manevi destek alma durumu ile ölçek puan ortalaması arasında istatistiksel olarak anlamlı farklılık saptanmadı (p>0,05).Hastaların Spiritüel İyi Oluşları ve Herth Umut İndeksi arasında ilişki saptanmadı (p>0,05).Hemşirelerin açık kalp cerrahisi geçirecek olan hastaların umut düzeyleri ve manevi iyi oluşlarını değerlendirmesi önerilmektedir. The aim of this study was to determine the level of hope and spiritual well-being of patients undergoing open-heart surgery. The sample of this descriptive and cross-sectional study consisted of 166 patients who applied to the cardiovascular surgery services of Kartal Koşuyolu High Specialization Training and Research Hospital between February and June 2019. Research data, descriptive data collection form, Herth Hope Index and Spiritual Well-being Scale were used. Number-percentage calculations, Kruskal Wallis test, pearson correlation analysis, Anova test and t test were used to evaluate the data. 69.3% of the patients were male, 72.3% were 51 and older, 45.3% were primary school graduates and 84.9% were married. Of the patients, 51.2% had undergone by-pass surgery, 84.3% had never undergone heart surgery, 41% had been hospitalized for 6 days and 62% had not received any spiritual support. The mean score of the Spritual Well-being Scale of the patients was 115.93±10 and the average score of Herth Hope Index was 39.11± 5.40. Spiritual well-being and hope levels of the patients were determined to be high.There was no statistically significant difference between the gender, age group, educational status, marital status, type of surgery, previous cardiac surgery, length of hospital stay, and spiritual support and the mean score of spiritual well-being (p> 0.05). There was a statistically significant difference between the educational status, previous cardiac surgery and hospital stay and mean Herth Hope Index (p <0.05). There was no statistically significant difference between sex, age group, marital status, type of surgery and spiritual support status and mean scale score (p> 0.05). There was no correlation between Spritual Well-being and Herth Hope Index (p> 0.05). It is recommended that nurses evaluate hope levels and spiritual well-being of patients undergoing open heart surgery. Key words: Hope,Open-heart surgery, Spiritual well-being
