Abstract
Bu tez çalışmasında amacımız, kliniğimizde geliştirilen trident apareyi ile infrazi-gomatik krest vidasından destek alarak gerçekleştirilen üst birinci molar diş meziali-zasyonunu lateral sefalometrik film ve dijital model analizleriyle değerlendirmektir. 5 kadın, 10 erkek toplam 15 bireyden mezializasyon öncesi ve sonrasında alınan se-falometrik filmler ve dijital modeller araştırmamızın gerecini oluşturmuştur. Araştır-maya dahil edilen bireylerin 15 yaş ve üstünde olmasına, daimi dentisyona geçmiş ol-masına, tek veya çift taraflı üst molar mezializasyonu ihtiyacının infrazigomatik krest vidası ve trident apareyi ile tedavi edilmiş olmasına ve sistemik olarak sağlıklı olması-na dikkat edilmiştir. Mezializasyon başlangıcında yaş ortalaması 19.2±6.7 olan 15 bireyde seviyeleme aşaması tamamlandıktan sonra 0.018 inç paslanmaz çelik teller üzerinde mezializas-yona başlanmıştır. İnfrazigomatik krest vidalarının yerleştirildiği seans mezializasyon başlanmıştır. Kapalı nikel-titanyum yaylar ile 300 gr kuvvet uygulanmıştır. Tedavi süresince meydana gelen değişiklikleri belirlemek amacıyla toplam 7 para-metre ölçülmüştür. Ölçümlerin değerlendirilmesi sonucu 7 parametrenin 6 tanesinde istatistiksel olarak anlamlı değişiklikler bulunmuştur. İstatistiksel veriye göre; üst birinci molar diş 3.25 mm mezialize, 0.4 mm ekstrüze olmuştur. 2.85˚ mezial yönde devrilme görülmüştür. 2.48˚ meziopalatinal rotasyon saptanmıştır. Üst keser dişlerde 7.23˚ proklinasyon bulunmuş, okluzal düzlem 2.63˚ saat yönünün tersine rotasyon yapmıştır.
The aim of this study is to evaluate the mesialization of upper first molars supported by infrazygomatic crest miniscrews and the trident appliance which was a method de-veloped in our clinic. Analysis was conducted with the help of lateral cephalometric radiographs and digital models. The cephalometric radiographs and digital dental casts were taken before and after mesialization from 5 females and 10 males, with a total of 15 individuals to compose the material of our research. Subjects over the age of 15, a permanent dentition, a need for single or bilateral upper molar mesialization with a infrazygomatic crest miniscrew and trident appliance, and absence of systemic diseases were the inclusion criteria for this study. The aligning process was completed before mesialization in 15 individuals with a mean age of 19.2±6.7. The mesialization phase began with 0.018 inch stainless steel wires and immediate force was applied after infrazygomatic crest miniscrew insertion. 300 gr of force was applied with nickel titanium closed coil springs. A total of 7 parameters were measured to determine the changes that occured during the treatment. As a result of the evaluation of the measurements, 6 of the 7 parameters were found to be statistically significant. According to the statistical data; upper first molar tooth was mesialized 3.25 mm and extruded 0.4 mm. 2.85˚ mesial tipping was observed. 2.48˚ mesiopalatinal rotation was detected. 7.23˚ proclination was found in the upper incisors, and the occlusal plane was rotated 2.63˚ counterclockwise.
