Article Citation Count: 0A 3-year-old child with multi-drug resistant epilepsy responding to pharmacological and nonpharmacological treatments(Taylor & Francis Ltd, 2023) Coskun, Yesim; Yildirim, CananIntroduction: Despite the development of new antiseizure drugs (ASDs), around one third of epilepsy patients become refractory to treatment or experience adverse events due to ASDs. Therefore, discovery of new ASDs and new therapy options are crucial to improve the quality of life. Herein, we report a 3-year-old child with multi-drug resistant epilepsy caused by perinatal asphyxia whose seizures were reduced by 90% after the introduction of ketogenic diet, vagal nerve stimulation (VNS) AspireSR (SR-seizure response) and oral cannabidiol.Case presentation: A 9-month-old female infant had a history of multidrug resistant epilepsy due to perinatal asphyxia. At admission, she was experiencing up to 20-25 seizures per day lasting for 2-3 minutes. In addition to antiseizure drugs (ASDs), she was put on ketogenic diet (KD), vagal nerve stimulation (VNS) aspire seizure response (AspireSR) was inserted and oral cannabidiol (CBD) was started sequentially. To demonstrate this, we consider a macroscopic problem: splitting a charged oil droplet into two identical smaller droplets in the rest frame K of the original droplet, and in another inertial frame K' moving with respect to K with some constant velocity, and arrive at a physically senseless solution in the framework of the standard approach. To solve this problem in a correct way, we suggest a modified Poynting theorem for an isolated charge and recalculate the momentum of the electromagnetic field of this charge, which yields the usual relativistic relationship between the field energy and momentum. Our approach is not at odds with the idea about "Poincare stresses"; however, in contrast to the standard method, it provides a usual relationship between the "Poincare stress-energy" and the associated momentum stress component. This current approach to chronic low back pain caused by facet joint pathology is usually performed with ultrasound guidance. Objectives: The accuracy of anatomical placement of the facet joint with ultrasonography guidance is determined by C-arm fluoroscopy image taken as reference. Patients and Methods: A total of 22 patients who were diagnosed with facet joint syndrome were involved in the study. After detecting the superolateral corner of the facet joint, which is the target point with ultrasound, the control was provided with C-arm fluoroscopy by giving radiopaque fluid. In order to verify the localization, a mixture of 40 mg triamcinolone and 1 cc 2% lidocaine was injected. Results: Nerve blockage was applied to 67 facet joints at L3 - L4, L4 - L5 and L5 - Si level in a total of 22 patients (15 female and seven male) diagnosed with facet joint syndrome. The patients' mean age was 63 (range, 48 - 80), the mean body mass index was 28.4 (range,18.9 -38.1) and the mean time to determine facet localization with ultrasonography (USG) was 240 seconds (range,140 -320). Patients' mean visual analog scale (VAS) decreased from 7 (range, 6 - 9) to 2.5 (range, 1- 6). In the C-arm fluoroscopy control after the injection of radio-opaque material, the needle was found to be located in the lamina in four segments and it was relocated. In addition to this, two facet joints were not localized in ultrasound. This study concluded that the location of the facet joint with USG guidance is possible with 91% sensitivity and 100% positive predictive value when C-arm fluoroscopy was regarded as a gold standard in determining facet joint localization. No complications were observed. Conclusion: The results showed that ultrasonography guided facet joint block can be considered as a minimally invasive procedure that could be easily applied without radiation exposure.Review Citation Count: 0Acute spinal epidural hematoma: A case report and review of the literature(Turkish Assoc Trauma Emergency Surgery, 2020) Akar, Ezgi; Ogrenci, Ahmet; Koban, Orkun; Yilmaz, Mesut; Dalbayrak, SedatSpinal epidural hematoma (SEH) is a rare but a significant cause of spinal cord compression and neurologic deficits. Its etiology is usually unknown and requires emergency intervention. The present study aims to review the clinical significance, treatment strategies and clinical outcomes of traumatic SEH with a rare case presentation. Our patient was a 42-year-old female who presented with back pain and loss of sensation and strength in the legs. The patient did not have any disease and did not use anticoagulant drugs. The patient developed numbness in her legs half an hour after having a traditional back walking massage due to occasional back pain. She was paraplegic and anesthetic when seen in our clinic. Thoracic computed tomography (CT) and magnetic resonance imaging (MRI) revealed posterior epidural hemorrhage at Th3-Th4 levels. In the 12th hour, the hematoma was evacuated by an emergency decompressive hemilaminectomy. At the postoperative 24th hour, the patient had symptomatic improvement, and in the sixth month, the patient was mobilized with support. SEH is a rare condition that should be considered in patients with sudden onset of back pain and extremity weakness. Although the gold standard diagnostic tool is MRI, CT is often sufficient to avoid delayed surgery. Immediate surgical decompression (laminectomy/hemilaminectomy) should be performed in cases diagnosed with SEH with neurological deficits.Article Citation Count: 101Adaptation, validity, and reliability of the metacognition questionnaire-30 for the Turkish population, and its relationship to anxiety and obsessive-compulsive symptoms(Turkiye Sinir ve Ruh Sagligi dernegi, 2008) Tosun, Ahmet; Irak, MetehanObjective: The present study aimed to evaluate the psychometric properties of the short form of the Metacognition Questionnaire (MCQ-30) in a population of Turkish university students. The metacognitions questionnaire (MCQ) measures individual differences in a selection of metacognitive beliefs, judgments, and monitoring tendencies considered important in the metacognitive model of psychological disorders. Method: The study included 850 university students from 15 Turkish universities. Mean age age of the participants was 21.22 years, 282 (33.17%) were female, and 558 (66.82%) were male. Results: Construct validity was evaluated by exploratory and confirmatory factor analysis (CIA). Exploratory factor analysis revealed that the Turkish version of MCQ-30 has 5 components, which is some factor structure as the original form. In addition, the fit indices of CIA suggested an acceptable fit to a 5-factor model consistent with the original MCQ-30. The Turkish MCQ-30 showed acceptable to good test-retest reliability, internal consistency, and convergent validity. Significant positive relationships between the subscales of MCQ-30 and measures of anxiety and obsessive-compulsive symptoms provided further support for the convergent validity of the Turkish version. Moreover significant negative correlations were observed between age and the MCQ-30 subscales, and the effect of gender was significant on some of the subscales. The aim of this study was to assess adherence to guideline-directed medical and device therapy for the treatment of patients with chronic HFrEF (left ventricular ejection fraction <= 40%). Methods: The Adherence to guideline-directed medical and device Therapy in outpAtients with HFrEF (ATA) study is a prospective, multicenter, observational study conducted in 24 centers from January 2019 to June 2019. Results: The study included 1462 outpatients (male: 70.1%, mean age: 67 +/- 11 years, mean LVEF: 30%+/- 6%) with chronic HFrEF. Renin-angiotensin system (RAS) inhibitors, beta-blockers, mineralocorticoid receptor antagonists (MRAs), and ivabradin were used in 78.2%, 90.2%, 55.4%, and 12.1% of patients, respectively. The proportion of patients receiving target doses of medical treatments was 24.6% for RAS inhibitors, 9.9% for beta-blockers, and 10.5% for MRAs. Among patients who met the criteria for implantable cardioverter-defibrillator (ICD) and cardiac resynchronization therapy (CRT), only 16.9% of patients received an ICD (167 of 983) and 34% (95 of 279) of patients underwent CRT (95 of 279). Conclusion: The ATA study shows that most HFrEF outpatients receive RAS inhibitors and beta-blockers but not MRAs or ivabradin when the medical reasons for nonuse, such as drug intolerance or contraindications, are taken into account. Method and materials: A total of 49 patients with celiac disease aged 6 to 13 years, separated into two groups as previously and newly diagnosed, and a control group of 32 healthy individuals were evaluated. In previously and newly diagnosed patients with celiac disease, body mass index Z-scores were calculated, calcium, alkaline phosphatase, vitamin D3, and parathormone levels were measured, and bone mineral density Z-scores were obtained from dual energy x-ray absorpti-ometry. In all patients, the fractal dimensions of the right and left temporomandibular condyles were evaluated with the fractal analysis method on panoramic radiographs. Results: The mean values of serum biomarker levels and the body mass index and bone mineral density Z-scores for both celiac groups were within the normal reference range. No statistically significant difference was determined between right and left condyle fractal dimen-sions values in the three groups examined. In terms of both right and left condyle fractal dimensions values, there was a statis-tically significant difference between groups. The highest fractal dimensions values were determined in the previously diagnosed group. Conclusions: Differences in fractal dimensions values were observed among patients with celiac disease following the glu-ten-free diet. Utilizing fractal analysis on panoramic radiographs can prove valuable for dental practitioners in evaluating bone min-eral density due to its cost-effect iveness, easy accessibility, and reduced radiation exposure for patients, enabling them to provide comprehensive oral health care and potential early interventions for patients with celiac disease. Üniversite öğrencisi olan 120 katılımcı üzerinde psikolojik dışlanma değişimlemesi sanal top oyunu yoluyla gerçekleştirilmiş ve katılımcıların duygusal tepkileri, ait olma, benlik değeri, kontrol ve anlamlı var oluş ihtiyaçlarının tatmin düzeyi ile kendilerini dışlayan grup üyeleri hakkındaki izlenimleri incelenmiştir. Bulgular, hem önceden haberli hem de habersiz olarak psikolojik dışlanmaya maruz kalanların olumlu duygularında, dört temel ihtiyacın tatmin düzeyinde ve kendilerini dışlayanlar hakkındaki olumlu izlenimlerinde psikolojik dışlanmaya maruz kalmayanlara kıyasla azalma olduğunu göstermiştir. Öte yandan, ait olma düzeyi bir bireysel ayrılık değişkeni olarak ele alındığında, psikolojik dışlanmaya gösterilen tepkilerde bir farklılaşma görülmemiştir. Bu bulgular Williams'ın, (2001; 2007; 2009) modelinde öne sürmüş olduğu, psikolojik dışlanmaya gösterilen anlık tepkilerin durumsal ve bireysel ayrılıklar değişkenlerinden etkilenmeyeceğine ilişkin görüşünü desteklemektedir. Bununla birlikte, önceden haberdar olanların benlik değeri kontrol grubundan daha düşük, ancak önceden haberdar olmayanlara kıyasla daha yüksek bulunmuştur. Different crosslinkers affect crosslinking affinity, which has a significant influence on microcapsule properties. The objective was to prepare in vitro microcapsules using calcium, barium, iron, manganese, nickel, and strontium as divalent cations to observe their potential for use in cell transplantation. Sodium alginate was added dropwise to the individually prepared crosslinkers to observe diffusion-based gelling. Alginate microcapsules were investigated regarding capsule stability, physiological properties, and cell viability. After 30 days of incubation, cell viability was greater than 90% for the cell-encapsulated microcapsules when crosslinked with CaCl2 and NiCl2. Viability decreased in the following order: CaCl2 > NiCl2 > BaCl2 > SrCl2 > MnCl2 > FeCl2. A compression test was performed to investigate the required force to deform 30% of microcapsules, and only MnCl2, FeCl2 (180mM), and NiCl2 (50mM) demonstrated higher resistance to the applied force than CaCl2. Except for the FeCl2 group, all cell-encapsulated microcapsules remained intact for 45 days. Potential sensitivities to CaCl2 during cell transplantation may compel alternative crosslinker usage, and our study revealed that NiCl2 and BaCl2 can be used as alternative crosslinkers to CaCl2 due to their high cell viability and consistent stability.Article Citation Count: 1Algısal çelişki ve tepki rekabeti: Stroop etkisine ilişkin olay-ilişkili potansiyeller(2009) Bekçi, Belma; Karakaş, SirelAmaç: Mevcut çalışmanın amacı, algısal çelişki ve tepki rekabeti hipotezleri ile açıklanmaya çalışılan Stroop etkisinin yol açtığı elektrofizyolojik aktiviteleri analiz etmektir.Yöntem: Örneklem üniversite popülasyonundan 50 (23 kadın 27 erkek) sağlıklı gönüllü yetişkini içermiştir. Uyarım, kayıt ve analiz işlemlerinde NeuroScan 4.2 donanım-yazılım sistemi kullanılmıştır.Bulgular: Stroop bozucu etkisi, davranışsal olarak, uyuşmayan uyarıcılara ilişkin tepki zamanının uzaması ve tepki verilmekte gecikilen uyarıcı sayısının artması olarak elde edilmiştir. Temel bileşenler analizinde (TBA) Stroop performansının seçici dikkat, bozucu etki ve bozucu etkiye direnç faktörlerini içerdiği belirlenmiştir. Stroop bozucu etkisi, elektrofizyolojik olarak ise uyuşmayan uyarıcılara ilişkin P3 ve N4, yanlış tepkilere ilişkin olarak da N2, P3, N3 ve N4 bileşenlerinin genliklerinde artış olarak elde edilmiştir.Sonuç: P3 ve N4 zirvelerine ilişkin genlik artışının çelişki belirleme sürecini yansıttığı sonucuna varılmıştır. Yanlış tepkilere ilişkin genlik artışı değişiklikleri ise karmaşıktır. N2 ve P3 bileşenlerindeki genlik artışı yalnızca uyuşmayan uyarıcılarda, N3 ve N4 bileşenlerinin genliği ise hem uyuşmayan hem de uyuşan uyarıcılarda artmıştır. Söz konusu daha erken ve geç aktivitelerin sırasıyla tepki rekabeti ve hata belirleme süreçleri ile ilişkili olduğu sonucuna varılmıştır. Our relativistic reformulation (which includes the energy conservation) constitutes a new example for the breakdown of the Lorentz invariance. Briefly speaking, even at rest or during the whole scattering process, the distance between the alpha particle and the gold nucleus is not invariant but depends on the frame of the observer attached to either object. Mean follow-up period was 32.2 (range, 24e64) months. Diagnosis of thepatients included congenital kyphosis in 11 patients, post-tuberculosis kyphosis in 3 patients andneuroŞbromatosis in 3 patients. The sagittal plane parameters (local kyphosis angle, lumbar lordosis,sagittal vertical axis, pelvic tilt, sacral slope and pelvic incidence) were measured in the preoperative andthe early postoperative periods and during the last follow-up on the lateral radiographs.Results: The mean preoperative localized kyphosis angle was 121.8(range, 101e149). The mean localkyphosis angle (LKA) was 71.5at postoperatively evaluation (p< 0.05). Complications were detected in12 patients (70.6%) with spinal shock in 4 patients, hemothorax in 3 patients, postoperative infection in 2patients, dural laceration in 2 patients, neurological deŞcit in 2 patients (1 paraplegia and 1 root injury),the shifted cage in 2 patients and rod fracture in 2 patients. Neurological events occurred in six patients(35%) with temporary neurological deŞcit in 5 patients and permanent neurological deŞcit in 1 patient.Conclusion: PVCR is an efŞcient and a successful technique for the correction of SAK. Publishing services by Elsevier B.V.Article Citation Count: 0Analysis of morphological parameters in pelvic radiography and hip MRI: A practical reporting recommendation(ARSMB-KVBMG, 2021) Cicek,E.D.; Cevik,H.Although numerous criteria have been proposed to define abnormal hip morphology, mostly used in the diagnosis of femoroacetabular impingement, it is not a practical approach to measure all of these parameters in all cases without clinical suspicion. In this study, our aim was to develop an evaluating and reporting standardization for routine hip examinations to define both hip morphology and impingement. A total of 108 patients with routine hip magnetic resonance imaging (MRI) and antero-posterior pelvic radiograph (PR) were included in this retrospective study. Alpha angle (AA), acetabular depth (AD), acetabular protrusion, acetabular anteversion, collodiaphyseal angle (CDA), lateral center-edge angle (LCEA) and Tönnis angle (TA) were measured. The differences and associations between these parameters were evaluated according to imaging modality or plane, and sex. Although a significant difference has been found between the axial AA and the coronal AA mean values measured on MRI, there was also a strong correlation. Coronal measurements were significantly higher. AA values measured in PR and coronal MRI were comparable. Males had higher AA in both planes as compared to females. There were no significant differences between CDA values in MRI and PR. There was a significant difference and a moderate correlation between AD values in MRI and PR. We suggest that routine reports should include a measurement of AA in two planes, and measurement of CDA in PR or MRI. Due to the difference in AD between MRI and radiography, LCEA or TA may represent better alternatives. For this reason, it is important to reduce the risk of pedestrian accidents to increase the safety of highway traffic. The majority of accidents involving pedestrians occur during street crossings where motor vehicles are present. Hence, pedestrians are encouraged to cross safely at crosswalks that are either controlled by a signal or not. Accidents involving pedestrians can be reduced by analysing pedestrian crossing behaviours and designing crosswalks accordingly. Crosswalk design as well as traffic signal phasing need to give enough time for pedestrians to complete their crossing during the green signal phase. For this reason, pedestrian crossing speed and factors affecting this have been analysed in this study. Several conditions for the uniform separability and Fredholmness in Banach-valued L (p) -spaces are given. Sharp uniform estimates for the resolvent are established. It follows from these estimates that the indicated operator is uniformly positive. Moreover, it is also the generator of an analytic semigroup. Çalışmada örtük ve açık bellek ölçümü için aynı KKT görevi kullanılırken; örtük ve açık bellek, farklı yönergeler kullanılarak ortaya çıkarılmıştır.Bulgular: Varyans analizleri ile yaş, yönerge türü, test edilme süresi ve somutluk düzeyi değişkenlerinin KKT puanı üzerindeki temel ve ortak etkileri incelenmiştir. The root canals (n = 20 in each group) were irrigated with NaOCl 2.5%, and the extruded debris was collected at pre-weighted Eppendorf tubes. The amount of apically extruded debris (mg) and the treatment time (s) were recorded. Tukey pairwise comparisons were performed to compare groups with a 95% confidence level. VR had significantly less debris extrusion values and treatment time than RE and TRN (P < 0.05). Although no difference was observed between RE and TRN in terms of apically extruded debris (P > 0.05), a significant difference in favour of RE was observed in terms of treatment time (P < 0.05). All the instruments extruded debris at varying weights.
