Sağlık yöneticilerinde stratejik planlama bilinç düzeyi ve stratejik yönetimde karşılaşılan sorunlar; Samsun ili örneği
Abstract
Bu çalışma Samsun ilindeki sağlık yöneticilerinde stratejik planlama bilinç düzeyi ve stratejik planlamada karşılaşılan sorunları belirlemek amacıyla tanımlayıcı ve kesitsel olarak planlanmış ve yapılmıştır. Araştırma Haziran – Kasım 2014 tarihleri arasında Samsun İl Sağlık Müdürlüğüne bağlı 18 devlet hastanesi, 1 ağız ve diş sağlığı hastanesi ve 2 ağız ve diş sağlığı merkezinde yürütülmüş olup, araştırmanın evrenini 111 sağlık yöneticisi oluşturmuştur. Hedef kitlenin % 77'sine (N=85) ulaşılmıştır. Verilerin istatistiksel analizinde, bilgisayarda Statistical Package For Social Sciences (SPSS) 17.0 paket programı kullanılmış olup, Tek Yönlü Varyans Analizi (ANOVA) ve t testleri uygulanarak, p<0.05 düzeyi anlamlı olarak kabul edilmiştir. Sağlık yöneticilerinin yaş ortalaması 42.14±6.11 (36-48 yaş aralığı)'dir. Sağlık yöneticilerinin şuan ki pozisyondaki görev sürelerine, cinsiyetlere, eğitim düzeylerine, yönetim eğitimi almalarına göre stratejik planlama bilinç düzeylerini değerlendirdiğimizde, gruplar arasındaki fark istatistiksel açıdan anlamlı bulunmuştur. Yönetim üzerine eğitim alan erkek sağlık yöneticilerin stratejik planlamayla ilgili, kanun ve bilgi düzeyleriyle stratejik planlamaya destek verdikleri, stratejik yönetimin kurumlar için gerekli olduğunu ve buna inandıklarını, stratejik planlama uygulama sürecinde ise etkin rol aldıkları belirlenmiştir. Sağlık yöneticilerinin yaşlara, cinsiyetlere ve toplam hizmet sürelerine göre stratejik yönetim uygulamada karşılaştıkları sorunlarını değerlendirdiğimizde, gruplar arasındaki fark istatistiksel açıdan anlamlı bulunmuştur. 40 yaş üzerinde olan sağlık yöneticilerinin, sağlık personelinden kaynaklanan sorunların sağlık kurumlarında stratejik yönetim uygulamasını engelleyeceğini ifade ettikleri belirlenmiştir. Kapasitesi yüksek olan sağlık kuruluşlarının yöneticilerinin yüksek lisans düzeyinde 'sağlık yönetimi' eğitimi almaları koşulu Sağlık Bakanlığınca zorunlu hale getirilebilir. Kapasitesi düşük olan sağlık kuruluşlarının yöneticilerinin ise stratejik yönetim üzerine eğitim almaları sağlanabilir. This study was planned and done as definitive and cross-sectional with the aim to determine the level of conscience of strategic planning in health managers in Samsun province and the problems encountered in strategic planning. The research was conducted in 18 state hospitals, 1 oral and dental health hospital and 2 oral and dental health centers of Samsun Provincial Directorate of Health between January – November 2014 and 111 health managers constituted the research population. 77% of the target group (N=85) were accesed. In the statistical analysis of the data, the package program of Statistical Package for Social Sciences (SPSS) 17.0 was used in the computer, and a P<0.05 level was considered as significant by applying Single-Way Variant Analysis (ANOVA) and t tests. The average age of health managers is 42.14±6.11 (age range 36-48). The difference between the groups was found statistically significant when we evaluated the level of conscience of strategic planning of health managers according to the duration of duty in their current position, gender, education levels, and their receiving management education. It was determined that male health managers who were trained on management, support strategic planning with the laws related to the strategic planning and with their level of knowledge, and that they deem strategic management as necessary for the institutions and they believe in it and that they have taken an active role in the implementation process of strategic planning. When we evaluated the problems that health care managers faced in implementing strategic management by age, gender and total duration of service, the difference between the groups was statistically found significant. It was determined that health care managers over 40 stated that the problems arising from the health personnel will prevent the application of strategic management in health care institutions. The condition of getting 'health management' education at the master's level for managers of health organizations with high capacity may be made compulsory by The Ministry of Health.
