Abstract
Çalışma, servikal diskopatiye bağlı boyun ağrısı olan hastalar da alet yardımlı yumuşak doku mobilizasyonu tekniğinin (IASTM) etkinliğini araştırmak amacıyla yapıldı. Çalışmaya, servikal diskopati tanısı almış 24 hasta dahil edildi. Hastalar basit randomizasyon yöntemiyle randomize edildi. Hastalar Grup 1 (konvansiyonel fizyoterapi) ve Grup 2 (konvansiyonel fizyoterapi ve IASTM) olmak üzere iki gruba ayrıldı. Tedavi öncesi ve tedavi sonrasında (dördüncü haftanın bitiminde) tüm hastaların ağrı şiddeti 'Vizüel Analog Skala' ile fonksiyonel durumu 'Kopenhag Boyun Fonksiyonel Özürlülük Skalası' ile özürlülük durumu 'Boyun Özürlülük İndeksi' ile anksiyete ve depresyon düzeyi 'Hastane Anksiyete ve Depresyon Skalası' ile değerlendirildi. Her iki gruba 4 hafta boyunca toplam 20 seans yüzeyel sıcak uygulama (Hotpack), Transkutanöz Elektriksel Sinir Stimulasyonu (TENS), Ultrason (US), boyun, omuz kuşağı ve sırt bölgesi egzersizlerini içeren konvansiyonel fizyoterapi programı uygulandı. Grup 2'de bulunan hastalara konvansiyonel fizyoterapi programına ilave olarak IASTM, 4 hafta boyunca, haftada 2 gün, toplam 8 seans uygulandı. Ayrıca tüm hastalara ev egzersiz programı verildi. Çalışma sonucunda; grup içi kıyaslamalarında ağrı şiddeti, fonksiyonellik, özürlülük durumu, anksiyete ve depresyon düzeyinde istatistiksel olarak anlamlı iyileşmeler görüldü (p<0.05). Gruplar arası karşılaştırmada ise ağrı şiddeti, fonksiyonellik ve özürlülük durumunda istatistiksel olarak anlamlı iyileşmeler bulunurken, anksiyete ve depresyon düzeyinde istatistiksel olarak anlamlı fark bulunmadı (p>0.05). Çalışmanın sonucuna göre; servikal diskopatisi olan hastalarda konvansiyonel fizyoterapi programına ek olarak uygulanan IASTM'nin ağrı şiddeti ve özürlülük durumunu azaltmada, fonksiyonelliği arttırmada etkili, anksiyete ve depresyon düzeyinde etkisi olmadığı bulundu. Servikal diskopatili hastaların fizyoterapisinde IASTM, tamamlayıcı ve destekleyici tedavi yöntemi olarak kullanılabilir. The study was made for investigating the effectiveness of instrument-assisted soft tissue mobilization (IASTM) technique in patients who have neck pain associated with cervical discopathy. Twenty-four patients who have diagnosed with cervical discopathy were included in the study. The patients were randomized by using a simple random sampling method. The patients were divided into two groups, Group 1 (conventional physiotherapy) and Group 2 (conventional physiotherapy and IASTM). For all patients' evaluations were made before and after(end of fourth week) their treatment. 'Visual Analogue Scale' was used to asses the pain level, 'Copenhagen Neck Functional Disability Scale' was used to asses functionality, 'Neck Disability Index' was used to asses disability level, 'Hospital Anxiety and Depression Scale ' was used to asses anxiety and depression level. Conventional physiotherapy program including 20 sessions of superficial hot application (Hotpack), Transcutaneous Electrical Nerve Stimulation (TENS), Ultrasound (US), neck, shoulder girdle and back region exercises were applied to both groups for 4 weeks. In addition to the conventional physiotherapy program, IASTM was administered to patients in Group 2 for a total of 8 sessions for 4 weeks, 2 days a week. In addition, all patients were given a home exercise program. As a result of the study; significant improvements in pain level, functionality, disability, anxiety and depression level were observed in-group comparisons (p <0.05). Significant improvements in pain level, functionality, disability level were found while significant improvement in anxiety- depression level was not found in the comparison between groups (p> 0.05). According to the result of the study; conventional physiotherapy combined with IASTM program has been shown to be effective in reducing pain and disability level, increasing functionality and not effective in reducing anxiety and depression level. IASTM program could be use as a complementary and supportive treatment method in physiotherapy of patients with who have cervical discopathy.
