Emziren annelerin kontraseptif kullanma niyetleri ve etkileyen faktörler

Abstract

Bu araştırma, 0-6 aylık bebeği olup bebeğini sadece anne sütü veren annelerin kontraseptif kullanma niyetlerini belirlemek amacıyla yapılmıştır. Tanımlayıcı ve kesitsel tipteki araştırma, Mart-Temmuz 2021 tarihleri arasında, İstanbul ili Avrupa yakasında hizmet veren bir aile sağlığı merkezine başvuran 165 anne ile gerçekleştirilmiştir. Veriler, tanıtıcı soru formu ve Kontraseptif Niyet Ölçeği ile elde edilmiştir. Verilerin değerlendirilmesinde, sayı yüzdelik hesaplamaları, ortalama, standart sapma, t testi, Anova testi ve Kruskal Wallis testleri kullanılmıştır. Annelerin yaş ortalaması 29,95±5,51 (min:18, maks:41) yaştır. Annelerin %43'ünün üç ve üzeri gebeliği olduğu, %80'inin gebeliğinin planlı olduğu, %43'ünün gebelikleri arasındaki sürenin 2 yıldan fazla olduğu ve %33,9'unun tekrar gebe kalmayı düşündüğü saptanmıştır. Annelerin %45,5'inin bebeğinin 1-3 aylık olduğu ve %97,6'sının doğumdan 41 gün ve sonra ilk cinsel birliktelik yaşadığı belirlenmiştir. Annelerin %41,2'sinin gebelik öncesinde modern yöntemleri tercih ettiği, %18,2'sinin şu anda herhangi bir yöntem kullanmadığı belirlenmiştir. Yöntem kullanmayanların %56,7'si kendisi istemediği için yöntem kullanmadığını, %60'ı aile planlaması danışmanlığı eğitimi almadığını, %68,5'i yöntem seçiminde eşi ile ortak karar verdiğini ve %24,8'i bebeğini emziriyor olmasının yöntem seçimindeki kararını etkilediği ifade etmiştir. Kontraseptif Niyet Ölçeği puan ortalaması 8,75±4,27 olarak bulunmuş ve kontraseptif kullanım niyetlerinin orta düzeyde olduğu şeklinde yorumlanmıştır. Annelerden, 1 yıldan az süredir evli olanların, geliri giderden az olanların kontraseptif kullanma niyetinin daha düşük olduğu; üç ve üzeri gebeliği olanların, gebeliği plansız olanların, önceki gebeliğini üzerinden 2 yıldan fazla süre geçenlerin ve tekrar gebe kalmayı düşünmeyenlerin kontraseptif kullanma niyetinin daha yüksek olduğu saptanmıştır. Annelerden gebelik öncesinde modern yöntem kullananların, şu anda aile planlaması yöntemi kullananların ve aile planlaması danışmanlığı almayanların kontraseptif kullanma niyetinin daha yüksek olduğu; aile planlaması yöntemini eşi seçen ve emzirmenin yöntem seçimindeki kararını etkilediğini belirtenlerin kontraseptif kullanma niyetinin daha düşük olduğu saptanmıştır. Emzirme döneminde annelerin kontraseptif kullanımı niyetinin belirlenmesi ve bu doğrultuda yöntem kullanımı konusunda annelerin bilinçlendirilmesi önerilmektedir. This study was carried out to determine the contraceptive use intentions of mothers with 0-6 months old infants who exclusively breastfeed. The descriptive and cross-sectional study was conducted with 165 mothers who applied to a family health center serving on the European side of Istanbul between March and July 2021. The data were obtained with the descriptive questionnaire and the Contraceptive Intention Scale. In the evaluation of the data, number percentage calculations, mean, standard deviation, t test, Anova test and Kruskal Wallis tests were used. Average age of mothers Age is 29.95±5.51 (min:18, max:41). It was determined that 43% of the mothers had three or more pregnancies, 80% had a planned pregnancy, 43% had more than 2 years between their pregnancies, and 33.9% thought of getting pregnant again. It was determined that 45.5% of the mothers had their babies 1-3 months old and 97.6% had their first sexual intercourse 41 days and after birth. It was determined that 41.2% of the mothers preferred modern methods before pregnancy, and 18.2% did not use any method at the moment. Of those who do not use a method, 56,7% do not use the method because they do not want it, 60% do not receive family planning counseling training, 68.5% decide on the method together with their spouse, and 24.8% are breastfeeding their baby. stated to be affected. Contraceptive Intention Scale mean score was 8.75±4,27 it was found to be and interpreted as moderate contraceptive use intentions. Among the mothers, those who have been married for less than 1 year and whose income is less than expenditure have a lower intention to use contraceptives; it has been determined that the intention to use contraceptives is higher in those who have three or more pregnancies, those with an unplanned pregnancy, those who have passed their previous pregnancy for more than 2 years, and those who do not plan to become pregnant again. Among the mothers, those who used modern methods before pregnancy, those who currently use family planning methods and those who do not receive family planning counseling have a higher intention to use contraceptives; it was determined that the intention to use contraceptives was lower for those whose partner chose the family planning method and stated that breastfeeding affected their decision in choosing the method. It is recommended to determine the intention of mothers to use contraceptives during breastfeeding and to raise awareness of mothers about the use of methods in this direction.
