Abstract
Bölgesel Anestezi İle Sezaryen Ameliyatı Olacak Hastalarda Ameliyat Öncesi Kaygı Düzeyinin Ameliyat Sırası Bulantı Kusmaya Etkisinin İncelenmesi Bu araştırma, bölgesel anestezi ile sezaryen ameliyatı olacak hastalarda ameliyat öncesi kaygı düzeyinin ameliyat sırası bulantı kusmaya etkisinin incelenmesi amacıyla yapılmıştır. Tanımlayıcı nitelikteki araştırmanın evrenini Sakarya ilinde bir üniversite hastanesinde bölgesel anestezi ile sezaryen ameliyatı yapılacak hastalar, örneklemini ise 18.07.2021-14.03.2022 tarihleri arasında bölgesel anestezi ile sezaryen ameliyatı yapılan 245 hasta oluşturmuştur. Araştırma verilerinde; tanımlayıcı veri toplama formu, Durumluk ve Sürekli Kaygı Ölçeği ve Ameliyat Sırası Bulantı Kusma Değerlendirme Formu kullanılmıştır. Verilerin değerlendirilmesi IBM SPSS 22 versiyon kullanılarak yapılmış olup; kategorik değişkenler için frekans dağılımı (sayı, yüzde), sayısal değişkenler için tanımlayıcı istatistikler (ortalama, standart sapma, medyan, minimum, maksimum), iki grup için Mann Whitney U testi, ikiden fazla grup için Kruskal Wallis testi, iki sayısal değişken arasındaki ilişkinin incelenmesinde Spearman korelasyon testi ve ölçeğin güvenilirliği için Cronbach's alfa kat sayısı kullanılmıştır. Hastaların yaş ortalamasının 29,29±5,28 yıl, %40,8'inin ilköğretim mezunu, %99,2'sinin evli, %5,7'sinin kronik hastalığı olduğu ve %18,4'ünün sigara kullandığı bulunmuştur. Araştırma kapsamındaki hastaların gebelik haftası ortalamasının 38,11±1,99 hafta olduğu, %35,5'inin 2 çocuğu olduğu, %75,5'inin daha önce hiç normal doğum gerçekleştirmediği, %44,1'inin bir kez sezeryan ile doğum yaptığı, %58,4'ünün kombine anestezi ile ameliyat olduğu ve %50,6'sının elektif olarak sezaryen ameliyatı olduğu belirlenmiştir. Hastaların ameliyat öncesi Durumluk Kaygı Ölçeği puanı 45,38±14,94 ve Sürekli Kaygı Ölçeği puanı 43,04±14,31'dir. Araştırmadaki hastaların %34,7'sinde ameliyat sırasında bulantısı bulunmakta olup puan ortalamasının 6,93±1,48, %25,2'sinde kusma yaşandığı bulunmuştur. Araştırmaya katılmaya gönüllü hastaların bulantı ve kusma yaşama durumlarına göre Durumluk Kaygı Ölçeği puanı, Sürekli Kaygı Ölçeği puanı arasında istatistiksel olarak anlamlı derecede bir farklılık bulunmaktadır (p<0,05). Buna göre, bulantı ve kusma olan hastaların durumluk kaygı ve sürekli kaygı puanı bulantı ve kusma olmayanlara göre daha yüksektir. Bu araştırma sonucunda hastaların ameliyat öncesi dönemde orta düzeyde kaygı yaşadığı, bulantı kusma varlığı olan hastaların kaygılarının daha yüksek olduğu bulundu. INVESTIGATION OF THE EFFECT OF PRE-SURGERY ANXIETY LEVEL ON POSITIONAL NAUSEA VOMITING IN PATIENTS WHICH WILL HAVE TO HAVE CESEREARE SURGERY WITH REGIONAL ANESTHESIA This study was conducted to investigate the effect of preoperative anxiety level on intraoperative nausea and vomiting in patients who will undergo cesarean section under regional anesthesia. The population of the descriptive study consisted of patients who would undergo cesarean section under regional anesthesia in a university hospital in Sakarya, and the sample consisted of 245 patients who underwent cesarean section under regional anesthesia between 18.07.2021 and 14.03.2022. In research data; descriptive data collection form, State and Trait Anxiety Scale and Intraoperative Nausea and Vomiting Evaluation Form were used. Evaluation of the data was made using IBM SPSS version 22; Frequency distribution for categorical variables (number, percentage), descriptive statistics for numerical variables (mean, standard deviation, median, minimum, maximum), Mann Whitney U test for two groups, Kruskal Wallis test for more than two groups, correlation between two numerical variables. Spearman correlation test and Cronbach's alpha coefficient were used for their reliability. The mean age of the patients was 29.29±5.28 years, 40.8% were primary school graduates, 99.2% were married, 5.7% had a chronic disease and 18.4% were smokers. The mean gestational week of the patients included in the study was 38.11±1.99 weeks, 35.5% had 2 children, 75.5% had never had a normal delivery before, 44.1% had a cesarean delivery once. It was determined that 58.4% of them had surgery with combined anesthesia and 50.6% of them had elective cesarean section. The preoperative State Anxiety Scale score of the patients was 45.38±14.94 and the Trait Anxiety Scale score was 43.04±14.31. 34.7% of the patients in the study had nausea during the operation, and the mean score was 6.93±1.48, and 25.2% of them experienced vomiting. There was a statistically significant difference between the State Anxiety Inventory score and the Trait Anxiety Inventory score according to the nausea and vomiting of the patients who volunteered to participate in the study (p<0.05). Accordingly, the state anxiety and trait anxiety scores of patients with nausea and vomiting were higher than those without nausea and vomiting. As a result of this study, it was found that patients had moderate anxiety in the preoperative period, and patients with nausea and vomiting had higher anxiety.
