Kamu ve özel hastane çalışanlarının sağlık turizmine ilişkin algılarının değerlendirilmesi

Abstract
Sağlık turizmi ülke ekonomisine ve refahına katkı sağlayabilecek önemli bir hizmet ihracıdır. Son yıllarda Türkiye'de de gelişme gösteren sağlık turizmi gerek kamu gerekse özel hastaneler için giderek daha da önemli bir gelir kaynağı ve hizmet alanı olmaktadır. Bu çalışmada sağlık turizmi hizmeti veren kamu ve özel hastane çalışanlarının algı yaklaşım ve düşüncelerinin incelenmesi amaçlanmıştır. Araştırma evrenini sağlık turizminde hizmet veren iki özel ve kamu hastaneleri birliklerine bağlı iki kamu hastanesinde çalışan toplam 1643 doktor ve hemşireden oluşmaktadır. %95 güven aralığı ile örneklem büyüklüğü 312 olarak belirlenmiştir. Çalışanlara ve hastane yöneticilerine yönelik olarak hazırlanan anketlerden elde edilen veriler SPSS 20 paket programına aktarılarak analiz edilmiştir. Betimleyici istatistikler verildikten sonra karşılaştırmalı analiz için Pearson Chi-Square ve Fisher's Exact Testi kullanılmıştır. Katılımcı doktor ve hemşirelerin algı ve düşüncelerine göre hizmet sunumundaki en önemli sorunlar; yabancı dil bilen personelin azlığı ve buna bağlı olarak yabancı hasta ve hasta yakınları ile yaşanan iletişim problemleri, hastanelerinin fiziki alt yapıları (hasta odaları ve bekleme odaları vb.), teknolojik donanımları ve otelcilik hizmetlerinin yetersiz olması olarak sıralanmaktadır. Bu hastanelerin yöneticileri de sorunları benzer olarak tanımlamaktadır. Sağlık turizmi ile ilgili verilen kurum içi eğitim alma durumu ile çalışılan hastane türü arasında anlamlı bir ilişki bulunmamıştır. Ancak iletişim teknikleri, yabancı dil, hasta hakları vb konular da hizmet içi eğitim alan özel hastane çalışanlarından personelin yabancı hastaların tedavi süreçlerinde hasta ve hasta yakınlarıyla iletişimde sorun yaşamadıkları görülürken; kamu hastanesi personelinin daha fazla sorun yaşadıkları tespit edilmiştir. Sağlık turizmi hizmeti veren personelin yabancı dil eğitiminin artırılmasının yaşanan iletişim sorununu azaltacağı düşünülmektedir. İstanbul'da sadece sağlık turizmi hizmeti verecek bir kamu hastanesinin fizibilite araştırmasının yapılması bir öneri olarak ortaya çıkmaktadır.
The health tourism is a significant service export for the national economy and also could contribute to the prosperity. The health tourism improvement in Turkey in recent years is becoming even more important an income source and service area for either the public or the private hospitals. In this study has aimed to investigation the perception, the approach and the thinking of employees' which provide the health tourism in public and private hospitals. The population of the investigation has consisted of over-all 1643 doctor and nurse who provide the health tourism in two private hospitals and two public hospitals which affiliated with Public Hospitals Association. The confidence interval has been %95 and the sample size has been 312 determined. The obtained data from the prepared questionnaires as for employees and hospital managers have been analyzed by SPSS 20 software. After given the descriptive statistics the Pearson Chi-Square and the Fisher's Exact Tests have been used for the comparative analysis. According to the participant doctor and nurses perception and thinking the most significant problems in service providing are aligned as lack of employees who knows foreign languages and accordingly encountered the communication problems with patient and their relatives, the physical infrastructure of the hospitals – patient rooms and waiting rooms etc.-, inadequately technological equipment and the services of the hotel management. Also the hospital managers have described the problems as similar. There was no occurred significant relation between the situation of taking the in-company training regarding to the health tourism and the type of the worked hospital. Merely it was obtained that the private hospital employees have no encountered communication problems during the treatment of patients and patient's relatives when the employees have taken the in-service training issue like communication skills, foreign language, patient rights etc.; it was detected that the employees of the public hospital encountered much more problems. Contemplated that the encountered communication problem would be reduced when the foreign language education increase of the employees who health tourism service provides. The emerging proposal was a feasibility study for a public hospital just for health tourism service providing in Istanbul.
