Abstract
Bu araştırma, genel anestezi sonrası ameliyat sonrası bakım ünitesinde tedavi gören hastalarda hipoksinin önlenmesi için uygulanan nazal kanül ve basit maske ile oksijen tedavisinin etkinliğinin karşılaştırılması amacıyla yapılmıştır. Tanımlayıcı nitelikteki araştırmanın örneklemini İstanbul ili Avrupa yakasında bir üniversite ahstanesinde 15 Ocak- 19 Mart 2021 tarihleri arasında genel anestezi ile ameliyat olan 301 hasta oluşturmuştur. Araştırma verileri hasta tanıtım formu ve hasta izlem formu ile toplanmıştır. Verilerin analiz edilmesinde tanımlayıcı istatistikler, Kolmogorow-Smirnov, Mann Whitney U Testi, Ki-Kare Homojenlık Testi, Wilcoxon İşaretli Sıra Testi ve Tekrarlayan Ölçümler Varyans Analizi (Repeated Measures ANOVA) ve Tekrarlayan Ölçümlerde Çok Yönlü Varyans Analizi Testi kullanılmıştır. Araştırma kapsamına alınan hastaların %82,1'i kadın olduğu, %56,21'inin kadın hastalıkları nedeniyle ameliyat olduğu, %69,1'inin ek hastalığı olmadığı, %48,2'sinin ameliyat sonrası problemi olmadığı, %32,6' sının ağrısının olduğu bulunmuştur. Araştırmada nazal kanül ve basit maske ile oksijen tedavisi uygulanan hastaların 0., 15., 30., 60. dakikalarda solunum sayısı, nabız sayısı, sistolik ve diastolik kan basıncı değerleri, ağrı puanı ve aldrete puanları oksijen satürasyonu ortalamaları arasında anlamlı farklılık saptanmamıştır (p<0,05). Nazal kanül ve basit maske ile oksijen tedavisi alan hastaların oksijen satürasyon değerleri arasında zaman göre aralarında istatistiksel olarak anlamlı bir fark bulunmuştur (F=4,647, p=0,008). Zamana göre 1. ile 3. ölçümler arasında istatistiksel olarak anlamlı bir fark olduğu belirlenmiştir. Bu araştırma sonucunda ameliyat sonrası dönemdeki hastaların oksijen tedavisinin etkinliğinde nazal kanül ve basit maske ile oksijen tedavisi arasında anlamlı farklılık bulunmadı. This study was conducted to compare the effectiveness of oxygen therapy with nasal cannula and simple mask applied to prevent hypoxia in patients treated in the post anesthesia care unıt after general anesthesia. The sample of the descriptive and experimental study consisted of patients who were operated for various reasons and taken to the recovery room for follow-up at Istanbul University Istanbul Medical Faculty Hospital between January 15 and March 19, 2021, and the sample consisted of 301 patients. Study data were collected with a descriptive data collection form. In the evaluation of the data, descriptive statics, Kolmogorow-Smirnov, Mann Whitney U Test, Chi-Square Homogeneity Test, Wilcoxon Signed Rank Test and Repeated Measures Analysis of Variance (Repeated Measures ANOVA) and Multi-Directional Analysis of Variance in Repeated Measures Test were used. Of the patients included in the study, 82.1% were women, 56.21% had surgery for gynecological diseases, 69.1% had no comorbidities, 48.2% had no postoperative problems, 32.6% It was found that he had pain. In the study, no significant difference was found between the oxygen saturation averages of respiratory rate, pulse rate, systolic and diastolic blood pressure values, pain score and aldrete scores at the 0, 15, 30, and 60 minutes of the patients who received oxygen therapy with a nasal cannula and simple mask (p). <0.05). A statistically significant difference was found between the oxygen saturation values of the patients who received oxygen therapy with nasal cannula and mask, according to time (F=4.647, p=0.008).
