Abstract
Bütün hizmet alanlarında olduğu gibi sağlık hizmetlerinde de teknoloji ve bilim alanlarında oluşan yenilikler çalışan gücünün planlanması ve sorumluluklarının belirlenmesinde iş analizi çalışmalarının yapılmasını önemli hale getirmiştir. Bu çalışma, İç Hastalıkları Servisi'nde çalışan hemşirelerin iş analizini belirlemek amacıyla tanımlayıcı ve analitik olarak yapılmıştır. Çalışmanın verileri 07 Kasım – 21 Kasım 2016 tarihleri arasında İstanbul Avrupa yakasında bulunan bir devlet hastanesinin İç Hastalıkları Servisi'nde görev yapan 8 hemşireden elde edilmiştir. Çalışmada, serviste yürütülen hemşirelik faaliyetleri ve bu faaliyetlere ayrılan sürelere göre hemşire iş yükü belirlenmiştir. DEMATEL Tekniği ile faaliyetlerin birbirine göre önem derecesi ve ANP Ağ Modeli ile önem ağırlıklandırması yapılmıştır. Ayrıca İş Analiz Formu yardımıyla işin yapılmasını etkileyen fiziksel, çevresel koşullar incelenmiş, performans için gerekli yetkinlikler belirlenmiştir. Hemşirelerin fiziksel çalışma koşullarının yönetmeliklerde belirlenen koşulları sağladığı belirlenmiştir. Hemşireler çalışma sürelerinin %52,44'ünü doğrudan hasta bakım aktiviteleri, %13,58'ini dolaylı hasta bakım faaliyetleri, %13,97'sini servis düzeni ve işleyişi ile ilgili faaliyetler, %20'ini bireysel faaliyetler ve %0,01'ini eğitim ve gelişim faaliyetleri için harcamaktadırlar. ANP yöntemi ile yapılan hesaplamalara göre; serviste yürütülen faaliyetler ve önem ağırlıkları incelendiğinde iş yükü arttığında doğrudan hasta bakım faaliyetlerinden yaşam bulgularının takibi ve kayıt edilmesi (%6,98); dolaylı hasta bakım faaliyetlerinden doktor vizitine eşlik edilmesi (%7,65); servis düzeni ve işleyişi ile ilgili faaliyetlerden hasta başı kullanılan ilaçların sistemden girişlerinin yapılması (%4,42); bireysel fizyolojik gereksinimlere yönelik faaliyetlerinden yemek molası (%8,07) ve eğitim - gelişim faaliyetlerinden ise hizmetiçi eğitim (%0.01) faaliyetlerinin etkileneceği belirlenmiştir. Sonuç olarak; Bakımın kalitesini artırmak için tüm servislerde çalışan hemşirelerin iş analizlerinin yapılarak iş performansını etkileyen koşulların belirlenmesi, etkin ve güvenli hasta bakımını sağlayacak çalışma ortamının oluşturulması önerilmektedir. Just like in all fields of service industry, executing work analysis practises in medical services to plan workforce, identify responsibilities and pursue technological and scientific innovations is gaining importance. This descriptive and analytic study is realized to identify the work analysis of nurses working in department of internal medicine. Data of the study is gathered from 8 nurses working in department of internal medicine of a state hospital in European Side of Istanbul between the dates of 7th – 21th November, 2016. Through the study, nurse workload is identified via considering nursing activities in the department and the time required for these activities, then significance level of the activities compared to each other are specified using DEMATEL method while significance weighting is actualised using ANP network models. Furthermore, physical and environmental conditions that affect the activity and required competence are determined through 'Form of Work Analysis.' Physical working conditions of the nurses are found to be in accordance with regulations. Nurses are spending 52.44% of their work time for direct patient care activities, 13.58% for indirect patient care activities, 13.97% for activities related to department order and operations, 20% to individual activities and 0.01% to education and development activities. When department activities and their significance weights in event of an increase in workload, are considered according to data gathered from the calculations via ANP method; weights are found as follows: recording and tracking of vital signs belonging to direct patient care is 6.98%; attendance to doctor visit belonging to indirect patient care activities is 7.65%; drug per patience entry to the medical system belonging to department order and operations is 4.42%; lunch break belonging to personal physiologic needs activities is 8.07% and in-service training belonging to education-development activities is 0.01%. As a result of the study; creating environments that ensures efficient and safe patient care and identifying conditions that affect the work performance of nurses working in all departments are suggested to increase the quality of patient care.
