Abstract
Bu çalışma Kocaeli ili Gebze ilçesinde faaliyet gösteren özel bir hastanede hematopoetik kök hücre nakli tedavisi alan hastaların bilişsel fonksiyonlarını incelemek amacıyla tanımlayıcı olarak yapılmıştır. Araştırmanın örneklemini Kocaeli ili Gebze ilçesinde faaliyet gösteren özel hastanede kemik iliği nakli tedavisi alan gönüllü 100 hasta oluşturmuştur. Veriler Kasım 2018-Mart 2019 tarihleri arasında 'Sosyodemografik Bilgi Formu', 'Standardize Mini Mental Test Ölçeği(SMMT)' ve 'Addenbrook Kognitif Muayenesi ACE-R' kullanılarak toplanmıştır. Verilerin değerlendirilirken NCSS (Number Cruncher Statistical System) 2007 (Kaysville, Utah, USA) programı kullanılmış. Student t testi, One-Way Anova testi, Mann Whitney U testi ve Kruskall Wallis testi kullanılmıştır. Araştırmaya katılan hastaların Standardize Mini Mental Test toplam puan sonuçlarının otolog ve allojeneik nakil tipi içinde yüksek olması istatistiksel olarak anlamlı bulunmuştur(p=0,033; p<0,05). Kayıt hafıza puan ortalamasının(p=0,046; p<0,05), dikkat hesap puan ortalamasının(p=0,028; p<0,05) ve hatırlama puan ortalamasının(p=0,026; p<0,05) istatistiksel olarak yüksek olması anlamlı bulunmuştur. Addenbrook Kognitif Muayenesi puan ortalaması yüksek olması istatistiksel olarak anlamlı bulunmuştur (p=0,029; p<0,05). Dikkat oryantasyon puan ortalamasının(p=0,046; p<0,05), bellek puan ortalamasının(p=0,028; p<0,05) ve görsel mekansal puan ortalamasının(p=0,039; p<0,05) yüksek olması istatistiksel olarak anlamlı bulunmuştur. Araştırmada elde edilen sonuçlar hematopoetik kök hücre nakli olan hastaların bilişsel işlevselliğinde bozulma olduğunu ortaya koymuştur. Anahtar Kelimeler: Hematopoetik kök hücre nakli, Kemik iliği nakli, Bilişsel işlevsellik, Psikoonkoloji.
COGNITIVE FUNCTIONALITY OF PATIENTS WITH HEMATOPOETIC STEM CELL TRANSPLANTATION AND INVESTIGATION OF EFFECTING FACTORS This study was carried out as a descriptive study to investigate the cognitive functions of hematopoietic stem cell transplantation treatment in a private hospital in gebze, kocaeli. The sample of the study consisted of 100 volunteer patients receiving bone marrow transplantation treatment in a private hospital in Gebze, Kocaeli. Data were collected between November 2018 and March 2019 by using if Sociodemographic Information Form, Standardized Mini Mental Test Scale (SMMT) 'and' Addenbrook Cognitive Examination Ace-R. Data were analyzed with 95% confidence interval. While evaluating the data, NCSS (Number Cruncher Statistical System) 2007 (Kaysville, Utah, USA) program was used. Student t test, One-Way Anova test, Mann Whitney U test and Kruskall Wallis test were used. The results of the Standardized Mini Mental Test total score of the patients included in the study were found to be high in both types of transplantation (p=0,033; p<0,05). It was found that the mean score of the record memory (p=0,046; p<0,05) , attention calculator mean score (p=0,028; p<0,05) and the mean score of the recall (p=0,026; p<0,05) were statistically significant. High average scores of Addenbrook Cognitive Examination were found statistically significant (p=0,029; p<0,05). It was found statistically significant that the The average of the attention orientation score(p=0,046; p<0,05) , average of memory score(p=0,028; p<0,05) and visual spatial score(p=0,039; p<0,05) were high. The results of the study revealed that the cognitive functioning of patients with hematopoietic stem cell transplantation was impaired. Keywords: Hematopoietic stem cell transplantation, Chemotherapy, Cognitive functioning, Psychooncology.
