Abstract
Obezite ve depresyon toplum sağlığını olumsuz etkileyen ve görülme sıklıkları giderek artan ciddi halk sağlığı sorunlarıdır. Obezite ile depresyon ve benlik saygısı durumunun ilişkisini değerlendirmek amacıyla yapılan bu çalışma kesitsel tiptedir. Çalışmanın örneklemini psikiyatrik hastalığı bulunmayan, gebe ve emziklilik döneminde olmayan, 18 yaş üstü 126'sı kadın 125'i erkek toplam 251 gönüllü kişi oluşturmuştur. İzmir Gaziemir 13 Nolu Sevgi Aile Sağlığı Merkezi'nde 35.12.038 Nolu Aile Hekimliği Birimi'nde Haziran-Temmuz 2019 tarihleri arasında yürütülmüştür. Bireylere genel özelliklerinin sorgulandığı bir anket formu yüz yüze görüşerek uygulanmış, bireylerin antropometrik ölçümleri (vücut ağırlığı, boy uzunluğu, bel ve kalça çevresi) alınmış ve beden kütle indeksi (BKİ) değerleri hesaplanmıştır. Bireylerin benlik saygısı durumu Coopersmith Benlik Saygısı Ölçeği (CBSÖ) ile depresyon durumu ise Beck Depresyon Ölçeği (BDÖ) ile belirlenmiştir. İstatistiksel analizler IBM Statistical Package for the Social Sciences (SPSS) 22,0 programı ile gerçekleştirilmiştir ve anlamlılık düzeyi p<0,05 olarak kabul edilmiştir. Araştırma sonucunda BKİ'ye göre bireylerin %28,7'si obez bulunmuştur. BKİ ile CBSÖ puanı arasında istatistiki olarak anlamlı bir ilişki gözlenmemişken (r=-0,045; p>0,05); BKİ ile BDÖ puanı arasında istatistiki olarak anlamlı, pozitif yönde ve zayıf düzeyde ilişki gözlenmiştir (r=0,126; p<0,05). CBSÖ puanı ile BDÖ puanı arasında istatistiki olarak anlamlı, negatif yönde ve orta düzeyde ilişki gözlenmiştir (r=-0,445; p<0,01). Kadınların bel çevresi kategorileri açısından (88 cm altı ile 88 cm ve üstü) BDÖ puanları arasında istatistiksel olarak anlamlı farklılık gözlenmiştir (p<0,05). Bel çevresi 88 cm ve üstü olan kadınların BDÖ puanı daha yüksek bulunmuştur. Sonuç olarak obezite psikososyal yönden de değerlendirilmeli ve tedavisi çok kapsamlı bir yaklaşımla ele alınmalıdır.
Obesity and depression are serious public health problems that negatively affect public health and their frequency is increasing. This study, conducted to evaluate the relationship between obesity and depression and self-esteem, is cross-sectional. The sample of the study consisted of a total of 251 volunteers, 126 of whom are women and 125 of whom are men, over 18 years of age, without psychiatric disease, pregnant and lactating. It was conducted between June-July 2019 at the Family Medicine Unit No. 35.12.038 in the İzmir Family Health Center No. 13, Gaziemir. A questionnaire form that inquired about the general characteristics of individuals was applied by face-to-face interview method, anthropometric measurements (body weight, height, waist circumference, hip circumference) of individuals and body mass index (BMI) values were calculated. Self-esteem status of individuals was determined by Coopersmith Self-Esteem Inventory (CSEI) and depression status was determined by Beck Depression Inventory (BDI). Statistical analyzes were made with the Statistical Package for the Social Sciences (SPSS) 22.0 program and the significance level was accepted as p <0.05. As a result of the research, 28,7% of individuals were found obese according to BMI. While no statistically significant relationship was observed between BMI and CSEI score (r = -0.045; p>0.05); A statistically significant, positive and weak level relationship was observed between BMI and BDI score (r = 0.126; p<0.05). A statistically significant, negative and moderate relationship was observed between CSEI score and BDI score (r = -0.445; p <0.01). A statistically significant difference was observed between the waist circumference of women (below 88 cm and above 88 cm) and BDI scores (p <0.05). Women with a waist circumference of 88 cm and above were found to have higher BDI scores. As a result, obesity should be evaluated from a psychosocial perspective and its treatment should be handled with a multi-dimensional approach.
