Abstract
Giriş ve Amaç: Bu çalışmanın amacı unilateral diz altı amputelerde denge eğitiminin yürüme aktivitesi üzerine etkisini incelemekti. Yöntem: Çalışmaya 40 unilateral diz altı ampute hasta alındı ve katılımcılar grup 1 fizyoterapi grubu (n=20) ve grup 2 (n=20) kontrol grubu olarak bilgisayar programı aracılığı ile randomizasyon yapılarak ayrıldı. Fizyoterapi grubuna klasik fizyoterapinin (destek ve kuvvetlendirme egzersizleri) yanı sıra denge egzersizleri, kontrol grubuna ise klasik fizyoterapi ev programı olarak uygulandı. Katılımcılara eklem hareket açıklığı değerlendirmesi, kuvvet değerlendirmesi, berg denge ölçeği, tek ayak üzerinde durma testi, tinetti denge ve yürüme testi, otur kalk testi ve gözlemsel yürüme analizi testleri uygulandı. Değerlendirmeler tedavi öncesinde v sonrasında olmak üzere 2 kez yapıldı. Bulgular: Kuvvet egzersizlerinin denge yetisine etkisi incelendiğinde hem çalışma grubunda hem de kontrol grubunda istatistiksel olarak anlamlı bir fark gözlemlendi (p <0,05). Otur-kalk, berg denge testi ve tek ayak gözler kapalı denge testine etkisi incelendiğinde ön test ve son test puanları arasında istatistiksel olarak anlamlı bir fark gözlemlendi (p<0,05). Araştırma grubunu oluşturan kontrol grubunda da kuvvet egzersizlerinin Tek Ayak Gözler Kapalı, Otur-Kalk ve Berge Denge testi ön test ve son test puanları arasında istatistiksel olarak anlamlı bir fark olduğu görüldü (p<0,05). Denge egzersizlerinin yürümeye etkisi incelendiğinde çalışma grubunda ön test ve son test puanları arasında istatistiksel olarak anlamlı bir fark varken (p<0,05) diğer test puanları arasında bir fark görülmedi (p >0,05). Denge egzersizlerinin toplam yürüme puanlarını etkilemesine ilişkin veriler incelendiğinde çalışma grubunun ön teste son test puanları arasında istatistiksel olarak anlamlı fark olduğu görüldü (p<0,05). Kontrol grubunda ise ön test-son test puanları arasında bir fark görülmedi (p>0,05). Sonuç: Bu çalışmanın sonucunda unilateral diz altı amputelere uygulanan 6 haftalık kuvvet egzersizlerinin denge yeteneğine olumlu etkisi olduğu görüldü. Ayrıca denge egzersizlerinin yürüme aktivitesine de olumlu etkisi olduğu görüldü
Introduction and purpose: The aim of this study was to investigate the effect of balance training on gait activity in unilateral sub-knee amputees. Method: Forty unilateral amputee patients were enrolled in the study and the participants were divided into two groups as physiotherapy group (n = 20) and group 2 (n = 20) as a control group. Apart from classical physiotherapy, balance exercises were applied to the physiotherapy group and classical physiotherapy home program was used to the control group. Participants were evaluated for range of motion, strength assessment, berg balance scale, one-foot stand test, tinetti balance and gait test, sit-up test and observational gait analysis tests. Evaluations were made twice before and after treatment. Results: When the effect of strength exercises on one foot eyes open balance ability was examined, a statistically significant difference was observed in both the study and control groups (p <0.05). When the effect of sit-stand, berg balance test and one-foot eyes closed balance test were examined, a statistically significant difference was observed between the pre-test and post-test scores (p <0.05). There was a statistically significant difference between the pre-test and post-test scores of the strength exercises on the One Foot Eyes Closed, Sit-Up and Berge Balance test in the control group (p <0.05). When the effect of balance exercises on walking was examined, there was a statistically significant difference between the pre-test and post-test scores in the study group (p <0.05), but there was no difference between the other test scores (p> 0.05). In the control group, there was no difference between the pre-test and post-test scores (p> 0.05). Conclusion: As a result of this study, it was seen that 6 weeks of strength exercises applied to unilateral sub-knee amputees had a positive effect on balance ability. In addition, balance exercises had a positive effect on walking activity.
