Serebral palsili bireylerde işeme bozuklukları, fonksiyonel bağımsızlık ve yaşam kalitesinin değerlendirilmesi
Abstract
Bu çalışma özel eğitim ve rehabilitasyon merkezinde SP tanısı ile rehabilitasyon gören bireylerin, kaba motor fonksiyon sınıflama sistemine göre, işeme bozuklukları, fonksiyonel bağımsızlık ve yaşam kalitesi arasındaki ilişki incelendi. Araştırma Binbir Hayat Özel Eğitim ve Rehabilitasyon Merkezi, Sağlam Adımlar Özel Eğitim ve Rehabilitasyon Merkezi ve Çamlıca Özel Eğitim ve Rehabilitasyon Merkezinde rehabilitasyon gören, 8-17 yaş aralığındaki 102 SP'li bireye sahip ebeveyn üzerinde randevu usulü ile yüz yüze görüşülerek yapıldı. Bireylerin sosyodemografik bilgileri kaydedildikten sonra, SP'li bireylerin fonksiyonel seviyeleri 'Kaba Motor Fonksiyon Sınıflama Sistemi' (KMFSS), işeme bozuklukları 'İşeme Bozuklukları Semptom Skoru' (İBSS), fonksiyonel bağımsızlık 'Pediatrik Fonksiyonel Bağımsızlık Ölçeği' (PFBÖ) ve yaşam kalitesi 'Çocuklar İçin Yaşam Kalitesi Ölçütü' (ÇİYKÖ) ile değerlendirildi. Sonuç olarak araştırmaya katılan SP'li bireylerin İBSS'ye göre üriner inkontinans semptomları sırasıyla aciliyet, gündüz idrar kaçırma, idrarını tutma manevraları, tam boşalamama, kesik kesik işeme, aşırı aktif mesane, kabızlık, gece idrar kaçırma, ağrı, ıkınma olarak saptandı. Araştırmaya katılan bireylerin KMFSS'ye göre; İBSS, PFBÖ ve ÇİYKÖ skorları arasında anlamlı farklılık bulundu. Araştırmaya katılan bireylerin SP sınıflaması ve yardımcı cihaz kullanım durumuna göre; İBSS skoru arasında anlamlı fark belirlenmedi. Bireyin ambulasyon durumuna göre; İBSS skoru arasında anlamlı fark görüldü. Bireylerin KMFSS ile İBSS skorları arasında anlamlı farklılık bulundu. PFBÖ ile ÇİYKÖ skorları arasında pozitif yönlü orta düzey anlamlı bir ilişki bulunmaktadır. ÇİYKÖ ile İBSS skorları arasında negatif yönlü orta düzey anlamlı bir ilişki bulunmaktadır. İBSS ile PFBÖ skorları arasında negatif yönlü orta düzey anlamlı bir ilişki bulunmaktadır. SP'li bireylerin kaba motor fonksiyonel seviyesi arttıkça işeme bozukluğunun arttığı, fonksiyonel bağımsızlığın ve yaşam kalitesinin azaldığı bulundu. In this study, the relationship between voiding disorders, functional independence, and quality of life was investigated in individuals who were rehabilitated with CP diagnosis in private education and rehabilitation center according to the Gross Motor Function Classification System. The research was conducted by face-to-face interviews with parents of 102 individuals with CP between the ages of 8-17, who were rehabilitated at Binbir Hayat Special Education and Rehabilitation Center, Sağlam Adımlar Special Education and Rehabilitation Centre, and Çamlıca Private Education and Rehabilitation Center. After recording the sociodemographic information of the individuals, the functional levels of the individuals with CP were evaluated by the 'Gross Motor Function Classification System' (GMFCS), the voiding disorders by the 'Dysfunctional Voiding and Incontinence Scoring System' (DVAISS), the functional independence by the 'Pediatric Functional Independence Scale' (WeeFIM) and the quality of life by the ''Pediatric Quality of Life Inventory' (PedsQL). As a result, urinary incontinence symptoms of individuals with CP who participated in the study according to DVAISS were determined as urgency, daytime urinary incontinence, urinary retention maneuvers, incomplete evacuation, intermittent urination, overactive bladder, constipation, nocturnal urinary incontinence, pain, straining. According to the GMFCS, we have found significant differences between the scores of DVAISS, WeeFIM, and PedsQL of the individuals participating in the research. According to the CP classification and assistive device usage status of the individuals participating in the research, there was no significant difference between the DVAISS scores. According to the ambulation status of the individual, there was a significant difference between the GMFCS and DVAISS scores. There is a positive and moderately significant relationship between WeeFIM and PedsQL scores. There is a moderately significant negative correlation between PedsQL and DVAISS scores. There is a moderately significant negative correlation between DVAISS and WeeFIM scores. As the gross motor functional level of individuals with CP increased, voiding dysfunction increased, and functional independence and quality of life decreased.
