Abstract
Nonalkolik yağlı karaciğer hastalığı (NALFD), karaciğer hastalıklarının önde gelen nedenidir, metabolik sendromun hepatik versiyonu olarak kabul edilir. Obezite, tip 2 diyabet, dislipidemi ve insülin direncine sahip kişilerde yaygındır. Dünya çapında ciddi bir halk sağlığı sorunu olmasına karşın hastalığın prevelansını belirlemek oldukça zordur. Bu çalışmada amaç; NAFLD tanısı almış hastaların, NAFLD uygun beslenme tedavisi, beslenme eğitimi ve düzenli egzersiz programı sonucunda kazandıkları doğru beslenme alışkanlıkları ve kaybettikleri ağırlıkla birlikte hastaların kan bulgularındaki düzelmenin saptanmasıdır. Çalışma, Ağustos 2016 - Ocak 2017 tarihleri arasında Bursa Çekirge Devlet Hastanesi dahiliye polikliniğinden diyet polikliniğine NAFLD tanısı konularak gönderilmiş 30 hasta retroprospektif olarak değerlendirilmiştir. Çalışmaya katılan 30 hastanın % 40'ı erkek, % 60'ı kadındır. Yaş ortalaması 45.80±10.27 yıldır. 30 hastanın 3 ay boyunca sadece beslenme tedavisi, beslenme eğitimi ve egzersiz önerisi sonucunda; ağırlık (kg), BKİ, yağ oranları, AKŞ, ALT, AST, GGT, CHOL, TG, LDL, HDL ve HbA1C seviyelerindeki farklılıklar incelenmiştir. 30 hastada başlangıç ile 3. ay sonunda ağırlık (kg), BKİ ve yağ oranları düşerek anlamlı azalmalar tespit edilmiştir. 30 hastada başlangıç ile 3.ay sonunda ağırlık(kg), BKİ ve yağ yüzdelerinde 89,88±12,94'dan 81,15±11,34'e, 32,97±5,47'dan 29,72±4,51'e ve 36,94±9,41'den 29,78±9,4'e düşerek anlamlı azalmalar tespit edilmiştir (p<0,001). Hastaların başlangıç ile 3. ay sonunda AKŞ, ALT, AST, GGT, CHOL, TG, LDL, HDL ve HbA1C seviyelerinde de anlamlı fark saptanmıştır (p<0,001). Bu çalışmada NAFLD uygun beslenme tedavisi, beslenme eğitimi ve egzersiz ile yaşam biçimi değişikliği sonucu ağırlık kaybı ile hastalık ile ilgili bulgularda düzelme olabileceği belirlenmiştir. Nonalcoholic fatty liver disease (NALFD) is a leading cause of liver disease and is considered as the hepatic side of metabolic syndrome and is extremely common in subjects with obesity, type 2 diabetes mellitus, dyslipidemia and insulin resistance. Although it is a serious public health problem worldwide, it is very difficult to determine the prevalence of the disease. In this study; NAFLD-diagnosed patients are aimed to determine the correct nutritional habits that NAFLD has acquired as a result of appropriate nutritional therapy, nutritional training and regular exercise programs and to improve blood tests in patients with lost weight. Study; 30 patients who were referred to Bursa Çekirge State Hospital's polyclinic from the polyclinic diet to NAFLD were retrospectively evaluated between August 2016 and January 2017. %40 out of 30 patients participanting in the study are male and %60 are female. The average age of participants (average ±standard deviation) is 45.80±10.27. Differences in kg, BMI, fat percentages, fasting blood glucose, ALT, AST, GGT, total chosterol, TG, LDL, HDL, HBA1C levels were investigated in 30 patients for 3 months only as nutrition therapy, nutrition education and exercise recommendations. In 30 patients, significant reductions in weight, BMI, and fat fractions were detected as a result of the baseline versus and the 3rd month comparisons. In 30 patients, significant reductions in weight, BMI, and fat fractions (respectively 89,88±12,94-81,15±11,34, 32,97±5,47-29,72±4,51 ve 36,94±9,41- 29,78±9,4) were detected as a result of the baseline versus and the 3 rd month comparisons (p<0.001).There was a significant difference in the levels of fasting blood glucose, ALT, AST, GGT, total chosterol, TG, LDL, HDL, HBA1C at baseline and 3rd month comparison of the patients (p <0.001). In this study, it was determined that appropriate nutrition therapy, nutrition education and exercise, lifestyle change after weight loss and improvement in disease related findings could be observed in NAFLD.
