Hastalar ve çalışanların gözünden covıd-19 pandemisinin radyasyon onkolojisi departmanına etkisi

Abstract

Kanser tanı ve tedavisi ile uğraşan tüm sağlık kurumlarında, Türkiye'deki ilk COVID-19 vakası görülmeden önce hastalık ve korunma yöntemleri ile ilgili bilgilendirmeler yapılmıştır. Hastalık Türkiye'de görüldüğü andan itibaren birçok güvenlik önlemleri hayata geçirilmiştir. Özellikle sağlık sistemlerinde hizmetin sürekliliğini sağlamak en önemli konu olmuştur. Onkolojik tedavilerin kesintiye uğramasının en az bu hastalık kadar ciddi yan etkilere sebep vereceği bilinciyle yola çıkılmış ve tedavilerin devamlılığı için gerekli önlemler alınmıştır. Kanser gibi hem kendisi hem de tedavisi bağışıklık sistemini zayıflatan bir hastalığın tanı ve takibi, bu özel süreçte her zamankinden daha dikkatli ve özenli bir yaklaşımı gerektirmektedir. Bu nedenle de klinik deneyimlerine ihtiyaç ortaya çıkmaktadır. Araştırma özel bir hastanenin radyasyon onkolojisinde tedavisi pandemi dönemine gelen onkoloji hastaları ve radyasyon onkolojisi çalışanları olan 14 katılımcı ile yüz yüze görüşmeler ile gerçekleştirilmiştir. Sorular literatür doğrultusunda oluşturulmuştur ve 17 adet birbiriyle bağlantılı sorular içermiştir. Görüşmelerin ortalama süresi 30 dk sürmüştür ve öncesinde bireylerden sözlü ve yazılı onam alınmıştır. Görüşmelerde ortaya çıkan unsurlar belirlenmiş ve açıklamaların sunulabilmesi için temalar ve temalar arasındaki ilişkiler haritalandırılmıştır. Araştırmanın analizinde MAXQDA Plus 2020 (20.0.08 versiyon) paket programı kullanılmıştır. Bu çalışmanın sınırlıkları vardır. Öncelikle çalışma bulguları görüşme yapılan katılımcılar ve çalışmanın yürütüldüğü merkez ile sınırlıdır. Ayrıca çalışma bulguları 01.01.2022 ve 01.05.2022 Tarihleri arasında toplanmıştır. Bu tarihten sonraki pandemiye ilişkin ortaya çıkan herhangi bir düzenleme çalışma kapsamına dahil edilmemiştir. Bu çalışma pandemi dönemi boyunca radyasyon onkolojisi departmanında ortaya çıkan zorlukların neler olduğu ve bu zorluklara sağlık yöneticisi bakış açısıyla nasıl çözümler bulunduğunu ortaya koyma amacıyla gerçekleştirilmiştir. Çalışma kapsamında yapılan görüşmeler neticesinde dört ana tema belirlenmiştir. Bu temalar COVID-19 endişesi, COVID-19'un radyasyon tedavisine etkileri, tedavinin sürdürülmesindeki zorluklar ve çalışma yaşantısına etkileri olarak belirlenmiştir.

Before the first COVID-19 case in Turkey, all health institutions which deals with cancer diagnosis and treatment was informed about the disease and the methods to prevent it. After the first case related to COVID-19 was seen in Turkey, many security measures have been implemented. Especially in health systems, the most important topic has been service persistence. With the awareness that interruption of oncological treatments will cause side effects at least as serious as this disease, necessary precautions have been taken for the continuation of the treatments. Cancer, which is a disease that itself and it's treatment weakens the immune system, requires a more careful and attentive approach for diagnosis and follow-up in this extraordinary moment. Therefore, the need for clinical experience arises. The research was carried out with face-to-face interviews with 14 participants, who were oncology patients and radiation oncology workers who were treated in the radiation oncology department of a private hospital during the pandemic period. The questions were created in line with the literature and included 17 interrelated questions. The average duration of the interviews lasted 30 minutes, and verbal and written consent was obtained from the individuals beforehand. The elements that emerged from the interviews were identified and the themes and the relationships between the themes were mapped in order to present the explanations. MAXQDA Plus 2020 (20.0.08 version) package program was used in the analysis of the research. This study has limitations. First of all, the findings of the study are limited to the interviewees and the center where the study was conducted. In addition, the study findings were collected between 01.01.2022 and 01.05.2022. Any regulation related to the pandemic after this date was not included in the scope of the study. This study was carried out in order to reveal the difficulties that emerged in the radiation oncology department during the pandemic period and how solutions were found to these difficulties from the point of view of a health manager. As a result of the interviews conducted within the scope of the study, four main themes were determined. These themes were determined as COVID-19 concern, the effects of COVID-19 on radiation therapy, the difficulties in maintaining the treatment and its effects on working life.
