Abstract
Bu araştırma, 2015-2017 yılları arasında İstanbul'da Özel bir Beslenme ve Diyet Danışmanlık Merkezi'ne başvuran ve 1 yıl takipte kalan, biyokimyasal tetkiklerle insülin direnci tanısı alan ve abdominal obezitesi olan bireylere uygulan tıbbi beslenme tedavisinin insülin direnci ve abdominal obeziteye etkisini araştırmak amacıyla yapılmış tanımlayıcı bir çalışmadır. Çalışmaya dahil edilen bireylerin antropometrik ölçümleri, biyokimyasal bulguları, demografik özellikleri, genel sağlık durumları anket formu kullanılarak, danışmanlık merkezinin hasta kayıt sisteminden elde edilmiştir. Çalışmaya 51 kadın, 23 erkek toplam 74 birey dahil edilmiştir. Araştırmaya katılan bireylerin yaşları 25 ile 71 yaş arası değişmekte olup, yaş ortalaması 42.70 ± 10.34'tür. Araştırmaya katılanlardan geldiklerinde, 6 ve 12. aylarda alınan antropometrik ölçüm verileri (bel çevresi, boy, ağırlık ve BKİ) ve kan bulguları (HOMA_IR, Total Kolesterol, LDL, HDL ve Trigliserit) değerleri için Friedman testi uygulanmıştır. Katılımcıların ilk dönem, 6.ay ve 12. Ay BKİ (kg/m2) değerleri arasında istatistiksel olarak önemli fark vardır (p<0,01). Üç farklı zamanda ölçülen kan HOMA_IR değerleri arasında anlamlı bir fark bulunmaktadır (p<0,01). Katılımcıların, ilk kan HOMA_IR ölçüm ortalaması 4.18 olup standart sapma değeri 1.17 bulunmuştur. Sonuç olarak; bu çalışma insülin dirençli ve abdominal obezitesi olan bireylere uygulanan tıbbi beslenme tedavisinin sağlık sorunlarının iyileştirilebileceği yönünde bulgular sunmaktadır. The impact of medical nutritional therapy on the level of insuline resistance of people with abdominal obesity who were also diagnosed with insuline resistance This research was conducted in a private nutrition counselling center in Istanbul during the period of 2015-2017. This research The aim of this research is at investigating the impact of medical nutritional therapy on the level of insulin resistance and abdominal obesity in people already with insulin resistance and abdominal obesity. The information on participant's anthropometric measurements, biochemical parameters, demographic features and general health conditions were obtained through a survey directly from the participants registered in patient record system of the mentioned counseling center. Among 74 people, who were included in the present research, 51 of them are female and 23 of them are male; furthermore, ages of the participants are ranging from 25 to 71 (mean: 42.70 ± 10.34). The anthropometric measurements and the level of HOMA-IR, Total Cholesterol, LDL, HDL and Triglyceride of the participants were recorded at three different stages of the therapy process (the first, the sixth and the twelfth months) and evaluated statistically by using Friedman Test. According to the results obtained from the present study, there is a statistical significance among the records of the participants' measurements and HOMA-IR levels in three different stages mentioned above (p<0.01).
