Acil Servise Başvuran Hastaların Akut Stres Düzeyleri ile Anksiyete Depresyon Belirtileri Arsındaki İlişkinin İncelenmesi

Abstract
Bu araştırma İstanbul'da bir eğitim araştırma hastanesinin acil servisine (AS) müracaat eden hastaların akut stres düzeyleri ile anksiyete depresyon belirtileri arasındaki ilişkinin incelenmesi amacıyla gerçekleştirilmiştir. Tanımlayıcı ve ilişki arayıcı nitelikteki araştırmanın evrenini bir eğitim araştırma hastanesinin acil servisine müracaat eden hastalar oluşturdu. Örneklemini 20.03.2023-29.05.2023 tarihleri arasında acil servise başvuran araştırmaya katılmak isteyen, aynı zamanda dahil edilme şartlarını sağlayan 220 hasta yer aldı. Sosyo-demografik veri formu, akut stres belirti şiddeti düzeyi ölçeği ve hastane anksiyete depresyon ölçeği kullanılarak araştırma verileri elde edildi. Hastaların yaş ortalamasının 51,54±16,94 (yıl) olduğu, 77 kişinin (%35,0) ≥60 yaş grubunda olduğu belirlenmiştir. 129 kişinin (%58,6) erkek olduğu, 144 kişinin (%65,4) evli olduğu, 94 kişinin (%42,7) ilköğretim mezunu ve 117 kişinin (%53,2) gelirinin giderine eşit olduğu belirledi. Hastaların Akut Stres Belirti Şiddeti Ölçeği (ASBÖ) puan ortalaması 8,77±3,43 olduğu belirlendi. Hastaların 40 yaş altında olanların ve eğitim düzeyi lise olanların ASBÖ toplam puan ortalamaları diğer gruplara kıyasla daha yüksektir. Hastaların HADÖ anksiyete alt boyutu puan ortalaması 9,14± 3,33 olarak bulundu. Hastaların %36.8'sinin HADÖ-anksiyete alt boyut puanlarının sınırda olduğu, %49.1'inin HADÖ-depresyon alt boyutu puanlarının anormal olduğu bulundu. HADÖ depresyon alt boyutu puan ortalaması 11,07± 4,41 olarak bulundu. Hastaların ASBÖ ile HAD – Anksiyete ölçeği arasında pozitif bir ilişki gözlemlenmesine karşın istatistiksel olarak zayıf bir anlamlılık düzeyi belirlendi (r=0,290; p<0,001). ASBÖ ile HADÖ – Depresyon alt boyutu arasında negatif bir ilişki gözlemlenmesine karşın istatistiksel olarak zayıf bir anlamlılık düzeyi belirlendi (r=-0,364; p<0,001). This research was conducted to investigate the relationship between the acute stress levels and symptoms of anxiety and depression among patients presenting to the emergency department of an educational research hospital in Istanbul. The study, designed in a descriptive and correlational type, targeted patients presenting to the emergency department of an educational research hospital as its population. The sample consisted of 220 patients who voluntarily met the inclusion criteria and presented to the emergency department between March 20, 2023, and May 29, 2023. Research data were obtained using a socio-demographic data form, the Acute Stress Symptom Severity Scale, and the Hospital Anxiety Depression Scale. The average age of the patients was determined to be 51.54±16.94 (years), with 77 individuals (35.0%) being in the ≥60 age group. Out of the total, 129 individuals (58.6%) were male, 144 individuals (65.4%) were married, 94 individuals (42.7%) had completed primary education, and 117 individuals (53.2%) reported income equal to expenses. The mean score of the Acute Stress Symptom Scale (ASBSS) for the patients was found to be 8.77±3.43. Patients under the age of 40 and those with a high school education had higher ASBSS total scores. The average score for the anxiety subscale of the Hospital Anxiety and Depression Scale (HADS) was 9.14±3.33. It was found that 36.8% of patients had borderline scores on the HADS-anxiety subscale, and 49.1% had abnormal scores on the HADS-depression subscale. The average score for the depression subscale of HADS was 11.07±4.41. A positive, weak, and statistically significant correlation was identified between ASBSS and HADS-Anxiety scale (r=0.290; p<0.001). A negative, weak, and statistically significant correlation was found between ASBSS and HADS-Depression subscale (r=-0.364; p<0.001).
