Tip 2 diyabet hastalarında cinsel disfonksiyon ve yaşam kalitesinin değerlendirilmesi

Abstract
Bu araştırma, Tip 2 Diabetes Mellitus tanılı hastaların cinsel disfonksiyonu ve yaşam kalitesinin değerlendirilmesi amacıyla planlanmıştır. Araştırmanın örneklemi 267 kadın, 218 erkek olmak üzere toplam 485 Tip 2 diyabet tanılı bireylerden oluşmuştur. Veriler, Genel Bilgi Formu, Kadın Cinsel Fonksiyon İndeksi (FSFI), Ereksiyon İşlevi Uluslararası Değerlendirme Formu (IIEF) ve Yaşam Kalitesi Ölçeği (SF-36) aracılığıyla toplanmıştır. Kadınların %91'inde cinsel disfonksiyon olduğu saptanmıştır. Kadınlarda cinsel disfonksiyon sırasıyla cinsel istek (%92,2), orgazm, uyarılma (%92,9) ve ağrı, doyum, lubrikasyon (%92,5) alanlarında yaşanmakta olduğu saptanmıştır. Erkeklerde erektil fonksiyon bozukluğu %91,3 olarak tespit edilmiş ve erektil fonksiyonun derecesi sırasıyla orta (%55), hafif (%21,6) ve ağır (%14,7) olduğu görülmüştür. Cinsel disfonksiyon alanlarıyla ilgili yaşanan sorunlar, genel tatmin %79,4, orgazmik fonksiyon %50,5 ve cinsel istek %49,1 alt boyutlarında olduğu saptanmıştır. Olgularda yaş, eğitim düzeyi, BKI, HbA1c ve LDL düzeyleri, diyabet tanı süresi, kullanılan ilaçlar, nöropati, retinopati, nefropati, diyabetik ayak, menopoz gibi faktörlerin cinsel disfonksiyonun oluşmasında etken olduğu saptanmıştır. Çalışmamızda erkeklerde ve kadınlarda cinsel disfonksiyonun yaşam kalitesini etkilediği ve ölçek alt boyutları istatistiksel olarak anlamlı bir farklılığa sahip olduğu (p<0.05), cinsel disfonksiyon bozukluğu olan erkeklerde fiziksel rol güçlüğü, fiziksel fonksiyon, ağrı, canlılık, emosyonel rol, ruhsal sağlık, sosyal fonksiyon alt boyut puanları cinsel disfonksiyon bozukluğu olmayanlara kıyasla anlamlı olarak daha düşük olduğu, kadınlarda fiziksel fonksiyon, sosyal fonksiyon, emosyonel rol güçlüğü, fiziksel rol güçlüğü ve ruhsal sağlık alt boyut puanı istatistiksel olarak anlamlı bir farklılığa sahip olduğu (p<0.05) saptanmıştır. Sonuç olarak Tip 2 Diyabeti olan kadın ve erkeklerde cinsel disfonksiyon yaygındır ve buna paralel olarak yaşam kalitesi de düşüktür. This study was planned to evaluate the sexual dysfunction and quality of life of patients with Type 2 Diabetes Mellitus. The sample of the study consisted of 485 individuals diagnosed with Type 2 diabetes, 267 women and 218 men. Data were collected through the General Information Form, the Female Sexual Function Index (FSFI), the International Erectile Function Assessment Form (IIEF), and the Quality-of-Life Scale (SF-36). It was found that 91% of women had sexual dysfunction. It was determined that sexual dysfunction in women was experienced in the areas of sexual desire (92.2%), orgasm, arousal (92.9%) and pain, satisfaction and lubrication (92.5%), respectively. Erectile dysfunction was found to be 91.3% in men, and the degree of erectile function was moderate (55%), mild (21.6%) and severe (14.7%), respectively. Problems related to the areas of sexual dysfunction were found in the sub-dimensions of general satisfaction 79.4%, orgasmic function 50.5% and sexual desire 49.1%. Factors such as age, education level, BMI, HbA1c and LDL levels, duration of diabetes diagnosis, drugs used, neuropathy, retinopathy, nephropathy, diabetic foot, and menopause were found to be effective in the development of sexual dysfunction. In our study, it was found that sexual dysfunction affects the quality of life in men and women and the scale sub-dimensions have a statistically significant difference (p<0.05), physical role difficulty, physical function, pain, vitality, emotional role, mental health, social function in men with sexual dysfunction sub-dimension scores were found to be significantly lower than those without sexual dysfunction, and there was a statistically significant difference in physical function, social function, emotional role difficulty, physical role difficulty and mental health sub-dimension scores in women (p<0.05). As a result, sexual dysfunction is common in men and women with Type 2 Diabetes, and accordingly, the quality of life is low.
