Yetişkin hematolojik kanserli hastalarda malnütrisyon ölçeklerinin değerlendirilmesi
Abstract
Bu araştırmada yetişkin hematolojik kanserli hastalarının farklı tarama testleri kullanılarak malnütrisyon durumlarının belirlenebilmesi ve tarama testlerinin birbiriyle kıyaslanması amaçlanmıştır. Hematolojik kanserli hastaların malnütrisyon durumları, malnütrisyona neden olan faktörler ve hastalıklarına bağlı olarak yaşadıkları semptomlar, farklı tarama testlerinden elde edilen veriler incelenmiştir. Araştırma, Nisan 2018 – Eylül 2018 tarihleri arasında Medicana International Ankara Hastanesi Hematoloji Servisine başvuran kişilerle gönüllülük esasına dayanılarak hazırlanmıştır. Çalışmaya 18-65 yaş arasındaki hematolojik kanser hastası olan %45,7'si kadın, %54,3'ü erkek olmak üzere toplam 116 birey katılmıştır. Kadınların yaş ortalaması 48,62±14,83 yıl, erkeklerin yaş ortalaması 48,19±16,82 yıldır. Kadınların beden kütle indeksi (BKİ) ortalaması 24,37±5,83 kg/m2, erkeklerin BKİ ortalaması 23,54±4,63 kg/m2'dir. Araştırmada uygulanan anketler yüz yüze görüşme yöntemiyle uygulanmış, antropometrik ölçümleri (boy uzunluğu, vücut ağırlığı, üst orta kol çevresi, baldır çevresi) yapılmıştır. Araştırmaya katılan hastalarda en sık görülen semptomlar yorgunluk, iştahsızlık, ağrı ve bulantı olarak belirlenmiştir. Üç farklı tarama testinin sonuçlarına göre hematolojik kanserli hastaların; Mini Nütrisyonel Değerlendirme'ye (MND) göre %55,7, Nütrisyon Risk Taraması–2002'ye (NRT–2002) göre %56,9, Malnütrisyon Genel Tarama Testi'ne (MGTT) göre %50,4 malnütrisyon ve/veya malnütrisyon riski taşıdıkları belirlenmiştir. Tarama testlerinden elde edilen sonuçlara göre; MND ile NRT – 2002'den elde edilen malnütrisyon sonuçları birbirine yakın iken, MGTT'den elde edilen sonuç, diğer iki tarama testinden daha az bulunmuştur. The aim of this study was to determine the malnutrition status of adult hematologic cancer patients using diferent screening test and to compare screening test with each other. Malnutrition status of patients with hematological cancer, the factors causing malnutrition, and the symptoms they experienced due to their diseases, and the data obtained from different screening tests were evaluated. The study was prepared on the voluntary basis with the applicants who applied to Medicana International Ankara Hospital Hematology Service between April 2018 – September 2018. Sample size; It consists of 116 individuals with hematological cancer between the ages 18-65 years. 45.7% of the participants were women and 54.3% were men. The mean age of the women was 48.62±14.83 years and the mean age of men was 48.19±16.82 years. The mean body mass index (BMI) of women is 24.37±5.83 kg/m2 and the mean BMI of men is 23.54±4.63 kg/m2. The questionnaires were applied by face to face interview, and anthropometric measurements (height, body weight, upper middle arm circumference, calf circumference) were performed. The most frequent symptoms in patients participating in the study were fatigue, loss of appetite, pain and nausea. According to the results of three different screening tests, patients with hematological cancer: 55.7% according to Mini Nutritional Assessment (MNA), 56.9% according to Nutritional Risk Screening–2002 (NRS-2002) and 50.4% according to Malnutrition General Screening Test (MGST) have been determined to have malnutrition and/or malnutrition risk. According to the results obtained from screening tests, malnutrition results of MNA and NRS-2002 were similar, whereas MGST defined a patient with malnutrition at a lower rate than the other two screening tests.
