Abstract
Araştırma preterm bebeklere iki farklı yağ kullanılarak uygulanan cilt bakımının cilt bütünlüğü ve büyüme parametrelerine etkisini belirlemek amacıyla randomize kontrollü deneysel çalışma olarak planlanmıştır. 1 Ekim 2020- 30 Aralık 2021 tarihleri arasında S.B.Ü Kocaeli DEAH 3.Düzey Yenidoğan Yoğun Bakım Ünitesinde gerçekleştirilmiştir. Veri toplamak amacıyla 'Yenidoğan Tanıtıcı ve Klinik Özellikleri Belirleme Formu' ve 'Yenidoğan Cilt Durum Değerlendirme Ölçeği' kullanılmıştır. Veriler SPSS 22.0 paket programında uygun istatistiksel yöntemlerle değerlendirilmiştir. Clinicaltrials.gov adresinden 21.03.2022 tarihinde Trial numarası (NCT04972747) alınmıştır. Araştırmada prematüre bebeklerin tanıtıcı özellik ve klinik durumları açısından gruplar arasındaki fark istatistiksel olarak anlamsız olup (p>0.05) gruplar homojen dağılmaktadır. Gruplar arası abdomen nem düzeyi ortalamaları incelendiğinde; hindistan cevizi yağı ve ayçiçek yağı uygulanan bebeklerin uygulamadan önce, 7.gün ve 14.gün abdomen nem düzeyi ortalamaları arasındaki farkın istatistiksel olarak anlamsız olduğu (p>0,05) belirlenmiştir. Gruplar arası bacak ve kol nem düzeyi incelendiğinde ise 14.günde hindistan cevizi yağı uygulanan grupta nem düzeyi ayçiçek yağı grubuna göre daha yüksek ölçülmüştür (p<0,05). Grup içi abdomen, bacak ve kol nem düzeyi ortalamaları incelendiğinde; her iki grupta da yenidoğanların uygulama öncesi nem düzeyleri ortalamalarının 7. gün ve 14. gün ölçümlerine göre daha düşük olduğu belirlenmiştir (p<0,001). Grup içi YCDD skoru ortalamaları incelendiğinde; her iki grupta da uygulama öncesi YCDD skoru ortalamalarının 14.gün ölçümlerine göre daha düşük olduğu belirlenmiştir (p<0,001). Gruplar arası vücut ağırlığı, boy uzunluğu ve baş çevresi ortalamaları incelendiğinde; her iki grupta uygulamadan önce, 7.gün ve 14.gün ortalamaları arasındaki farkın istatistiksel olarak anlamsız olduğu belirlenmiştir (p>0,05). Araştırma sonucunda preterm bebeklere cilt bakımı sırasında nemlendirici olarak ayçiçek ve hindistan cevizi yağı uygulamasının cilt nem düzeyini arttırmada ve cilt bütünlüğünün korunmasında etkili bir yöntem olduğu belirlenmiştir. Bununla birlikte nem düzeyini artırmada daha etkili bulunan hindistan cevizi yağının cilt bakımı sırasında doğal bir yöntem olarak kullanılması önerilebilir. The research was planned as a randomized controlled experimental study to determine the effect of skin care applied to preterm babies using two different oils on skin integrity and growth parameters. It was carried out between 1 October 2020 and 30 December 2021 at S.B.Ü Kocaeli DEAH 3rd Level Neonatal Intensive Care Unit. 'Newborn Descriptive and Clinical Characteristics Form' and 'Newborn Skin Condition Assessment Scale' were used to collect data. The data were evaluated with appropriate statistical methods in the SPSS 22.0 package program. Trial number (NCT04972747) retrieved from clinicaltrials.gov on 21.03.2022 mIn the study, the difference between the groups in terms of introductory features and clinical conditions of premature babies was statistically insignificant and the groups were homogeneously distributed (p>0.05). When the abdominal moisture level averages between the groups were examined; It was determined that the difference between the averages of abdominal moisture level on the 7th and 14th days before the application of coconut oil and sunflower oil was statistically insignificant (p>0.05). When the leg and arm moisture levels between the groups were examined, the moisture level in the group that was applied coconut oil on the 14th day was higher than the sunflower oil group (p>0.05). When the averages of abdominal, leg and arm moisture levels within the group were examined; In both groups, it was determined that the mean humidity levels of the newborns before the application were lower than the measurements on the 7th and 14th days (p<0,001). When the in-group YCDD score averages are examined; In both groups, it was determined that the mean YCDD score before the application was lower than the 14th day measurements (p<0.001). When the averages of body weight, height and head circumference between the groups were examined; It was determined that the difference between the averages of the 7th and 14th days before the application in both groups was statistically insignificant (p>0.05). As a result of the research, it was determined that the application of sunflower and coconut oil as moisturizers during skin care for preterm babies is an effective method in increasing the skin moisture level and protecting the skin integrity. However, it can be recommended to use coconut oil, which is more effective in increasing the moisture level, as a natural method during skin care.
