Abstract
Amaç: Bu çalışma Covid-19 döneminde Yenidoğan Yoğun Bakım Ünitesi (YYBÜ)'nde çalışan hemşirelerin karşılanamayan bakımları ile aile merkezli bakıma yönelik tutumları arasındaki ilişkiyi belirlemek amacıyla yapıldı. Yöntem: Çalışma tanımlayıcı ve ilişki arayıcı tasarım olarak Nisan – Temmuz 2021 tarihleri arasında İstanbul'da 3. Düzey YYBÜ bulunduran yedi kamu hastanesinde çalışan 182 hemşire ile yapıldı. Çalışmada hemşirelere yönelik tanıtıcı bilgi formu, Karşılanamayan Bakım Anketi-Pediatrik Formu ve Aile Merkezli Bakım Tutum Ölçeği kullanıldı. Covid-19 pandemi süreci nedeniyle hemşirelere veri toplama formları online olarak Google Forms aracılığıyla gönderildi. Çalışma için İstanbul Okan Üniversitesi etik kurulundan (Tarih: 10.03.2021; No: 134), Sağlık Bakanlığı'ndan, kurumlardan, çalışmaya katılan hemşirelerden ve ölçek sahiplerinden izin alındı. Bulgular: Hemşirelerin karşılanamayan bakımları arasında ilk üç sırada; 'ağız bakımı verme', 'gerekli tüm hemşirelik verilerinin tam olarak kaydedilmesi' ve 'vardiya değişimi veya devir teslim sırasında ilgili tüm bilgileri iletme' yer aldı (n=179, %98,35). Hemşirelerin karşılanamayan bakım nedenleri arasında en yüksek puanın iletişim alt boyutunda olduğu bulundu (2,50±0,74). Hemşirelerin Aile Merkezli Bakım Tutum Ölçeği puan ortalamalarının 4,35±0,43 olduğu belirlendi. Hemşirelerin Karşılanamayan Bakım puanları ile Aile Merkezli Bakım Tutum puanları arasında anlamlı bir ilişki olmadığı bulundu (p> 0,05). Sonuç: Hemşirelerin son vardiyasında en az bir hemşirelik bakımını karşılayamadıkları ve bunun nedeninin en çok iletişim ile ilgili problemlerin olduğu belirlendi. Covid-19 sürecinde ünite içi yapılan kısıtlamalara rağmen, hemşirelerin aile merkezli bakıma yönelik tutumlarının çok yüksek olduğu bulundu. Hemşirelerin karşılanamayan bakımlarını azaltmak için iletişim becerilerinin geliştirilmesine yönelik eğitim ve politikaların oluşturulması önerilebilir.
Objective: This study was conducted to determine the relationship between the misscare and attitudes towards family-centered care of nurses working in the Neonatal Intensive Care Unit (NICU) during the Covid-19 period. Method: The study was conducted as a descriptive and correlational design with 182 nurses working in seven public hospitals in 3rd Level NICU between April and July 2021 in Istanbul. The introductory information form for nurses, the MISSCARE Survey – Pediatric Version and the Family Centered Care Attitude Scale were used in the study. Due to the Covid-19 pandemic process, data collection forms were sent to nurses online via Google Forms. Permission for the study was obtained from the ethics committee of Istanbul Okan University (Date: 10.03.2021; No: 134), the from Ministry of Health, from institutions, from nurses participating in the study, and from scale owners. Results: Among the nurses' misscare, the first three are; These included 'Mouth care', 'Full documentation of all necessary data' and 'Communication of all relevant information during shift change or handover' (n=179, 98.35%). It was found that the highest score among nurses' reasons for misscare was in the communication sub-dimension (2.50±0.74). It was determined that the mean score of the Nurses' Family Centered Care Attitude Scale was 4.35±0.43. It was found that there was no significant relationship between the Nurses' Misscare scores and their Family Centered Care Attitude scores (p> 0.05). Conclusion: It was determined that the nurses missed at least one nursing care in their last shift and the reason for this was mostly communication related problems. Despite the in-unit restrictions during the Covid-19 process, nurses' attitudes towards family-centered care were found to be very high. In order to reduce the misscare of nurses, it can be suggested that education and policies should be developed for the development of communication skills.
