Gebelerde fiziksel aktivite düzeyinin inkontinans ve yaşam kalitesine etkisinin incelenmesı

Abstract
Bu araştırmada gebelerde fiziksel aktivite düzeyinin, üriner inkontinans ve yaşam kalitesi üzerine etkisi araştırıldı. Bununla birlikte trimester dönemlerine göre fiziksel aktivite ve yaşam kalitesi düzeyleri incelendi.Araştırmaya Ege Üniversitesi Kadın Hastalıkları ve Doğum Polikliniği'ne gelen 18-40 yaş arasında 303 gebe alındı. Gebelerin sosyodemografik ve klinik özellikleri kaydedildi.Gebelerin fiziksel aktivite seviyeleri Gebelik Fiziksel Aktivite Anketi (GFAA) ile, inkontinans şiddeti İnkontinans Şiddet İndeksi (İŞİ) ile, üriner inkontinansın yaşam kalitesine etkisi İnkontinans Etki Anketi-Kısa Formu (İEA-7) ile mesane fonksiyonları ve ürogenital semptomlar Ürogenital Disstres Envanteri (ÜDE-6)ile, genel yaşam kalitesi ise SF-36 ile değerlendirildi. Gebelerin trimester dönemlerine göre SF-36 fiziksel fonksiyon ve ağrı alt boyutlarında istatistiksel olarak anlamlıfark bulundu. SF-36 fiziksel rol güçlüğü alt boyutunda sezaryenle doğum yapan bireylerin skoru daha yüksek bulundu (p<0,05). SF- 36 fiziksel rol güçlüğü alt boyut skoru ile GFAA'nın spor/egzersiz alt boyut skoru dışındaki tüm parametreler arasında düşük ve orta şiddetli korelasyon olduğu görüldü (p<0,05). SF-36'nın emosyonel rol güçlüğü alt boyut skoru ile GFAA'nın sedanter aktivite alt boyut skoru arasında düşük düzeyde korelasyon olduğu bulundu (p<0,05). İEA-7 ile GFAA -Mesleki aktiviteler arasında düşük dereceli negatif korelasyon olduğu gözlendi (p<0,01). ÜDE-6, GFAA -Sedanter Aktivite ve GFAA -Ev İşleri ile zayıf korelasyona sahipti (p<0,05). SF36-Fiziksel Rol Güçlüğü, GFAA -Şiddetli Aktivite ve GFAA -Spor/Egzersiz ile anlamlı negatif korelasyon gösterdi (p<0,05). SF36-Enerji/Vitalite ile GFAA -Sedanter Aktivite ve GFAA -Ev İşleri'nde de negatif zayıf korelasyon olduğu belirlendi (p<0.05).Gebelerde fiziksel aktivite tipi ile inkontinansa özel yaşam kalitesi arasında düşük düzeyde negatif ilişki vardır. Gebelerde fiziksel aktivite tipi ürogenital semptomlar arasında negatif düşük düzeyde korelasyon vardır. Gebelerde fiziksel aktivite şiddeti ve tipi genel yaşam kalitesinin bazı alt boyutlarını düşük düzeyde etkilemektedir.
In this study, the effect of physical activity level on urinary incontinence and quality of life in pregnant women was investigated. In addition, physical activity and quality of life levels were examined according to trimesters.303 pregnant women between the ages of 18-40 who came to the Ege University Gynecology and Obstetrics Polyclinic were included in the study. Sociodemographic and clinical characteristics of the pregnant women were recorded. Physical activity levels of pregnant women with Pregnancy Physical Activity Questionnaire (GFAA), incontinence severity with Incontinence Severity Index (ISI), the effect of urinary incontinence on quality of life Incontinence Impact Questionnaire-Short Form (IEA-7), bladder functions and urogenital symptoms Urogenital Distress Inventory (UDE) -6) and overall quality of life with SF-36.A statistically significant difference was found in SF-36 physical function and pain sub-dimensions according to the trimesters of the pregnant women. In the SF-36 physical role difficulty sub-dimension, individuals who gave birth by cesarean section had a higher score (p<0.05).It was observed that there was a low and moderate correlation between the SF-36 physical role difficulty sub-dimension score and all parameters except the sport/exercise sub-dimension score of GFAA (p<0.05).It was found that there was a low level of correlation between the emotional role difficulty sub-dimension score of SF-36 and the sedentary activity sub-dimension score of GFAA (p<0.05).A low-grade negative correlation was observed between IEA-7 and GFAA-Occupational activities (p<0.01). UDE-6 was weakly correlated with GFAA - Sedentary Activity and GFAA - Housework (p<0.05). SF36 - Physical Role Difficulty, GFAA - Vigorous Activity and GFAA - Sport/Exercise were significantly negatively correlated (p<0.05).There was also a weak negative correlation between SF36-Energy/Vitality and GFAA - Sedentary Activity and GFAA - Housework (p<0.05).There is a low negative correlation between physical activity type and incontinance spesific quality of life in pregnant women. There is a low negative correlation between physical activity type and urogenital symptoms in pregnant women. The severity and type of physical activity in pregnant women affect some sub- dimensions of the general quality of life at a low level.
