Bronkoloji ünitesinde kapsamlı hemşirelik hizmetinin operasyon öncesi ve sonrası anksiyete, hemşirelik bakım kalitesi üzerine etkisi
Abstract
Bu araştırmanın amacı; bronkoloji ünitesinde ayaktan bronkoskopi işlemi yapılacak hastalara verilecek olan kapsamlı hemşirelik hizmetinin hastaların anksiyete seviyelerine ve hemşirelik bakım kalitesinden memnuniyet düzeylerine etkisinin belirlenmesidir. Araştırma, tek kör randomize kontrollü tipte, Nisan 2021 – Nisan 2022 tarihleri arasında Bronkoloji Ünitesine başvuran 18 yaş ve üzeri hastalarda yapılmıştır. Deney grubu 88 hasta, kontrol grubu 88 hasta olmak üzere 176 katılımcı örneklemi oluşturmuştur. Verilerin toplanmasında anket formu, Beck Anksiyete Ölçeği ve Hemşirelik Bakım Kalitesine İlişkin Hasta Memnuniyeti Anketi kullanılmıştır. Verilerin istatistiksel analizi IBM SPSS Statistics Version 22. Programı ile yapılmıştır. Araştırmada, deney ve kontrol grupları arasında Beck Anksiyete Ölçeğinden aldıkları puan açısından istatistiksel olarak anlamlı bir fark bulunmuştur (F=6,641, p=0,02). Ayrıca ölçekten alınan puanların deney grubu (X2=33,914; p < 0,001) ve kontrol grup içi (X2=47,772; p < 0,001) değerlendirmesi yapıldığında farkın anlamlı olduğu ve deney grubundaki katılımcıların başlangıç puanlarının kontrol grubuna göre istatistiksel olarak daha düşük olduğu saptandı (U=2326, 500; p <0,001). Deney ve kontrol grupları arasında hemşirelik bakım kalitesi memnuniyetinden aldıkları puan açısından istatistiksel olarak anlamlı bir fark bulunmamaktadır (p> 0,05). Fark tespit edilmemekle birlikte deney grubunda ki katılımcıların hemşirelik bakım kalitesinden memnuniyet düzeyleri kontrol grubuna göre daha yüksekti. Çalışmamızda gruplar arasında bronkoskopi işlemi öncesi ve sonrası yaşam bulguları kabul edilebilir aralıkta olup, gruplar arasında anlamlı bir farklılık tespit edilmemiştir. Sonuçlar neticesinde, kapsamlı hemşirelik hizmetinin bronkoskopi olan hastaların anksiyete düzeylerini düşürmesine rağmen hemşirelik bakımından memnuniyetleri üzerine etkisi olmadığı görülmüştür. Bu bağlamda, ayaktan tedavi alan hastalarda hemşirelik bakım hizmetinden memnuniyeti arttırmak amacıyla yeni uygulamalar geliştirilmesine ihtiyaç duyulduğu öngörülmektedir.
The aim of this study is to determine the effect of the comprehensive nursing service to patients to be operated at outpatient bronchology unit, on their anxiety levels and satisfaction with the quality of nursing care. The study was a single-blind randomized controlled study. Participants aged 18 years and older were recruited from the Bronchology Unit between April 2021 and April 2022. The sample consisted of 176 participants, 88 patients assigned as a experimental group and 88 patients assigned as a control group. Questionnaire form, Beck Anxiety Inventory and Patient Satisfaction with Nursing Care Quality Questionnaire were used to collect data. Statistical analysis of the data was performed with IBM SPSS Statistics Version 22. Program. In the study, a statistically significant difference was found between the experimental and control groups in terms of the scores of the Beck Anxiety Inventory (F=6.641, p=0.02). In addition, there were a significant within group differences in the experimental group (X2=33,914; p < 0.001) and the control group (X2=47.772; p < 0.001). The initial scores of the participants in the experimental group were statistically lower than the control group (U=47.772; p < 0.001). There was no statistically significant difference between the experimental and control groups in terms of the scores of satisfaction of nursing care quality (p> 0.05). Although no difference was found, participants' satisfaction of nursing care quality who were in the experimental group were higher than control group. In our study, vital signs of patients were within the acceptable rate before and after the procedure, no significant difference was found between the groups. As a result, comprehensive nursing service reduced the anxiety levels of patients with bronchoscopy, however, it did not have an effect on their satisfaction with nursing care. In this context, it was predicted that there is a need to develop new practices in order to increase satisfaction with nursing care in outpatients clinics.
