Abstract
Merhamet yorgunluğu bakım verme rolleriyle tanınan hemşireler arasında yaygın olarak görülmektedir. Merhamet yorgunluğu yaşayan hemşireler zararlı fiziksel, ruhsal ve duygusal davranışlar sergilemektedir. Bu olumsuz davranışlar, hemşirelerin performanslarının düşmesinin yanı sıra bakım davranışlarına da olumsuz şekilde etki edebilmektedir. Bu çalışma hemşirelerde merhamet yorgunluğu ve bakım davranışları arasındaki ilişkinin incelenmesi amacıyla kesitsel tasarımda planlanmıştır. Çalışmanın evrenini İstanbul Asya yakasında Sağlık Bakanlığına bağlı üç hastanede hizmet veren hemşireler oluşturmuştur (N:1.620). Örneklem seçiminde, evreni bilinen örneklem hesabı kullanılmış olup, %95 güvenirlik düzeyinde minimum örneklem hacmi 278 olarak hesaplanmıştır. Bu kapsamda örneklem bir Acil Durum Hastanesi (126), iki Eğitim ve Araştırma Hastanesi (113) ve (107) 'de görev yapan hemşirelerden oluşmaktadır (N:346). Araştırma katılıma gönüllü olan, belirtilen hastanede görev yapmakta olan hemşireler ile yüz yüze olarak gerçekleştirilmiştir. Bu araştırmada verilerin toplanmasında Hemşire Mesleki ve Kişisel Bilgiler, Merhamet Yorgunluğu- Kısa Ölçeği (MY-KÖ) ve Bakım Davranışları Ölçeği- 30 kullanılmıştır. Verilerin analizinde SPSS analiz programı kullanılmıştır. Araştırma sonucunda katılımcıların %78,6'sı kadın, yaş ortalaması 27,97±4,64, haftalık çalışma saati 47,22±10,52'dir ve %83,5'i vardiyalı çalıştığı ortaya konmuştur. Ayrıca katılımcıların %33,5'i kurumda çalışmaktan, %13,0'ü birimde çalışmaktan memnun değildir. Katılımcıların merhamet yorgunluğu ölçek puanı 60,62±23,26, bakım davranışları ölçek puanı 159,37±20,69 olarak bulunmuştur. Kadın katılımcıların, kurumda ve birimde çalışmaktan memnun olmayanların merhamet yorgunluğunun yüksek olduğu, memnun olanların bakım davranışları puanlarının yüksek olduğu (p<0,05) görülmüştür. Çalışmadan elde edilen veriler sonucunda merhamet yorgunluğu ve bakım davranışları ölçek ve alt boyut skorları arasında istatistiksel olarak anlamlı bir ilişki bulunmamaktadır. Diğer yandan haftalık çalışma saati ile merhamet yorgunluğu ve bakım davranışları ölçek skorları arasında pozitif yönde istatistiksel olarak anlamlı derecede bir ilişki bulunduğu ortaya konmuştur (p<0,05). Bu çalışma sonucunda hemşirelerin merhamet yorgunluğunun orta düzeyde olduğu, kadınların ve çalıştığı birimden memnun olmayanların yüksek merhamet yorgunluğu yaşadığı söylenebilir. Bu kapsamda hemşirelerin mevcut çalışma ortamlarından memnuniyetlerinin artmasını sağlayacak yönetsel düzenlemeler ile merhamet yorgunluğunun önüne geçilerek bakım kalitesinin artması sağlanabilir. Hemşirelerde merhamet yorgunluğuna ilişkin kapsamlı ampirik veriler elde edilmesi ve literatüre kazandırılması, geleceğin sağlık politikalarının şekillendirilmesi adına önerilir.
Compassion fatigue is common among nurses who are recognised for their caregiving role. Nurses experiencing compassion fatigue exhibit harmful physical, mental and emotional behaviours. These negative behaviours may negatively affect nurses' care behaviours as well as their performance. This study was planned in a cross-sectional design to examine the relationship between compassion fatigue and care behaviours in nurses. The study population consisted of nurses in three hospitals affiliated with the Ministry of Health on the Asian side of Istanbul (N: 1,620). The minimum sample volume was calculated as 278 at a 95% reliability level with the sample calculation with the known population. In this context, the sample consists of nurses working in one Emergency Hospital (126), two Training and Research Hospitals (113) and (107) (N: 346). The research was conducted face-to-face with the nurses who volunteered to participate and worked in the specified hospitals. In this study, data was collected by Nurse Professional and Personal Information, Compassion Fatigue - Short Scale (CF-SC) and Caring Behaviours Scale-30. SPSS program was used to analyse the data. 78.6% of the participants are women; the average age is 27.97±4.64, the weekly working hours are 47.22±10.52, and 83.5% work in shifts. Additionally, 33.5% are unsatisfied with working in the institution, and 13.0% are unsatisfied with working in the unit. The participants' compassion fatigue scale score is 60.62±23.26, and their caring behaviour scale score is 159.37±20.69. It was observed that female participants who were dissatisfied with working in the institution and unit had high compassion fatigue, and those who were satisfied had high care behaviour scores (p<0.05). As a result of the data obtained from the study, there is no statistically significant relationship between compassion fatigue and caring behaviours scale and subscale scores. It was revealed that there was a statistically significant positive relationship between weekly working hours and compassion fatigue and caring behaviours scale scores (p<0.05). As a result of this study, it can be said that nurses' compassion fatigue is at a medium level, and women and those who are dissatisfied with the unit they work in experience high compassion fatigue. It has been observed that increasing weekly working hours is associated with increased compassion fatigue and negative attitudes toward caring behaviours. In this context, managerial arrangements that will increase nurses' satisfaction with their current working environment can prevent compassion fatigue and improve care quality. Obtaining comprehensive empirical data on compassion fatigue in nurses and contributing it to the literature is recommended to shape future health policies.
