Abstract
Bu çalışmanın amacı rotator cuff sendromlu (RCS) hastalarda skapular retraksiyon egzersizlerinin propriyosepsiyon, omuz fonksiyonları ve skapular hareketlilik üzerine etkisini araştırmaktır. Çalışma Bafra Devlet Hastanesi Fizik Tedavi ve Rehabilitasyon Ünitesine başvuran RCS tanısı konmuş 25 – 55 yaş aralığında 40 hasta üzerinde yapıldı. Tedavi grubuna (n=20; 5gün/3hafta) elektroterapi sonrası Wand egzersizleri, dirençli egzersiz bandı ile omuz kuvvetlendirme ve skapular retraksiyon egzersizleri verilirken kontrol grubuna (n=20; 5gün/3hafta) elektroterapi sonrası sadece Wand egzersizleri ve dirençli egzersiz bandı ile omuz kuvvetlendirme egzersizleri verildi. Değerlendirmeler tedavi öncesi ve sonrası yapıldı. Katılımcıların ağrı değerleri vizüel analog skala; eklem hareket açıklığı gonyometre; kas kuvveti manuel kas testi; skapular hareketlilik ise akromiyon uzaklığı, lateral skapular kayma testi ve skapular izometrik sıkıştırma testi ile; propriyosepsiyon lazer imleçli açı tekrarlama testi ile; genel sağlık durumları Nothingham Sağlık profili (NSP) ile ve omuzun fonksiyonel değerlendirmesi ise Western Ontario Rotator Cuff (WORC) İndeksi ile değerlendirildi. Çalışmada, grup içi değerlendirmelerde her parametre için istatistiksel olarak anlamlı fark gözlenmiştir (p<0,05). Gruplar arasında tedavi öncesi ve sonrası ağrı, kas kuvveti değerleri, akromiyon uzaklığı değerleri, lateral skapular kayma testi sonuçları, skapular izometrik sıkıştırma testi, eklem pozisyon hissi ve NSP değerleri arasında istatistiksel olarak anlamlı bir fark bulunmamıştır (p>0.05). Gruplar arasında tedavi öncesi ve sonrası kıyaslamalarda tedavi sonrası eklem hareket açıklığı değerleri ve WORC değerleri arasında tedavi grubu lehine istatistiksel olarak anlamlı bir fark bulunmuştur (p<0,05). Çalışmanın sonuçları RCS tedavisinde rehabilitasyona eklenen skapular retraksiyon egzersizlerinin eklem hareket açıklığına ve omuz fonksiyonelliğine olumlu etkileri olduğunu göstermiştir. The aim of this study is to investigate the effects of scapular retraction exercises on proprioception, shoulder functions and scapular mobility in patients with rotator cuff syndrome (RCS). The study was conducted on 40 patients between the ages of 25 and 55 who were diagnosed with RCS and applied to the Bafra State Hospital Physical Therapy and Rehabilitation Unit. Wand exercises, shoulder strengthening and scapular retraction exercises with a resistance exercise band were given to the treatment group (n=20; 5days/3weeks) after electrotherapy, while the control group (n=20; 5days/3weeks) was given only Wand exercises and shoulder strengthening with a resistance exercise band after electrotherapy. Evaluations were made before and after treatment. Pain values of the participants with visual analog scale; joint range of motion with a goniometer; muscle strength with manual muscle test; scapular mobility was determined by acromion distance, lateral scapular slide test and scapular isometric squeeze test; proprioception by laser pointer angle repetition test; general health status was assessed with the Nottingham Health profile (NSP) and functional assessment of the shoulder was evaluated with the Western Ontario Rotator Cuff (WORC) Index. In the study, a statistically significant difference was observed for each parameter in group evaluations (p<0.05). There was no statistically significant difference between the groups in pain, muscle strength values, acromion distance values, lateral scapular slip test results, scapular isometric compression test, joint position sense and NSP values between the groups (p>0.05). In the comparisons between the groups before and after the treatment, a statistically significant difference was found between the joint range of motion values and WORC values after the treatment in favor of the treatment group (p<0.05). The results of the study showed that scapular retraction exercises added to rehabilitation in RCS treatment have positive effects on joint range of motion and shoulder functionality.
