Abstract
Bu araştırma, hemodiyaliz hastalarının arteriovenöz fistül durumlarının ve öz bakımlarının değerlendirilmesi amacıyla tanımlayıcı türde yapılmıştır. Araştırmanın evreni Ağrı ilinde bir eğitim ve araştırma hastanesinde fistül ile hemodiyaliz tedavisi alan 85 hasta olup örneklemde evrenin tamamına ulaşılmıştır. Araştırma 21.06.2021-28.07.2021 tarihleri arasında Hasta Tanıtım Formu, Hemodiyaliz Tedavisi Alan Hastalarda Arteriyovenöz Fistül Değerlendirme Ölçeği, Arteriyovenöz Fistüle İlişkin Öz Bakım Davranışlarını Değerlendirme Ölçeği kullanılarak yürütülmüştür. Veriler SPSS 24.0 programı kullanılarak analiz edilmiştir. Verilerinin normal dağılıma uygunluğunu değerlendirmek için Kolmogorow-Smirnov testi kullanılmıştır. Verilerin analizinde tanımlayıcı veriler sayı yüzdelik hesaplamaları, One way Anova, bağımsız gruplarda t testi, Kruskal Wallis testi ve Pearson korelasyon analizi kullanılmıştır. Elde edilen bulgular %95 güven aralığında ve %5 anlamlılık düzeyinde değerlendirilmiştir. İstatistiksel anlamlılık için p<0,05 düzeyi kabul edilmiştir. Araştırmada hastaların Arteriovenöz Fistül Değerlendirme Ölçeği'nden 11,90± 5,56 puan, Arteriyovenöz Fistüle İlişkin Öz Bakım Davranışlarını Değerlendirme Ölçeği'nden ise 55,17±11,76 puan aldıkları, iki ölçek arasında istatistiksel olarak negatif yönde anlamlı ilişki olduğu saptanmıştır (p<0,05). Hastaların kronik böbrek yetmezliği nedeni, fistülün bulunduğu bölge, fistül varlığı süresi ve hemodiyaliz tedavi sıklığına göre Arteriovenöz Fistül Değerlendirme Ölçeği'nden aldıkları puanlar arasında istatistiksel olarak anlamlı farklılık bulunmuştur (p<0,05). Araştırma kapsamındaki hastaların eğitim düzeyine, fistül bakımını yapan bireye göre Arteriyovenöz Fistüle İlişkin Öz Bakım Davranışlarını Değerlendirme Ölçeği'nden aldıkları puanlar arasında istatistiksel olarak anlamlı farklılık bulunmuştur (p<0,05). Bu araştırma sonucunda hastaların arteriovenöz fistülün komplikasyon riskinin az olduğu, özbakım davranışlarının orta düzeyde olduğu, arteriovenöz fistülün komplikasyon riskinin özbakım davranışlarını negatif yönde etkilediği bulunmuştur. Araştırmanın daha büyük örneklem ile tekrar edilmesi önerilmektedir
This descriptive study was conducted to evaluate the arteriovenous fistula status and self-care of hemodialysis patients. The population of the study consisted of 85 patients who received hemodialysis treatment with fistula in a training and research hospital in the province of Ağrı, and the entire population was reached in the sample. The study was conducted between 21.06.2021 and 28.07.2021 by using the Patient Information Form, the Arteriovenous Fistula Evaluation Scale in Patients Receiving Hemodialysis Treatment, and the Arteriovenous Fistula-Related Self-Care Behaviors Evaluation Scale. The data were analyzed using the SPSS 24.0 program. Kolmogorow-Smirnov test was used to evaluate the suitability of the data for normal distribution. In the analysis of the data, descriptive data number percentage calculations, One way Anova, independent groups t test, Kruskal Wallis test and Pearson correlation analysis were used. The findings were evaluated at 95% confidence interval and 5% significance level. A p<0.05 level was accepted for statistical significance. In the study, it was determined that the patients got 11.90±5.56 points from the Arteriovenous Fistula Evaluation Scale and 55.17±11.76 points from the Arteriovenous Fistula-Related Self-Care Behaviors Evaluation Scale, and there was a statistically negative significant correlation between the two scales ( p<0.05). A statistically significant difference was found between the scores of the patients from the Arteriovenous Fistula Evaluation Scale according to the cause of chronic renal failure, the region of the fistula, the duration of the presence of fistula and the frequency of hemodialysis treatment (p<0.05). A statistically significant difference was found between the scores obtained from the Arteriovenous Fistula Self-Care Behaviors Assessment Scale according to the education level of the patients within the scope of the study and the individual who cares for the fistula (p<0.05). As a result of this study, it was found that the risk of complications of arteriovenous fistula of the patients was low, their self-care behaviors were moderate, and the risk of complications of arteriovenous fistula negatively affected their self-care behaviors. It is recommended to repeat the study with a larger sample.
