Abstract
Amaç: İnsanda en sık görülen anomalilerden biri olan konjenital diş eksikliği, bir veya daha fazla diş germinin hiç oluşmaması sonucu ağızdaki diş sayısının normalden daha az olması olarak tanımlanmaktadır. Konjenital diş eksikliğinin etiyolojisi tam olarak bilinmiyor olsa da, dişlerin gelişim aşamalarında meydana gelen çevresel ve genetik nedenlere bağlı olarak meydana geldiği düşünülmektedir. Eksikliği en sık görünen maksiller lateral dişler, fonasyon problemleri ve estetik kaygılara neden olduğundan bireylerin sosyal hayatlarını etkilemekte ve ortodontik tedaviye başvurmalarına neden olmaktadır. Ortodontik tedavi seçenekleri olarak, eksik lateral diş boşluğunu kapatarak kanin substitüsyonu, yeterli diş boşluğu açarak protetik/restoratif restorasyonlar veya dental implant ve protetik restorasyon değerlendirilebilmektedir. Tedavi yönteminin belirlenmesi vaka özelinde olmalı ve ideal tedavinin estetik ve fonksiyonel ihtiyaçları karşılayan en konservatif seçenek olması gerektiği göz önünde bulundurulmalıdır. Gereç ve Yöntemler: Bu tez çalışması Marmara Üniversitesi Tıp Fakültesi Klinik Araştırmalar Etik Kurulu tarafından (09.2022.1170) onaylanmıştır. Çalışmaya 2018-2021 yılları arasında İstanbul Okan Üniversitesi, Diş Hekimliği Fakültesi, Ortodonti Ana Bilim Dalı kliniğinde, konjenital üst lateral diş eksikliği, modifiye edilmiş TPA protokolü ile tedavi görmüş ve çalışmaya dahil edilme kriterlerine uygun olan hastaların arşivdeki tedavi başlangıç ve bitiminde alınmış sefalometrik röntgen ve üç boyutlu tarama verileri dahil edilmiştir. Hastaların tedavi öncesi ve sonrası sefalometrik röntgen ölçümleri ve üç boyutlu dijital modellerdeki açıları karşılaştırılmıştır. Tüm sefalometrik röntgenler Planmeca ProMax® 3D Mid cihazı (Planmeca, Helsinki, Finlandiya) cihazı kullanılarak elde edilmiştir. Dijital modeller ise Trios® (3SHAPE A/S, Danimarka) Trios® 3 kullanılarak elde edilmiştir. Elde edilen sefalometrik röntgenlerin analizinde NemoCeph sefalometrik yazılım kullanılmıştır. İstatistiksel analizler için IBM® SPSS® Statistics 22 programı kullanılmıştır. Parametrelerin normal dağılıma uygunluğu Shapiro Wilks testi ile değerlendirilmiştir. Çalışma verileri değerlendirilirken tanımlayıcı istatistiksel metotların (minimum, maksimum, ortalama, standart sapma, medyan) yanı sıra niceliksel verilerin karşılaştırılmasında Wilcoxon işaret testi kullanılmıştır. Anlamlılık p<0,05 düzeyinde değerlendirilmiştir. Bulgular: Çalışmada üçü (%25) erkek, dokuzu (%75) kadın olmak üzere 12 hasta verisi kullanılmıştır. Hastaların tedavi öncesi ve sonrası verileri karşılaştırıldığında, U6-VD (p:0,002), U6-HD (p:0,002), I-VD (p:0,004), Overjet (p:0,006), NLA (p:0,003), Į-Ī (p:0,002), Mx Hayce-Nance (p:0,002), 3.Ruga-UR6 (p:0,002), 3.Ruga-UL6 (p:0,002), UR6 Rotasyon (p:0,005), UL6 Rotasyon (p:0,002) değerlerindeki düşüşün istatistiksel olarak anlamlı olduğu görülmüştür. Sonuç: Bu çalışmanın sonuçlarından elde edilen veriler, bu tedavi yönteminin alternatif bir tedavi yöntemi olarak klinik uygulamada kullanılmasının yararlı olacağını ortaya koymuştur. Ancak daha kesin sonuçların elde edilmesi için daha büyük hasta grupları ile yapılacak çalışmaların literatüre katkıda bulunacağı açıktır. Objectives: Congenital absence of teeth, a frequent human abnormality, is characterized by a reduced number of teeth as a result of missing one or more tooth germs. The causes of this anomaly are not entirely known, but they are believed to be due to environmental and genetic factors that arise during tooth development. The absence of maxillary lateral incisors is a common condition that can have an impact on a person's speech and appearance, which can affect their social life and lead them to seek orthodontic treatment. Canine substitution for closing the missing lateral tooth gap, prosthetic/restorative restorations by opening enough tooth space, or dental implant and prosthetic restoration can be assessed as orthodontic treatment alternatives. The objective of this thesis is to assess the efficacy of treating patients with congenital lateral deficiency through maxillary dentoalveolar mesialization by conducting pre- and post-treatment scans of cephalometric x-rays and three-dimensional digital models. Materials and Methods: This thesis study was approved by the Marmara University Faculty of Medicine Clinical Research Ethics Committee (09.2022.1170). The investigation incorporated cephalometric X-rays and three-dimensional scan data obtained from the archive of patients who met the inclusion criteria and received a modified TPA protocol for congenital upper lateral tooth deficiency at the Department of Orthodontics, Faculty of Dentistry, Istanbul Okan University between 2018 and 2021. The pre-and post-treatment cephalometric X-ray measurements and angles in three-dimensional digital models were compared. Cephalometric X-rays were acquired through the use of the Planmeca ProMax® 3D Mid equipment (Planmeca, Helsinki, Finland) while digital models were produced using Trios® 3 device (3SHAPE A/S, Denmark). Subsequently, NemoCeph software was employed to analyze the cephalometric X-rays. All statistical analyses were undertaken using the IBM® SPSS® Statistics 22 programme. The Shapiro-Wilks test was utilized to assess the conformity of parameters to a normal distribution. In addition to using descriptive statistical methods (including minimum, maximum, mean, standard deviation, and median), we utilized the Wilcoxon signed-rank test to compare quantitative data. All tests of significance were evaluated at a level of p<0.05. Results: Data from 12 patients, comprising three (25%) males and nine (75%) females, were utilized in the research. Statistically significant decreases were observed in the values U6-VD (p:0,002), U6-HD (p:0,002), I-VD (p:0,004), Overjet (p:0,006), NLA (p:0,003), Į-Ī (p:0,002), Mx Hayce-Nance (p:0,002), 3.Ruga-UR6 (p:0,002), 3.Ruga-UL6 (p:0,002), UR6 Rotation (p:0,005), UL6 Rotation (p:0,002) in comparing pre- and post-treatment data. Conclusion: The results from this study indicate that utilizing this treatment approach as an alternative in clinical practice would be beneficial. Nevertheless, it is evident that conducting further research with larger patient cohorts would enhance the literature for obtaining more accurate outcomes.
