Abstract
Ameliyat sırasında istenmeyen hipotermi ameliyat sonrası hipotermiye neden olabilmektedir. İstenmeyen hipoterminin önlenmesinde aktif yöntemlerden birisi olan ısıtılmış irrigasyon sıvısı uygulanmaktadır. Bu araştırma sonucu total laparoskopik histerektomi ameliyatında ısıtılmış irrigasyon sıvısının ameliyat sonrası hipotermi üzerine etkisinin incelenmesi amacıyla yapıldı. Bu araştırma sonucu 01.07.2020–01.09.2021 tarihleri arasında Zeynep Kâmil Kadın ve Çocuk Hastalıkları Eğitim Araştırma Hastanesi' nde total laparoskopik histerektomi ameliyatı olan 64 hasta ile randomize kontrollü türde gerçekleştirildi. Verilerin değerlendirilmesi IBM SPSS 21 programı ile yapıldı. İstatiksel analizde sayı, yüzde, ortalama, standart sapma ve medyan değerleri, verilerin normal dağılım durumuna bağımsız iki grubun karşılaştırmalarında bağımsız gruplarda örneklem t test, Mann Whitney U testi, bağımlı iki grubun karşılaştırmasında ise bağımlı örneklem t testi, tekrarlı ölçümlerin karşılaştırılmasında çok yönlü varyans analizleri kullanıldı. Araştırmaya katılan müdahale ve kontrol grubu hastaların sosyodemografik ve tıbbi özelliklerine göre homojen oldukları (p>0.05), ameliyat sırasında 30., 45., 60., 120. ve 150. dakikalarda vücut sıcaklığı arasında istatistikselsel olarak anlamlı fark olduğu bulundu (p<0.05). Müdahale ve kontrol grubu hastaların ameliyat sonrası 15., 30., 45. ve 60.dakikada ölçülen vücut sıcaklığı arasında istatistikselsel olarak anlamlı fark olduğu saptandı (p<0.05). Ameliyat sonrası dönemde, tekrarlı ölçümlerde çok yönlü varyans analizi sonucunda hastaların vücut sıcaklığı ortalamaları arasında grup (F=146.117, p=0,000), zaman (F=93.497, p=0,000) ve grup/zaman (F=7.105, p=0,001) etkileşimi açısından istatistikselsel olarak anlamlı bir fark olduğu saptandı (p<0.05). Bu araştırmada total laparoskopik histerektomi ameliyatında ısıtılmış irrigasyon sıvısının ameliyat sonrası hipotermi üzerine etkisinin olduğu sonucuna ulaşıldı. Undesirable hypothermia during surgery may cause post-operative hypothermia. Heated irrigation fluid, which is one of the active methods in the prevention of undesirable hypothermia, is applied. This study was conducted to investigate the effect of heated irrigation fluid on postoperative hypothermia in total laparoscopic hysterectomy surgery. This study was carried out in a randomized controlled type with 64 patients who had total laparoscopic hysterectomy surgery between 01.07.2020 - 01.09.2021 at Zeynep Kamil Gynecology and Pediatrics Training and Research Hospital. Evaluation of the data was done with IBM SPSS 21 program. Number, percentage, mean, standard deviation and median values in statistical analysis, sample t test in independent groups in comparison of two groups independent of the normal distribution of the data, Mann Whitney U test in comparison of two dependent groups, dependent sample t test in comparison of two dependent groups, multidirectional analysis of variance in comparison of repeated measurements used. It was found that the intervention and control group patients participating in the study were homogeneous according to their sociodemographic and medical characteristics (p>0.05), and there was a statistically significant difference between the body temperature at the 30th, 45th, 60th, 120th and 150th minutes during the operation (p<0.05). A statistically significant difference was found between the body temperature measured at the 15th, 30th, 45th and 60th minutes after the intervention and control group patients (p<0.05). In the postoperative period, as a result of multi-directional analysis of variance in repeated measurements, the mean body temperature of the patients included group (F=146.117, p=0.000), time (F=93.497, p=0.000) and group/time (F=7.105, p=0.001). There was a statistically significant difference in terms of interaction (p<0.05). In this study, it was concluded that heated irrigation fluid had an effect on postoperative hypothermia in total laparoscopic hysterectomy surgery.
