Tiroid hastalarında COVİd-19 korkusu ve uyku kalitesinin incelenmesi

Abstract

Bu araştırmada, Tiroid hastalarında COVID-19 korkusu ve uyku kalitesinin incelenmesi amaçlanmıştır. Araştırma, İstanbul'da bulunan bir eğitim ve araştırma hastanesinde girişimsel radyoloji bölümüne tiroid ince iğne aspirasyon biyopsisi için başvuran 400 tiroid hastası üzerinde gerçekleştirilmiştir. Veriler etik kurul onayı ve kurum izinleri alındıktan sonra Ekim 2020-Mart 2021 tarihleri arasında elde edilmiştir. Veriler; Hasta Bilgi Formu, Koronavirüs (COVID-19) Korkusu Ölçeği ve Pittsburgh Uyku Kalitesi İndeksi ile toplanmıştır. Elde edilen veriler IBM SPSS 21.0 versiyon istatistik programı ile değerlendirilmiştir. Kolmogorov-Smirnov testi sonucunda verilerin normal dağılım göstermediği belirlenmiş olup sıklık tabloları, Mann-Whitney U testi, Kruskal-Wallis H testi, Spearman Korelasyon analizleri kullanılmıştır. Sonuçlar yorumlanırken anlamlılık düzeyi olarak p<0,05 kabul edilmiştir. Araştırma kapsamındaki tiroid hastalarının Koronavirüs (COVID-19) Korkusu Ölçeği puan ortalaması 31,12±6,68 ve Pittsburgh Uyku Kalitesi İndeksi puan ortalaması 13,99±3,44 olarak bulunmuştur. Tiroid hastalarında koronavirüs korkusu arttıkça uyku kalitesi düzeyinin etkilendiği görülmektedir. Katılımcıların yaş ortalaması 47,79±13,65 (Min=18; Mak=82) olup, %81,25'i (n=325) kadındır. Sosyodemografik özelliklere göre koronavirüs korkusu düzeyleri ve PUKİ toplam puanları incelendiğinde evli olanların puanlarının bekar olanlara ve çocuğu olanların puanlarının çocuğu olmayanlara göre anlamlı derecede yüksek olduğu (p<0,05); eşi veya arkadaşı ile birlikte yaşayanların puanlarının, anne-babası ya da eşi-çocukları ile birlikte yaşayanların puanlarına göre anlamlı derecede yüksek olduğu (p<0,05); okuryazar olan veya ilkokul mezunu olanların puanlarının, lise veya üniversite mezunlarına göre anlamlı derecede yüksek olduğu (p<0,05), herhangi bir işte çalışmayanların puanlarının, maaşlı çalışan veya işsiz olanların puanlarına göre anlamlı derecede yüksek olduğu (p<0,05) saptanmıştır. Araştırma sonucunda, pandemi döneminde tiroid hastalarının COVID-19 hastalığına yakalanma korkusu arttıkça sağlıklı yaşamın bir parçası olan uyku kalitesinin bozulduğu belirlenmiştir. Araştırma sonuçlarının hemşirelik bakımının sunumu ve hasta/hasta yakını eğitimi açısından klinik alana yarar sağlayacağı, farkındalığı artıracağı, hemşirelere ve kuruma yol göstereceği düşünülmektedir.

This study aimed to examine the fear of COVID-19 and sleep quality in Thyroid Patients. The study was carried out on 400 patients who applied for thyroid fine-needle aspiration biopsy at the interventional radiology department in a training and research hospital in Istanbul. The data were obtained between October 2020 and March 2021, after obtaining the ethics committee's approval and institutional permissions. Data were collected with the descriptive information form, the Coronavirus (COVID-19) Fear Scale, and the Pittsburgh Sleep Quality Scale. The obtained data were evaluated with the IBM SPSS 21.0 version statistical program. As a result of the Kolmogorov-Smirnov test, it was determined that the data did not show normal distribution, and frequency tables, Mann-Whitney U test, Kruskal-Wallis H test, and Spearman Correlation analyzes were used. While interpreting the results, p<0.05 was accepted as the level of significance. The mean score of the Coronavirus (COVID-19) Fear Scale of thyroid patients within the scope of the study was 31.12±6.68 and the mean score of the Pittsburgh Sleep Quality Index was found 13.99±3.44. It is seen that the level of sleep quality is affected as the fear of coronavirus increases in thyroid patients. The mean age of the participants was 47.79±13.65 (Min=18; Max=82), and 81.25% (n=325) of the participants were women. When the coronavirus fear levels and total PSQI scores were examined according to sociodemographic characteristics. It was found that the scores of those who were married were significantly higher than those who were single and those who had children had significantly higher scores than those who had no children (p<0.05). The scores of those living with their spouse or friend were significantly higher than the scores of those living with their parents or spouse-children (p<0.05). It was determined that the scores of those who are literate or primary school graduates were significantly higher than those who are high school or university graduates (p<0.05). The scores of those who do not work in any job were significantly higher than those who are salaried or unemployed (p<0.05). As a result of the research, it was determined that the fear of COVID-19 disease in thyroid patients during the pandemic period affects sleep quality, which is a part of healthy life. It is thought that the results of the research will benefit the clinical field in terms of nursing care services and patient/patient relative education, increase awareness, and guide the nurses and the institutions.
