Abstract
Bu araştırma kardiyovasküler bypass ameliyatı olacak hastalarda anksiyete, depresyon ve cerrahi korkunun ameliyat sonrası ağrıya etkisinin incelenmesi amacıyla ilişki arayıcı türde yapılmıştır. Araştırmanın evrenini İstanbul ili Anadolu yakasında bulunan bir eğitim hastanesinde 02.04.2021-30.04.2022 tarihleri arasında kardiyovasküler bypass ameliyatı olacak hastalar, örneklemini ise 02.04.2021-30.04.2022 tarihleri arasında örnekleme dahil edilme kriterlerine uyan 171 hasta oluşturmuştur. Araştırma verileri, Hastane Anksiyete Depresyon Ölçeği, Cerrahi Korku Ölçeği ve Sayısal Ağrı skalası ile toplanmıştır. Araştırma verilerinin istatistiksel analizinde R vers. 2.15.3 programı (R Core Team, 2013) kullanılmıştır. Araştırma verilerinin değerlendirilmesinde tanımlayıcı istatiksel analiz yöntemler, ki-kare testi, bağımsız gruplarda t-testi, Mann Whitney-U testi, One-Way ANOVA ve Kruskall Wallis testleri kullanılmıştır. İstatistiksel anlamlılık p<0.05 düzeyinde değerlendirilmiştir Araştırma kapsamındaki hastaların %50,3'ünün kadın, %83,6'sının evli, %32,7'sinin ilkokul mezunu, %35,7'sinin geliri giderinden az, %73,1'inin kronik hastalığı olduğu, kronik hastalığı olan hastaların kronik %51,5'inin diabetus mellutus olduğu saptanmıştır. Hastaların Hastane Anksiyete Depresyon Ölçeği anksiyete alt boyutundan ortalama 11,90±2,25 puan, depresyon alt boyutundan ortalama 12,06±2,38 puan, Cerrahi Korku Ölçeği'nden ortalama 62,3±11,38 puan aldıkları bulunmuştur. Hastaların ameliyat sonrası 12.saat ağrı puanı ortalaması 8,4±0,78, 24.saat ağrı puanı ortalaması 6,02±1,15, 48.saat ağrı puanı ortalaması ise 4,73±1,36'dır. Araştırma kapsamındaki hastaların Hastane Anksiyete Depresyon Ölçeği anksiyete alt boyut puanları ile ameliyat sonrası 12., 24. ve 48. saatte ağrı puanlarının aralarında istatistiksel olarak pozitif yönde anlamlı ilişki, Hastane Anksiyete Depresyon Ölçeği depresyon alt boyut puanlarıyla ameliyat sonrası 24.saat ağrı puanı arasında pozitif yönde, Cerrahi Korku Ölçeği kısa dönem alt boyut puanları ile ameliyat sonrası 12., 24. ve 48. Saat ağrı puanları aralarında istatistiksel olarak pozitif yönde anlamlı ilişki saptanmıştır (p<0,01). Bu araştırmanın sonucunda hastaların ameliyat öncesi dönemde anksiyete ve depresyon için risk altında olduğu, cerrahi korkularının yüksek olduğu, ameliyat öncesi anskiyetenin ameliyat sonrası 12.,24.ve 48.saat ağrı puanını etkilediği, kısa dönem cerrahi korkunun ameliyat sonrası 24.saat ağrıyı etkilediği bulundu. Araştırmanın daha fazla örneklem ile tekrarlanması önerilmektedir.
This research was carried out in a relationship-seeking type in order to examine the effects of anxiety, depression and fear of surgery on postoperative pain in patients who will undergo cardiovascular bypass surgery. The population of the study consisted of patients who will undergo cardiovascular bypass surgery between 02.04.2021 and 30.04.2022 in a training hospital located on the Anatolian side of Istanbul, and the sample consisted of 171 patients who met the criteria for inclusion in the sampling between 02.04.2021 and 30.04.2022. Research data were collected with Hospital Anxiety Depression Scale, Surgical Fear Scale and Numerical Pain Scale. In the statistical analysis of research data, R vers. Of the patients within the scope of the study, 50.3% were women, 83.6% were married, 32.7% were primary school graduates, 35.7% had a lower income than their expenses, 73.1% had a chronic disease, It was determined that 51.5% of the patients with chronic diabetes mellitus had diabetes mellitus. It was found that the patients scored an average of 11.90±2.25 points from the Hospital Anxiety-Depression Scale anxiety sub-dimension, 12.06±2.38 points from the depression sub-dimension, and 62.3±11.38 points from the Surgical Fear Scale. The mean postoperative pain score of the patients at the 12th hour was 8.4±0.78, the mean of the 24th hour pain score was 6.02±1.15, and the mean 48th hour pain score was 4.73±1.36. There was a statistically significant positive correlation between the Hospital Anxiety Depression Scale anxiety sub-dimension scores and postoperative pain scores at the 12th, 24th and 48th hours of the patients within the scope of the study. There was a statistically significant positive correlation between the Surgical Fear Scale short-term sub-dimension scores and the postoperative 12th, 24th and 48th hour pain scores (p<0.01). As a result of this study, it was found that the patients were at risk for anxiety and depression in the preoperative period, their fear of surgery was high, preoperative anxiety affected the postoperative 12, 24 and 48 hours pain score, and short-term surgical fear affected the postoperative 24 hour pain. . It is recommended to repeat the study with more samples.
