Hastanelerde akılcı ilaç kullanımına yönelik kalite çalışmalarının değerlendirilmesi

Abstract

Akılcı İlaç Kullanımı (AİK), kişilerin klinik bulgularına ve özelliklerine göre uygun ilacı, uygun süre ve dozajda, en uygun maliyetle ve kolayca sağlayabilmektir. Hastane Hizmet Kalite Standartları (HHKS) AİK kılavuzunun yürürlüğe girmesiyle, çalışmalar hızlanmıştır. İlaçların gereğinden fazla veya yanlış kullanılması hasta güvenliğine zarar vermekle beraber, kaynakların da boşa harcanmasına neden olmaktadır. Bu çalışma HHKS kapsamında yürütülen, AİK çalışmalarının irdelenmesi, etkinliğinin belirlenmesi, yeterliliğinin değerlendirilmesi, çalışanların farkındalık ve görüşlerinin tespit edilmesi ve AİK'nın hasta güvenliğindeki öneminin vurgulanması amacıyla planlanmıştır. Çalışma, Malatya Devlet Hastanesindeki 400 sağlık çalışanında, AİK çalışmalarına yönelik uzman görüşü ve bilimsel verilerden yararlanılarak hazırlanan soruları içeren bir anket kullanılarak yapılmıştır. Veriler, IBM SPSS Statistics 19.0 aracılığıyla değerlendirilmiştir. Çalışma sonucunda çalışanların çoğunluğunun AİK eğitimi aldığı ve eğitim alanların çoğunluğunun AİK'i doğru tanımladığı belirlenmiştir. Çalışanların büyük bir kısmı hastane AİK komitesinin varlığını bildiği ve ihtiyaç duyduğunda ulaşabildiğini, teşhir ünitelerinin görsel açıdan etkili olduğu, sayılarının yeterli, yerleştirme konumlarını uygun, AİK uygulamalarının hasta güvenliğinin sağlanmasında etkili olduğunu belirtmişlerdir. Yine çalışanların çoğunluğu AİK çalışmalarında sağlık personelinde işbirliğinin önemli, AİK çalışmalarının halkın sağlığını ve toplumun çıkarlarını korumaya yönelik olduğunu düşündüklerini belirtmişlerdir. HHKS kapsamında yapılan çalışmaların AİK'nda etkili olduğu, çalışanlarda farkındalık ve bilinci arttırdığı, olumlu davranış değişikliği oluşturduğu tespit edilmiştir. Rational Drug Use (RDU) is to provide the appropriate medication due to patients' clinical findings, with the most cost-effective and easy way, in appropriate time and dosage. The guide of RDU in Service Quality Standards of Hospital (SQSH) has been established and has been given importance. In addition to harm for patient safety, the misuse or overuse of the drugs causes wasted resources. We aimed to emphasize the importance of RDU on safety of the patients, to investigate the efficiency and to evaluate the effectiveness of RDU that is performed in the hospitals, and to determine the views and ability of employees, in the scope of SQSH. The subject in this study was 400 employees who have been selected randomly in the 1200 health-care personnel in the Malatya State Hospital. We interviewed each person face to face and used a questionnaire about RDU. Analyses of the data were performed by using appropriate statistical methods by IBM SPSS 19.0. It was identified that most employees were educated for RDU and they explained what the RDU was. Much more employees declared that they have known there was a committee of RDU in the hospital, were able to reach in the case of requirement. They also declared that exhibition units are impressive, adequate and suitable places. In addition, they reported that it was effective in ensuring the safety of patients. The majority of the employees noted that the cooperation of health staff for studies of RDU was important, and they believed that studies of RDU aimed to protect the health and benefit of public. It was concluded that the studies in scope of SQSH have had an effect on RDU, increased awareness and consciousness of workers, and led a positive change in behavior. Execution and scoring of work in hospitals under the SQSH caused positive results. Keywords: Rational Drug Use, Service Quality Standards of Hospitals, Patients' safety
