Epilepsi hastalarında psikolojik iyi oluş ve umut düzeyi ilişkisi
Abstract
Bu araştırma, Nisan- Ağustos 2019 tarihleri arasında özel bir üniversite hastanesinin Nöroloji servisinde epilepsi tanısı almış; ek psikolojik rahatsızlığı olmayan, yetişkin bireylerin psikolojik iyi oluş ve umut düzeylerinin belirlenmesi ve her iki durumun birbiri üzerine etkisinin ortaya konulması amacıyla 132 hasta ile gerçekleştirilmiştir. Veriler, sosyo-demografik Bilgi Formu, Herth Umut İndeksi (HUİ) ve Psikolojik İyi Oluş Ölçeği (PİO) ile elde edilmiştir. Verilerin değerlendirilmesinde, sayı, yüzde, ortalama ve standart sapma değerleri, Kruskal Wallis, Mann Whitney U testi ve Spearman Korelasyon Katsayısı kullanılmıştır. Araştırma kapsamındaki hastaların %52,3'ünün 1-5 yıldır epilepsi hastası olduğu, %78'inin nadiren epilepsi nöbeti geçirdiği, %58,3'ünün epilepsinin günlük yaşamı 'bazen' etkilediği ve %53,8'inin epilepsinin arkadaşlarla ilişkisini 'bazen' etkilediği belirlenmiştir. Hastaların %26,5'i epilepsi hastası olduğunu ilk öğrendiğinde çaresizlik hissettiğini, %56,8'i epilepsinin iş yaşamını 'bazen' etkilediğini ve %50'si epilepsinin geleceğe dair planları 'her zaman' etkilediğini belirtmiştir. Hastaların HUİ ölçeği puan ortalaması 33,43±3,96 iken; PİO ölçeğinin ortalaması 38,29±8,60 olarak bulunmuş, HUİ toplam puanı ve PİO toplam puanları arasında orta derecede-pozitif yönlü ilişki olduğu görülmüştür (r=,60) (p<0,05). Hastaların PİO puanını yordayan değişkenler 'medeni durum' (β=0,150; p=0,30), 'öğrenim durumu' (β=0,287; p=0,000) ve 'genel ruh hali' (β= -0,198; p=0,002) olarak tespit edilmiştir (F=14,85; p=0,000). HUİ puanını yordayan değişkenler 'çalışma durumu' (β= -0,158; p=0,48), 'genel ruh hali' (β= -0,287; p=0,001) ve 'hastalık nedeniyle destek olacak kişi varlığı' (β= -0,295; p=0,001) olarak tespit edilmiştir (F=5,81; p=0,00). Epilepsi hastalarına bakım veren hemşirelerin, hastaların psikolojik iyi oluş ve umut düzeylerini etkileyen faktörlerini hemşirelik süreçlerini planlamada ele almaları önerilmektedir.
This research was diagnosed as epilepsy in a neurology service of a private university hospital between April and August 2019; This study was conducted with 132 patients who did not have additional psychological disorders, to determine the psychological well-being and hope levels of adult individuals and to demonstrate the effects of both conditions on each other. The data were obtained with the socio-demographic Information Form, Herth Hope Index (HUI) and Psychological Well-Being Scale (PIO). In the evaluation of the data, number, percentage, mean and standard deviation values, Kruskal Wallis, Mann Whitney U test and Spearman Correlation Coefficient were used. It was determined that, 52.3% of the patients in the study were epileptic patients for 1-5 years, 78% of them rarely had epileptic seizures, epilepsy affected 'sometimes' 58.3% of the patients' daily life, and 53.8% of the patients' relationship with friends. 26.5% of the patients stated that when they first learned that they had epilepsy, they felt helplessness, 56.8% of them stated that epilepsy affected the work life 'sometimes', and 50% of them said that, epilepsy 'always' affected their future plans. The mean score of the HUI scale was 33.43 ± 3.96; and the mean score of the PIO scale was 38.29 ± 8.60. It was observed that there was a moderate-positive correlation between HUI total score and PIO total score (r =, 60) (p <0.05). Variables predicting PİO scores of the patients were; 'marital status' (β = 0,150; p = 0,30), 'educational Status' (β = 0,287; p = 0,000) and 'gGeneral mood' (β = -0,198; p = 0,002) (F = 14.85; p = 0.000). The variables that predict the HUI score were, 'working status' (β = -0,158; p = 0,48), 'general mood' (β = -0,287; p = 0,001) and 'presence of a person to support due to illness' (β = -0,295 ; p = 0.001) (F = 5.81; p = 0.00). It is recommended that nurses who care for epilepsy patients should consider the factors that affect the psychological well-being and hope levels of patients in planning nursing processes.
