Özel sağlıklı yaşam merkezine gelen bireylerde uyku kalitesi, fiziksel aktivite ve beslenme durumu arasındaki ilişkinin belirlenmesi

Abstract
Son dönemlerde vücut ağırlığı fazlalığı ve obezite büyük bir halk sağlığı problemi olarak görülmektedir. Obezite ise beraberinde birçok hastalığı getirmektedir. Yetersiz beslenme, fiziksel aktivite yetersizliği, uyku kalitesizliğinin obeziteye neden olabileceği çalışmalarla desteklenmiştir. Bu çalışmada; özel bir sağlıklı yaşam merkezine gelen bireylerde uyku kaliteleri, fiziksel aktivite düzeyleri ve beslenme durumlarını saptamak ve aralarındaki ilişkinin incelenmesi amaçlanmıştır. Çalışma Ocak-Mart 2020 tarihleri arasında Pelin Topbaş Sağlıklı Yaşam Merkezine başvuran 19-65 yaş arası 175 birey (E:%30,9, K:%69,1) üzerinde gerçekleştirilmiştir. Çalışmaya katılan bireylerin yaş ortalamaları 39,4±13,4 yıldır. Çalışmaya katılan birey arasında zayıf birey bulunmaz iken %50,3 oranında bireyin Beden Kütle İndeksi (BKİ) normal aralıktadır. Bireylerin %27,5'i obezdir. Uluslararası Fiziksel Aktivite Anketi Kısa Formu (UFAA-KF) puan sınıflamasına göre; yeterli düzeyde fiziksel aktivite yapan birey bulunmamaktadır. Fiziksel aktivite yapmayan bireylerin oranı %44,6, yetersiz fiziksel aktivite yapan bireylerin oranı %55,4'tür. Fiziksel aktivite yapmayan erkeklerin oranı (%60,4) kadınlara göre (%37,7) daha fazladır (p<0,05). Fiziksel aktivite yapmayan (FAY) bireylerin vücut ağırlığı ortalaması 83,4±19,3 kg iken yetersiz fiziksel aktivite (YFA) yapan bireylerinki 68,9±16,0 kg'dır (p=0,000). Bel çevresi, BKİ, kalça çevresi, boyun çevresi, bel boy oranı, bel kalça oranı, vücut yağ yüzdesi, vücut yağ kütlesi, yağsız vücut kütlesi ortalamaları FAY grubunda YFA grubuna göre daha yüksektir (p>0,000). Toplam UFAA-KF puanıyla yaş, vücut ağırlığı, BKİ, bel çevresi, kalça çevresi, boyun çevresi, bel kalça oranı, bel boy oranı, vücut yağ yüzdesi, vücut yağ kütlesi, yağsız vücut kütlesi ve yağsız vücut yüzdesi arasında negatif kolerasyon bulunmaktadır. Pittsburgh Uyku Kalitesi Ölçeği (PUKİ) ölçeğine göre; kötü uyku kalitesine sahip bireylerin oranı %33,7, iyi uyku kalitesine sahip bireylerin oranı %66,3'tür. İyi ve kötü uyku kalitesine sahip bireylerin BKİ değerleri; 25,9±4,7 ve 27,5±5,1 kg/m2'dir. Bireylerin PUKİ ölçeğinden aldıkları puan ile yaş, vücut ağırlığı, BKİ, bel çevresi, bel kalça oranı, vücut yağ yüzdesi, vücut yağ kütlesi arasında pozitif kolerasyon bulunmaktadır. Bireylerin uyku kalitesi azaldıkça (PUKİ puanı artmaktadır) yağsız vücut kütlesi azalmaktadır (p<0,05). Sonuç olarak kaliteli uyku ve fiziksel aktivitenin obezite tedavisinin bir parçası olduğu çalışmamızda da yinelenmiştir.
Recently, excessive body weight and obesity are seen as a major public health problem. Obesity causes many diseases along with itself. In these circumstances, the causes of obesity have been examined and it has been supported by studies that malnutrition, insufficiency of physical activity, sleep poorness may cause obesity. In this research, it was aimed to determine the sleep quality, physical activity levels and nutritional status of the individuals, who came to a private health center, and to examine the relationship between them. The research was conducted on 175 individuals (M:30,9%, F:69,1%) aged between 19-65 years, who applied to Pelin Topbaş Healthy Life Center between January-March 2020. The average age of the individuals participating in the research is 39,4±13,4 years. While there are no thin individuals among the participants in the research, 50,3% of the individual's BMI values are in the normal range. 27,5 percent of the individuals are obese. According to International Physical Activity Questionnaire Short Form (IPAQ-SF) score classification; there is no individual doing physical activity sufficiently. The proportion of individuals, who do not perform physical activity is 44,6% and the proportion of individuals who perform physical activity insufficiently, is 55,4%. The proportion of men, who do not perform physical activity, (60,4%) is higher compared to the women (37,7%) (p <0,05). While the average body weight of individuals who do not perform physical activity (NPPA), was 83,4±19,3 kg, the average rate of those who perform physical activity insufficiently, (PPAI) is 68,9±16,0 kg (p=0,000). There is a negative correlation between the total IPAQ-SF score and age, body weight, BMI, waist circumference, hip circumference, neck circumference, waist hip ratio, waist height ratio, body fat percentage, body fat mass, lean body mass and lean body percentage. According to the The Pittsburgh Sleep Quality Index (PSQI) scale; the rate of individuals with poor sleep quality is 33,7%, and the rate of individuals with good sleep quality is 66.3%. BMI values of individuals with good and poor sleep quality are; 25,9±4,7 and 27,5±5,1 kg/m2. There is a positive correlation between the score obtained by the individuals on the PSQI scale and the age, body weight, BMI, waist circumference, waist hip ratio, body fat percentage, body fat mass. As the sleep quality of individuals decreases (PSQI score increases), lean body mass decreases (p<0,05). As a result, quality sleep and physical activity were also repeated in our study as part of obesity treatment.
