Madde bağımlılığı tanısı almış bireylerin savunma biçimlerinin tedavi motivasyonu ile ilişkisinin incelenmesi
Abstract
Bu çalışma, bir psikiyatri hastanesinin AMATEM kliniğinde yatarak tedavi gören madde bağımlılığı tanısı almış bireylerin savunma biçimlerinin tedavi motivasyonu ile ilişkisinin incelenmesi amacıyla yapılmıştır. Bu çalışma, tanımlayıcı ve kesitsel yöntem ile yapılmıştır. Çalışmanın örneklemini bir psikiyatri hastanesinde yatarak tedavi gören madde bağımlılığı tanısı olan 170 katılımcı oluşturmuştur. Çalışmanın verileri, Sosyodemografik Veri Formu, Savunma Biçimleri Testi ve Tedavi Motivasyonu Anketi kullanılarak elde edilmiştir. Verilerin değerlendirilmesinde ortalama, standart sapma gibi tanımlayıcı istatistiksel yöntemler, demografik verilere dair frekans dağılımları (sayı, yüzde, ortalama, standart sapma), Kruskal-Wallis, Mann Whitney U, T-testi, One Way Anova ve Spearman's rho korelasyon analizleri kullanılmıştır. Çalışmamızın analiz sonucunda, çalışmayan ve çocuk sahibi olan katılımcıların nevrotik savunmalarda daha yüksek puanlar aldığı tespit edilmiştir. Ayrıca çocuk sahibi olanların dışsal motivasyon puanlarının daha yüksek olduğu belirlenmiştir. Daha önce maddeye bağlı yatışı olan katılımcıların immatür ve nevrotik savunmalarda daha yüksek puan aldığı saptanmıştır. Maddeyi bırakabilme düşüncesi olan katılımcıların nevrotik ve olgun savunmalarda daha yüksek puanlar aldığı gözlenmiştir. Savunma biçimlerinin tedaviye güven puanlarını yordadığı; immatür savunmaların tedaviye güven puanlarını negatif yönde anlamlı düzeyde yordadığı sonucuna ulaşılmıştır. Bu çalışmanın sonucunda madde bağımlılığı tanısı olan bireylerde savunma mekanizmasının birçok değişken ile ilişki olduğu görülmüştür. Tedavi motivasyonunun bir alt boyutu olan tedaviye güvenin bireylerin sahip oldukları savunma mekanizmasından etkilendiği tespit edilmiştir. Bu bağlamda madde bağımlılığı tedavisi ve bakımda bireylerin yaşamlarını etkileyen savunma mekanizmaları göz ardı edilmemesi önerilmektedir. This study was carried out to examine the relationship between the defense styles and treatment motivation of individuals diagnosed with substance addiction who are hospitalized in the AMATEM clinic of a psychiatric hospital. This study was conducted with descriptive and cross-sectional method. The sample of the study consisted of 170 participants with a diagnosis of substance addiction who were hospitalized in a psychiatric hospital. The data of the study were obtained by using the Sociodemographic Data Form, the Defense Styles Test, and the Treatment Motivation Questionnaire. Descriptive statistical methods such as mean and standard deviation, frequency distributions of demographic data (number, percentage, mean, standard deviation), KruskalWallis, Mann Whitney U, T-test, One Way Anova and Spearman's rho correlation analyzes were used to evaluate the data. As a result of the analysis of our study, it was determined that the participants who did not work and had children had higher scores in neurotic defenses. In addition, it was determined that those who have children have higher extrinsic motivation scores. It was determined that the participants who had previously been hospitalized due to the substance had higher scores in immature and neurotic defenses. It was observed that the participants with the idea of quitting the substance received higher scores in neurotic and mature defenses. Defense styles predicted treatment trust scores; It was concluded that immature defenses predicted treatment trust scores negatively and significantly. As a result of this study, it was observed that the defense mechanism of individuals with a diagnosis of substance addiction was associated with many variables. It has been determined that trust in treatment, which is a sub-dimension of treatment motivation, is ii affected by the defense mechanism of individuals. In this context, it is recommended that the defense mechanisms that affect the lives of individuals in substance abuse treatment and care should not be ignored.
