Abstract
Patellofemoral Ağrı Sendromunda hastalarda, alt ekstemite dizilim bozuklukları, kas kuvveti kaybı ve esneklik kaybına bağlı fonksiyonel kapasitede ve yaşam kalitesinde azalma görülür. Hastalarda anksiyete, depresyon, kinesiofobi insidansı yüksektir. Bu nedenle hastaların sağlık durumunu etkilediği gibi tedavi masrafları, iş gücü kaybı gibi nedenlerle ciddi maddi kayıplara da sebep olmaktadır. Bu nedenle Patellofemoral Ağrı Sendromu oluşturacak risk faktörlerinin belirlenmesi önem kazanmıştır. Bu çalışmada kas iskelet sistemi riskleri ve ergonomik risklerin Patellofemoral Ağrı Sendromlu hastaların ağrı ve işlevsel kayıpları ile ilişkileri araştırıldı. Çalışma, İstanbul Okan Üniversitesi Hastanesi Ortopedi ve Travmatoloji ve Fizik Tedavi Rehabilitasyon Kliniklerinde gerçekleşti. Çalışmaya Patellofemoral Ağrı Sendromu tanılı ve gönüllü olmak için onam veren 132 hasta alındı. Katılımcılara ait sosyo-demografik veriler oluşturulan formla kaydedildi, Hastaların kas iskelet sistemine ait risk faktörleri ve semptomları Dutch Kas İskelet Sistemi anketiyle, farklı vücut bölgelerinde mevcut olan kas iskelet sistemi rahatsızlıkları Cornell Kas İskelet Sistemi Rahatsızlıkları Anketi ile, Patellofemoral ağrı durumu Kujala Patellofemoral Skorlama Sistemi ile, alt eksremitelerin fonsiyonel durumu ise Alt Ekstremite Fonksiyonel Ölçeğiyle değerlendirildi. İstatistiksel anlamlılık p<0,05 olarak belirlendi. Sonuç olarak Patellofemoral Ağrı Sendromlu kişilerde, ağrı ve alt ekstremite fonksiyon bozukluklarının yaş, cinsiyet, vücut kütle indeksi, eğitim durumu, meslek ve sigara kullanımı gibi kişisel özelliklerden etkilendiği, bu etkilenimlerin kas-iskelet sistemi semptomlarından bağımsız olduğu, genel sağlık durumunu iyi olarak algılayan, serbest zamanlarını egzersiz ve sporla aktif olarak geçiren kişilerin patello femoral ağrılar ve alt ekstremite fonksiyonları açısından daha iyi durumda oldukları, tekrarlı hareketleri sık yaparak çalışanların patellofemoral ağrılarının daha kötü etkilendikleri, kas-iskelet sistemi semptomlarının varlığı ve şiddetinin çalışmamıza konu olan parametreleri etkilemediği saptandı.
Patients with Patellofemoral Pain Syndrome have reduced functional capacity and quality of life due to lower extremity alignment, loss of muscle strength and loss of flexibility. The incidence of anxiety, depression and kinesiophobia is high in patients. Therefore, it affects the health status of the patients and also causes serious financial losses due to treatment costs and labor loss. Therefore, determination of risk factors for Patellofemoral Pain Syndrome has gained importance. In this study, the relationship between musculoskeletal risks and ergonomic risks with pain and functional loss of patients with Patellofemoral Pain Syndrome was investigated. The study was carried out in Istanbul Okan University Hospital Orthopedics and Traumatology and Physical Therapy Rehabilitation Clinics. 132 patients with Patellofemoral Pain Syndrome who gave consent to volunteer were included in the study. The socio-demographic data of the participants were recorded by using the form. The risk factors and symptoms of the musculoskeletal system were evaluated by the Dutch Musculoskeletal Questionnaire, the musculoskeletal system disorders in different body regions by the Cornell Musculoskeletal System Questionnaire, Patellofemoral pain status by Kujala Patellofemoral Scoring System. Functional status of the lower extremities was evaluated using the Lower Extremity Functional Scale. Statistical significance was set at (p <0.05). In conclusion, it was found that pain and lower extremity dysfunctions were affected by personal characteristics such as age, sex, body mass index, education level, occupation and smoking, and that these effects were independent of musculoskeletal symptoms, and perceived general health status as good. It was found that people who spend their free time actively with exercise and sport were better in terms of patellar femoral pain and lower extremity functions, repetitive movements were frequently affected by workers' patellofemoral pain, and the presence and severity of musculoskeletal symptoms did not affect the parameters of our study.
