Abstract
Bu araştırma, metabolik sendromu (MetS) olan bireylerin tıbbi beslenme tedavisine (TBT), 'oleuropein' maddesinin eklenmesinin bireylerin antropometrik ölçümleri ve biyokimyasal parametreleri üzerindeki etkisinin incelenmesi amacıyla gerçekleştirilmiştir. Çalışma, hekim tarafından MetS tanısı almış 18-49 yaş arası 34 kişi ile yürütülmüştür. Katılımcılar sadece TBT alan grup (Kontrol grubu, n=17) ve TBT + 400 mg oleuropein alan grup (Deney grubu, n=17) olarak 2 gruba randomize edilmiştir. Tüm katılımcılar 6 hafta süreyle takip edilmiş, müdahale öncesi demografik bilgileri içeren anket formu ve üç günlük besin tüketim kaydı yüz yüze görüşmelerde araştırmacı tarafından doldurulmuş, müdahale öncesi ve sonrası biyokimyasal parametreleri ve antropometrik ölçümleri araştırmacı tarafından kayıt altına alınmıştır. Müdahale öncesi gruplar tüm veriler açısından homojen dağılmıştır. Veriler SPSS 25.0 programı ile değerlendirilmiştir. Çalışma sonunda TBT uygulanan tüm gruplarda ağırlık, BKİ, kas kütlesi, yağ kütlesi, yağ yüzdesi, vücut suyu, kol çevresi, göğüs çevresi, kalça çevresi ve bel/boy oranının başlangıca göre azaldığı saptanmıştır (p<0,05). Deney grubunda erkek katılımcılarda ağırlık, yağ kütlesi, yağ yüzdesi ve bel çevresindeki düşüşlerin kontrol grubuna göre daha fazla olduğu belirlenirken (p<0,05); kadın katılımcılarda antropometrik ölçümler açısından gruplar arasında anlamlı bir fark gözlenmemiştir. Laboratuvar parametrelerine bakıldığında tüm katılımcılarda AKŞ, insülin, HOMA-IR, HbA1c, C-peptid, TG, toplam kolesterol, LDL-K, HDL-K ve CRP düzeylerinin başlangıca göre düştüğü bulunmuştur (p<0,05). Deney grubunda tüm katılımcılarda TG, total kolesterol, LDL-K ve HDL-K seviyelerindeki düşüşlerin kontrol grubuna oranla daha yüksek olduğu saptanmıştır (p<0,05). Ayrıca kadın katılımcılarda deney grubunda C-peptid seviyelerinin kontrol grubuna oranla daha fazla düştüğü belirlenmiştir (p<0,05). Sonuçlar TBT'nin MetS'si olan kişilerde olumlu etkileri olduğu ve TBT'ye 400 mg oleuropein eklenmesinin, lipid profili ile ilgili parametreleri iyileştirdiği, MetS'nin gelişiminin önlenmek için faydalı olabileceğini göstermiştir. This study was conducted to investigate the effect of adding oleuropein to the medical nutrition therapy (MNT) of individuals with metabolic syndrome (MetS) on their anthropometric measurements and biochemical parameters. The study was conducted with 34 individuals aged 18-49 years who were diagnosed with MetS by a physician. Participants were randomized into 2 groups as MNT only group (Control group, n=17) and MNT + 400 mg oleuropein group (Experimental group, n=17). All participants were followed up for 6 weeks, a questionnaire form including demographic information and a three-day food consumption record were completed by the researcher in face-to-face interviews, and biochemical parameters and anthropometric measurements before and after the intervention were recorded by the researcher. The pre-intervention groups were homogeneously distributed in terms of all data. Data were evaluated with SPSS 25.0 program. At the end of the study, weight, BMI, muscle mass, fat mass, fat mass, fat percentage, body water, arm circumference, chest circumference, hip circumference and waist/height ratio decreased in all MNT groups compared to the baseline (p<0.05). While it was determined that the decreases in weight, fat mass, fat percentage and waist circumference in male participants in the experimental group were higher than in the control group (p<0.05); no significant difference was observed between the groups in terms of anthropometric measurements in female participants. When laboratory parameters were analyzed at the end of the study, it was found that the levels of BPG, insulin, HOMA-IR, HbA1c, C-peptide, TG, total cholesterol, LDL-C, HDL-C and CRP decreased in all participants compared to the baseline (p<0.05). In the experimental group, decreases in TG, total cholesterol, LDL-C and HDL-C levels were found to be higher in all participants compared to the control group (p<0.05). In addition, it was determined that C-peptide levels decreased more in the experimental group compared to the control group in female participants (p<0.05). The results showed that MNT has positive effects in people with MetS and the addition of 400 mg oleuropein to MNT improved the parameters related to lipid profile and may be useful for preventing the development of MetS.
