Kronik bel ağrılı hastalarda germe platformu ile yapılan egzersizlerin ağrı, propriosepsiyon, denge ve mobiliteye etkisi
Abstract
Çalışmamızın amacı, kronik bel ağrılı hastalarda konservatif tedaviye ek olarak germe platformu ile uygulanan egzersizlerin ve sadece konservatif tedavinin ağrı, propriosepsiyon, denge ve mobiliteye etkisinin incelenmesi ve karşılaştırılmasıydı. Çalışma, kronik bel ağrısı tanısı ile Fizyopedi Egzersiz Danışmanlık Merkezi'ne başvuran, 25-65 yaş aralığındaki 55 katılımcı ile yapıldı. Katılımcılar randomize olarak iki gruba ayrıldı. Grup 1'e dahil edilen katılımcılar 6 hafta boyunca, haftada 3 gün konservatif tedavi programına alınırken; Grup 2'ye dahil edilen katılımcılar ise 6 hafta boyunca, haftada 3 gün konservatif tedaviye ek olarak germe platformu ile uygulanan egzersiz programına alındı. Katılımcıların Görsel Analog Skalası ile ağrı şiddeti, pasif pozisyonlamanın ekstremite desteklenmeden aktif olarak yeniden oluşturulması yöntemi ile propriosepsiyon duyusu, Modifiye Schober Testi, El Parmak-Zemin Ölçümü ve Gövde Lateral Eğilme Ölçümü ile mobilitesi, Fonksiyonel Uzanma Testi ile denge düzeyi, Oswestry Yetersizlik Anketi ile fonksiyonelliği ve EuroQol Group 5D-3L Anketi ile yaşam kalitesi değerlendirildi. Çalışmada, grup içi değerlendirmelerde ağrı, propriosepsiyon, Modifiye Schober Testi, El Parmak-Zemin ve Gövde Lateral Eğilme Ölçümü, Fonksiyonel Uzanma Testi, Oswestry Yetersizlik ve EuroQol Group 5D-3L Anketi skorları arasında istatistiksel olarak anlamlı fark gözlendi (p<0,05). Gruplar arası değerlendirmelerde ise ağrı değerlendirmesinde aktivite sırasındaki GAS skoru ve propriosepsiyon değerlendirmesinde 15° sağ ayak bileği plantar fleksiyonu, konservatif tedaviye ek olarak germe platformu ile uygulanan egzersiz grubunda, sadece konservatif tedavi uygulanan gruba göre daha üstündü (p<0,05). Çalışmamızın sonucunda kronik bel ağrısı tedavisinde konservatif tedavi ve germe platformu ile uygulanan egzersizlerin ağrı, propriosepsiyon, mobilite, denge, fonksiyonellik ve yaşam kalitesi üzerine olumlu etkiler sağladığı gözlendi.
The aim of our study was to investigate and compare the effects of exercises applied with a stretching platform in addition to conservative treatment in patients with chronic low back pain and only conservative treatment on pain, proprioception, balance and mobility. The study was conducted with 55 participants between the ages of 25-65 who applied to the Fizyopedi Exercise Counseling Center with the diagnosis of chronic low back pain. Participants were randomly divided into two groups. While the participants included in Group 1 were taken to the only conservative treatment program 3 days a week for 6 weeks; participants included in Group 2 were included in an exercise program applied with stretching platform in addition to conservative treatment. The pain intensity of the participants was assessed with the Visual Analog Scale; proprioception was evaluated with passive positioning method; lumbar region mobility was assessed with Modified Schober's Test, Hand Finger-Ground Measurement and Trunk Lateral Bending Measurement; balance level was evaluated with Functional Reach Test; functionality was assessed with the Oswestry Disability Questionnaire; quality of life was assessed with the EuroQol Group 5D-3L questionnaire. In the study, a statistically significant difference was observed between pain, proprioception, Modified Schober's Test, Hand Finger-Ground and Trunk Lateral Bending Measurement, Functional Reach Test, Oswestry Insufficiency and EuroQol Group 5D-3L Questionnaire scores in inter group evaluations (p<0.05). In the evaluations between groups, the VAS score during activity in pain evaluation and 15° right ankle plantar flexion in proprioception evaluation were superior in the exercise group applied with a stretching platform in addition to conservative treatment compared to the group in which only conservative treatment was applied (p<0.05). As a result of our study, it was observed that conservative treatment and exercises applied with a stretching platform in the treatment of chronic low back pain had positive effects on pain, proprioception, mobility, balance, functionality and quality of life.
