Abstract
Bu araştırma, mastektomi ameliyatı olan kadınların ilaca ve hastalığa uyumlarının incelenmesi amacıyla tanımlayıcı ve ilişki arayıcı türde yapıldı. Araştırmanın evrenini Sağlık Bilimleri Üniversitesi Haydarpaşa Numune Eğitim Araştırma Hastanesinde mastektomi ameliyatı olmuş hastalar, örneklemini ise Sağlık Bilimleri Üniversitesi Haydarpaşa Numune Eğitim Araştırma Hastanesinde 30 Eylül 2019 – 30 Aralık 2019 tarihleri arasında başvuran mastektomili 112 kadın hasta oluşturdu. Araştımanın verileri Hasta Bilgi Formu, İlaç Uyum Bildirim Ölçeği ve Hastalığa Psikososyal Uyum Öz Bildirim Ölçeği kullanılarak toplandı. Araştırma verilerinin değerlendirilmesinde, sayı-yüzdelik hesaplamaları, Shapiro-Wilk testi, t testi, Mann-Whitney U testi, Kruskal Walls testi ve Pearson korelasyon analizi kullanıldı. Araştırmaya katılan kadınların yaş ortalamasının 56,29±9,06 yıl olduğu, tümünün evli, %30,4'ünün ilköğretim mezunu, %47,3'ünün gelirinin giderinden az olduğu, %50'sinin kanserin III evresinde olduğu, tümünün kemoterapi ve cerrahi aldığı, %58,9'unun total mastektomi ameliyatı olduğu bulundu. Kadınların İlaç Uyum Ölçeği puan ortalamasının puan ortalamasının 5,76±1,17 olduğu, Hastalığa Psikososyal Uyum Ölçeğinin sağlık bakımına oryantasyon alt boyutu puan ortalamasının 7,11±2,30, mesleki çevre alt boyutunun puan ortalamasının 6,48±2,14, aile çevresi alt boyutu puan ortalamasının 9,70±3,60, seksüel ilişkiler puan ortalamasının 6,91±2,28, geniş aile ilişkileri puan ortalamasının 3,55±1,72, sosyal çevre alt boyutu puan ortalamasının 7,01±2,81 ve psikolojik baskı alt boyutu puan ortalamasının 6,12±2,64 olduğu bulundu. Bu araştırmanın sonucunda hastaların psikososyal uyumlarının orta düzeyde olduğu, ilaç uyumlarının iyi düzeyde olduğu, ilaç uyumu ile psikososyal uyum arasında pozitif yönde anlamlı bir ilişki olduğu sonucuna ulaşıldı. Hastaların yaşı, eğitim düzeyi, çalışma durumu, kanser evresine göre ilaç uyumu arasında anlamlı farklılık bulundu. Benzer şekilde çalışıyor olmak, iyi ekonomik gelir, erken evre, parsiyel mastektomi hastalığa daha yüksek psikososyal uyum gösterilmesi ile ilişkili bulundu. Özellikle çalışmayan, düşük ekonomik gelirli, ileri evre ve yaşta bulunan, ilaçlarını önerilen doz ve süreyle kullanmayan ve total mastektomi ameliyatı geçirmiş meme kanserli kadınlara psikolojik destek verilmesinin gerektiği önerildi.
This research was carried out in descriptive and correlational type in order to examine the compliance of women with mastectomy to the drug and disease. The population of the study consisted of patients who had mastectomy surgery at Haydarpaşa Numune Education and Research Hospital of Health Sciences University and 112 women with mastectomy who applied the sample between September 30, 2019 - December 30, 2019 at Haydarpaşa Numune Education and Research Hospital. The data of the research were collected using the Patient Information Form, Drug Compliance Reporting Scale, and Psychosocial Adaptation to Disease Self Reporting Scale. In evaluating the research data, number-percentage calculations, Shapiro-Wilk test, t test, Mann-Whitney U test, Kruskal Walls test and Pearson correlation analysis were used. The average age of the women participating in the study is 56.29 ± 9.06 years, all are married, 30.4% are primary school graduates, 47.3% are less than their income, 50% are in the III stage of cancer, all It was found that he received chemotherapy and surgery and 58.9% had total mastectomy surgery. The average score of the Women's Psychosocial Adaptation Scale for Healthcare Orientation sub-dimension of the Psychosocial Adaptation Scale of Disease was 5.76 ± 1.17, the mean score of the professional environment subscale was 6.48 ± 2.14, family environment subscale mean score is 9.70 ± 3.60, sexual relations point average is 6.91 ± 2.28, extended family relationship point average is 3.55 ± 1.72, social environment subscale point average is 7.01 ± 2 The mean score of 81 and psychological pressure sub-dimension was found to be 6.12 ± 2.64. As a result of this study, it was concluded that psychosocial adjustment of patients was moderate, drug compliance was good, and there was a positive and positive relationship between drug compliance and psychosocial adjustment. In addition, good education level, being employed, early stage and young age were associated with better drug compliance. Similarly, working was associated with good economic income, early stage, higher psychosocial adjustment to partial mastectomy disease. It was suggested that psychological support should be given to women with breast cancer, who do not v work, who have low economic income, who are at an advanced age and who are not using their medication for the recommended dose and duration, and who have undergone total mastectomy.
