Kardiyovasküler Cerrahi Yoğun Bakım Ünitesi Hemşirelerinin Güvenli Kan ve Kan Ürünleri Transfüzyonu Öz Yeterlilik Düzeyleri ve Çevrimiçi Eğitimin Etkisinin İncelenmesi
Abstract
Amaç: Araştırma, Kardiyovaskülercerrahi Yoğun Bakım Ünitesi hemşirelerinin güvenli kan ve kan ürünleri transfüzyonu öz yeterlilik düzeyleri ve çevrimiçi eğitiminin etkisinin incelenmesi amacıyla ön test-son test düzeninde yarı deneysel bir çalışma olarak yürütüldü. Yöntem: Araştırmanın örneklemini bir Eğitim ve Araştırma Hastanesinde KVC Yoğun Bakım Ünitesi çalışan toplam 90 hemşire oluşturdu. Olasılıksız örnekleme ile seçilen hemşireler rastgele randomizasyon yöntemiyle uygulama (45) ve kontrol (45) grubuna ayrıldı. Araştırmanın uygulaması 15/05/2024-/15/07/2024 tarihlerinde gerçekleştirildi. Araştırmanın verileri, araştırmacı tarafından geliştirilen Kişisel Bilgi Formu ve Güvenli Kan ve Kan Transfüzyonu Çevrim İçi Eğitim İçeriği ile Hemşireler İçin Güvenli Kan ve Kan Ürünleri Transfüzyonu Öz Yeterlilik Ölçeği (GKKT-ÖYÖ )kullanılarak toplandı. Uygulama grubundaki hemşirelere 40 dakikalık online güvenli kan transfüzyon uygulamaları eğitimi verildi. Kontrol grubuna herhangi bir eğitim verilmedi. SPSS 22.0 paket programı ile yapıldı. İstatistiksel analizlerde p<0.05 istatistiksel olarak anlamlı kabul edildi. Verilerin değerlendirilmesinde sayı, yüzde, ortalama, standart sapma, tek yönlü varyans analizi, bağımsız gruplar t testleri kullanıldı. Bulgular: Araştırmada, hemşirelerin GKKT-ÖYÖ toplam puan ortalaması alt boyut son testinden aldıkları puan ortalamalarının dağılımlarına bakıldığında uygulama grubundakilerin puan ortalamasının 215,22±10,17, kontrol grubunun ise 197,11±24,47 olduğu saptandı. Yapılan analiz sonucu tüm alt boyutlar açısında son testinden test puan ortalamaları açısından anlamlı fark olduğu ve uygulama grubunun kontrol grubuna göre daha yüksek puanlar aldığı belirlenmiştir (p<0,05). Sonuç: Çalışma sonucunda online eğitim alan hemşirelerin puanlarının eğitim alamayan hemşire grubuna göre arttığı, online eğitimin etkin olduğu bulundu. Sonuç olarak hemşirelerin çalıştıkları kurumlarda güvenli kan tranfüzyonu uygulamalarında bilgi düzeyi ve tutumların geliştirilmesi için güncel içerikli eğitimlerin hazırlanarak, bu eğitimlerin belirli aralıklarla tekrarlanması önerilmektedir. Aim: The research was conducted as a quasi-experimental study with a pre-test-post-test design in order to investigate the self-efficacy levels of Cardiovascular Surgery Intensive Care Unit nurses on safe blood and blood product transfusion and the effect of online training. Method: The sample of the research consisted of 90 nurses working in the CVS Intensive Care Unit in a Training and Research Hospital. The nurses selected with non-probability sampling were divided into intervention (45) and control (45) groups by random randomization method. The application of the research was carried out between 15/04/2024 and 15/06/2024. The data of the study were collected using the Personal Information Form and Safe Blood and Blood Transfusion Online Education Content and the Safe Blood and Safe Blood and Blood Products Transfusion Self-Efficacy Scale (SBT-SES). The nurses in the intervention group were given 40-minute online safe blood transfusion practices training. The control group did not receive any training. It was conducted using the SPSS 22.0 package program. In statistical analyses, p<0.05 was considered statistically significant. Number, percentage, mean, standard deviation, one-way variance analysis, independent groups t-tests were used in the evaluation of the data. Results: In the study, when the distribution of the mean scores of the nurses from the (SBT-SES) total mean score sub-dimension post-test was examined, it was determined that the mean score of the nurses in the intervention group was 215.22±10.17 and the control group was 197.11±24.47. As a result of the analysis, it was determined that there was a significant difference in terms of the mean test scores from the all sub-dimension post-test and that the intervention group received higher scores than the control group (p<0.05). Conclusion: As a result of the study, it was found that the scores of the nurses who received online training increased compared to the group of nurses who did not receive training, and that online training was effective. As a result, it is recommended that up-to-date content trainings be prepared and these trainings be repeated at certain intervals in order to develop the knowledge level and attitudes of nurses in safe blood transfusion practices in the institutions where they work.
