Sağlık çalışanlarında ekip çalışması etkinliği ile hasta güvenliği kültürü arasındaki ilişkinin belirlenmesi: Bir kamu hastanesi örneği

Abstract

Araştırma kapsamında sağlık sektöründe farklı sağlık meslek gruplarını içeren, tanı, tedavi, bakım sürecinde doğrudan veya dolaylı hasta ile teması olan sağlık çalışanlarının (hemşire, ebe, hekim, teknisyen, tekniker, vb.) ekip çalışması etkinliği ile hasta güvenliği kültürü arasındaki ilişki ve sosyokültürel özelliklerin ekip çalışması etkinliği ve hasta güvenliği kültürü ile ilişkisi ele alınmıştır. Hasta güvenliği kültürü ve ekip çalışması kavramları konusunda yapılmış olan araştırmalar incelendiğinde; sağlık sektöründe her iki kavramın son yıllarda önem kazanmaya başladığı, sağlık sektöründe iki kavram ilişkisinin incelendiği tez çalışmasının ülkemizde sınırlı sayıda yer aldığı görülmektedir. Bu nedenle çalışmada sağlık çalışanlarının ekip çalışması etkinliği ve hasta güvenliği kültürü düzeylerinin belirlenmesi ve aralarındaki ilişkinin karşılaştırılması amaçlanmıştır. Tez üç bölümden meydana gelmiştir. Birinci bölümde ekip çalışması, ikinci bölümde hasta güvenliği kültürü ve üçüncü bölümde ise ekip çalışması etkinliği ve hasta güvenliği kültürü arasındaki ilişkiyi belirlemek amacıyla İstanbul ilinde bir kamu hastanesinde görev yapan sağlık çalışanları ile yapılan araştırmaya yer verilmiştir. Araştırmanın sonucunda; sosyo-kültürel özellikler ile ekip çalışması etkinliği ve hasta güvenliği kültürü arasında anlamlı farklılıklar olduğu tespit edilmiştir. Ayrıca sağlık çalışanlarının orta düzeyde etkin bir ekip çalışmasının olduğu ve hasta güvenliği kültürü alt boyutlarından hata raporlama, istihdam, görev değişimi, hataların değerlendirilmesi düzeyinin zayıf olduğu görülmüştür. Diğer önemli sonucu ise; ekip çalışması etkinliğinin hasta güvenliği kültürü üzerinde zayıf düzeyde ve pozitif yönde etkisinin olduğu anlaşılmıştır.

The relationship between teamwork efficiency and patient safety culture of the healthcare professionals (nurse, midwife, physician, operator, technician, etc.) incorporating various health profession groups in the healthcare sector who have direct or indirect contact with the patient during the diagnosis, treatment, care processes as well as the relationship of socio-cultural particularities with teamwork efficiency and patient safety culture are addressed within the scope of the study. When the researches which had been carried out regarding the subjects of patient safety culture and teamwork concepts; it is being observed that both concepts started come into prominence in the last years in the healthcare sector, and yet that thethesis studies, in which the relationship of two notions on the healthcare sector is scrutinized, figure in limited number in our country. Hence, the identification of teamwork efficiency of the healthcare professionals and the levels of patient safety culture as well as the comparison of the relation among them are sought in the study. The thesis is composed of three chapters. The research; performed alongside with medical staff; serving at a public hospital in the Istanbul province is covered with the purpose of determining the teamwork in the first chapter, patient safety culture in the second chapter and the relationship between the effectiveness of teamwork and patient safety culture in the third chapter. It is established in consequence of the study that there are significant differences between the socio-cultural properties and teamwork efficiency and patient safety culture. Moreover, it is observed that the health professionals display teamwork of mid level effectiveness and error reporting, deployment, task substitution, and assessment of the mistakes; among lower dimensions of patient safety culture; are poor. And it is comprehended that the teamwork efficiency has low level and positive impact upon patient safety culture as another important result.
