Review Citation Count: 0Acute spinal epidural hematoma: A case report and review of the literature(Turkish Assoc Trauma Emergency Surgery, 2020) Akar, Ezgi; Ogrenci, Ahmet; Koban, Orkun; Yilmaz, Mesut; Dalbayrak, SedatSpinal epidural hematoma (SEH) is a rare but a significant cause of spinal cord compression and neurologic deficits. Its etiology is usually unknown and requires emergency intervention. The present study aims to review the clinical significance, treatment strategies and clinical outcomes of traumatic SEH with a rare case presentation. Our patient was a 42-year-old female who presented with back pain and loss of sensation and strength in the legs. The patient did not have any disease and did not use anticoagulant drugs. The patient developed numbness in her legs half an hour after having a traditional back walking massage due to occasional back pain. She was paraplegic and anesthetic when seen in our clinic. Thoracic computed tomography (CT) and magnetic resonance imaging (MRI) revealed posterior epidural hemorrhage at Th3-Th4 levels. In the 12th hour, the hematoma was evacuated by an emergency decompressive hemilaminectomy. At the postoperative 24th hour, the patient had symptomatic improvement, and in the sixth month, the patient was mobilized with support. SEH is a rare condition that should be considered in patients with sudden onset of back pain and extremity weakness. Although the gold standard diagnostic tool is MRI, CT is often sufficient to avoid delayed surgery. Immediate surgical decompression (laminectomy/hemilaminectomy) should be performed in cases diagnosed with SEH with neurological deficits.Article Citation Count: 9Adherence to guideline-directed medical and device Therapy in outpAtients with heart failure with reduced ejection fraction: The ATA study(Turkish Soc Cardiology, 2020) Kocabas, Umut; Kivrak, Tarik; Oztekin, Gulsum Meral Yilmaz; Tanik, Veysel Ozan; Ozdemir, Ibrahim Halil; Kaya, Ersin; Pehlivanoglu, SeckinObjective: Despite recommendations from heart failure guidelines on the use of pharmacologic and device therapy in patients with heart failure with reduced ejection fraction (HFrEF), important inconsistencies in guideline adherence persist in practice. The aim of this study was to assess adherence to guideline-directed medical and device therapy for the treatment of patients with chronic HFrEF (left ventricular ejection fraction <= 40%). Methods: The Adherence to guideline-directed medical and device Therapy in outpAtients with HFrEF (ATA) study is a prospective, multicenter, observational study conducted in 24 centers from January 2019 to June 2019. Results: The study included 1462 outpatients (male: 70.1%, mean age: 67 +/- 11 years, mean LVEF: 30%+/- 6%) with chronic HFrEF. Renin-angiotensin system (RAS) inhibitors, beta-blockers, mineralocorticoid receptor antagonists (MRAs), and ivabradin were used in 78.2%, 90.2%, 55.4%, and 12.1% of patients, respectively. The proportion of patients receiving target doses of medical treatments was 24.6% for RAS inhibitors, 9.9% for beta-blockers, and 10.5% for MRAs. Among patients who met the criteria for implantable cardioverter-defibrillator (ICD) and cardiac resynchronization therapy (CRT), only 16.9% of patients received an ICD (167 of 983) and 34% (95 of 279) of patients underwent CRT (95 of 279). Conclusion: The ATA study shows that most HFrEF outpatients receive RAS inhibitors and beta-blockers but not MRAs or ivabradin when the medical reasons for nonuse, such as drug intolerance or contraindications, are taken into account. In addition, most eligible patients with HFrEF do not receive target doses of pharmacological treatments or guideline-recommended device therapy. Article Citation Count: 0Analysis of complications following posterior vertebral column resection for the treatment of severe angular kyphosis greater than 100(2017) Balioğlu, Mehmet Bülent; Albayrak, Akif; Atıcı, Yunus; Kargın, Deniz; Mert, Muhammed; Kaygusuz, Mehmet AkifObjective: The aim of this study was to evaluate the complications, efŞcacy and safety of posteriorvertebral column resection (PVCR) in severe angular kyphosis (SAK) greater than 100.Methods: The medical records of 17 patients (mean age 17.9 (range, 9e27) years) with SAK who underwent PVCR, were reviewed. Collaborative unmanned systems are used in surveillance, tracking, and military operations. The optimal assignment of these tasks requires cooperation among the vehicles in order to generate a strategy that is efficient with respect to overall mission duration and satisfies all problem constraints. The key motivation behind this paper is to design an unmanned vehicle team that mitigates the disadvantages caused by the structures and characteristics of unmanned ground vehicles (UGVs) and unmanned aerial vehicles (UAVs), which are used in these systems. The design steps of the developed system are explained in three sections in a multidisciplinary fashion. In the first section, the optimal energy consumption profile is estimated by simulating the UAV's flight on a Nvidia Jetson TX2 embedded system, located on the UGV. In the second section, a UAV charger design and implementation is done via development of a TMS320F28335 DSP controlled 65 W inductive wireless power transfer circuit, and in the third section an algorithm that calculates the optimal range and route for the user-selected way points using the state of charge of the UAV's LiPo batteries and a preflight simulation using the simulated annealing method are developed. The experiments are performed in real time with an example scenario and the proposed strategy's suitability and effectiveness are verified for cooperative unmanned vehicle teams.Article Citation Count: 1Castleman's disease presenting with mechanical intestinal obstruction: A rare case(Turkish Assoc Trauma Emergency Surgery, 2020) Kartal, Abdulcabbar; Atli, Eray; Vural, Gurcan; Ferhatoglu, Murat Ferhat; Filiz, Ali IlkerCastleman's disease (CD) is a lymphoproliferative disorder and the occurrence of CD in the small bowel is rare. In this study, we present one case of CD causing mechanical intestinal obstruction due to involvement of terminal ileum. A 50-year-old man was admitted to the hospital with signs and symptoms of mechanical intestinal obstruction without history previous surgery. After examination and obtaining abdominal computed tomography, diagnosis of mechanical intestinal obstruction was reached and emergency surgery was performed with a median incision. On abdominal exploration a tumor like mass that also held distal small intestine mesentery, and ileocecal valve causing complet intestinal obstruction was observed. Ileocecal resection and ileocolonic anastomosis were performed. CD is a rare entity and should be kept in mind during the differential diagnosis of mechanical intestinal obstruction provided that wall thickening in terminal ileum mimicking mass, and accompanying enlargement mesenteric lymph nodes observed during preoperative investigations or intraoperative exploration.Letter Citation Count: 1Causality Association Between COVID-19 Infection and Aortic Dissection(Turkish Society of Cardiology, 2022) Altuğ Tuncer,M.; Sheıkhvatan, Mehrdad; Sheikhvatan,M.; Toulany,M.; Tıbbi Biyoloji Anabilim Dalı / Department of Medical Biology[No abstract available]Article Citation Count: 0Centile charts of cervical length in singleton and twin pregnancies between 16 and 24 weeks of gestation(Galenos Publ House, 2023) Akar, Bertan; Ceylan, Yasin; Kahraman, Alper; Kole, Emre; Caliskan, ErayObjective: The aim of this study was to determine the standard mid-trimester cervical lengths of singleton and twin pregnancies.Material and Methods: This study was conducted by retrospective analysis of mid-trimester transvaginal cervical measurements of women with singleton and twin pregnancies that were examined by a single perinatologist in a single center. Results: A total of 4621 consecutive asymptomatic pregnant women admitting for advanced obstetric ultrasound screening were evaluated. (J Turk Ger Gynecol Assoc 2023; 24: 114-9)Article Citation Count: 2Clinical characteristics, late effects and outcomes in pineoblastomas in children: a single center experience(Turkish J Pediatrics, 2021) Gorgun, Omer; Koç, Şirin; Kebudi, Rejin; Wolff, Johannes E.; Kebudi, Abut; Darendeliler, EminBackground. Pineoblastomas (PB) are rare tumors of the central nervous system and are more common in children. There is no consensus about standard of care. The objective of this study is to analyze the outcome of children with PB. Methods. Six patients with PB who were diagnosed between 1990-2012 were evaluated retrospectively. Demographics, age of diagnosis, first complaint, tumor region, diagnosis type, seeding metastasis to the spinal axis or cerebrospinal fluid (CSF), treatment and survival of these patients were recorded. Results. Three patients had subtotal resection and all patients received chemotherapy and craniospinal irradiation (CSI) after diagnosis. Median follow-up after treatment was 5.5 (range:1-19) years. Two patients are alive with no evidence of disease for 7.5 and 10 years, one of whom was diagnosed with papillary thyroid carcinoma 9.5 years after treatment. One of the patients who died had lived for 19 years after diagnosis. Conclusions. Pineoblastomas are rare but very aggressive tumors; more effective treatment strategies are needed. Survivors should be followed up for late effects such as second malignancies and endocrine deficiencies.Article Citation Count: 1Clinicopathologic and prognostic features in appendiceal malignancies: does tumor invasiveness matter?(Turkish Surgical Assoc, 2019) Senolu, Kazim; Ferhatoglu, Murat Ferhat; Tihan, DenizObjective: To evaluate the survival rates of appendiceal tumors and prognostic factors affecting survival. Material and Methods: Demographic features, tumor characteristics and pre- and post-operative outcomes of the patients were analyzed retrospectively. The study was performed according to the Helsinki declaration. Results: Twenty-three of the 2840 specimens were investigated prospectively. Median age of the patients was 28 (range: 1-89) years, with a male (n=1730, 60.9%) to female (n=1110, 39.1%) ratio of 1.55. Pediatric group did not present appendiceal malignancy. Carcinoid tumors were reported in 17 (0.59%) and adenocarcinoma was reported in 6 (0.20%) patients. Multivariate analyses of the subtypes showed serosal invasion as an independent risk factor for mucinous and non-mucinous adenocarcinoma (HR: -2.70, 95% CI: 0.006-0.755, p=0.029). Median follow-up time was 48 months (range: 28-61 months) and disease specific survival rates of carcinoid tumors, mucinous- and non-mucinous adenocarcinomas were 36(95% CI 32-40), 30 (95% CI 13-46), 43 (95% CI 30-55) months, respectively (p=0.749). Factors affecting survival in the univariate analyses were advanced tumor stage, serosal invasion and tumor invasion depth. In multivariate analyses, tumor invasion depth was the only independent prognostic factor with poor survival rates in all subtypes of appendiceal malignancies (HR=1.31 (95% CI: 1.01-13.5), p=0.047). Conclusion: Tumor subtype and tumor invasiveness are important risk factors for survival. Besides other treatment modalities, appendectomy still remains the survival benefit with better clinical outcomes.Article Citation Count: 1Clinicopathologic and Prognostic Features in Gallbladder Malignancies: Retrospective Analysis of 5206 Cases(Galenos Yayincilik, 2019) Ferhatoglu, Murat Ferhat; Gürkan, Alp; Kivilcim, Taner; Kartal, Abdulcabbar; Gurkan, Alp; Genel Cerrahi / General SurgeryAim: Gallbladder cancer is the sixth most common cancer of the gastrointestinal system. Clinical presentation may not be distinguished from cholelithiasis or cholecystitis and most patients are diagnosed intraoperatively or in the postoperative histologic examination. In this study, we aimed to investigate the association of incidentally detected gallbladder cancer with gallbladder premalignant lesions, age, gender, ultrasonography features and gallbladder stones. Methods: Demographic and clinical characteristics and pathology results of 5206 patients who underwent cholecystectomy between January 2012 and December 2015 were evaluated retrospectively. Results: Three thousand eight hundred and eighty four (74.6%) patients were female. Pathologic reports showed pre-malignant and malignant lesions in 102 (1.95%) cases. Metaplasia was significantly more common in females, while no significant difference was found in development of dysplasia and cancer between genders. Gallbladder stone was found to be a risk factor for the development of metaplasia. Gallbladder wall thickening and advanced age are the most important risk factors for gallbladder cancer. Conclusion: Female gender and gallstone are important risk factors for the development of metaplasia. Advanced age, gallstone and gallbladder wall thickening on ultrasonography are the most important factors in the development cancer. Cholecystectomy should be kept in mind as the most effective method to prevent cancer development in elderly patients with gallbladder stones.Article Citation Count: 2Comparative surgical resection of the ligamentum teres hepatis in a cadaveric model and a patient with ovarian cancer(Galenos Yayincilik, 2019) Selcuk, Ilker; Basarir, Zehra Ozturk; Ohri, Nurian; Akar, Bertan; Caliskan, Eray; Gungor, TayfunResection of all tumor implants with the aim of maximal cytoreduction is the main predictor of overall survival in ovarian carcinoma. However, there are high risk sites of tumor recurrence, and the perihepatic region, especially the point where the ligamentum teres hepatis enters the liver parenchyma under the hepatic bridge (pont hepatique), is one of them. This video demonstrates the resection of the ligamentum teres hepatis both in a cadaveric model and in a patient with ovarian cancer.Article Citation Count: 2Comparison of Perioperative Outcomes and Urethral Complications Between Using 24-French and 26-French Resectoscope Sheaths in Holmium Laser Enucleation of the Prostate(Galenos Publishing House, 2022) Yildiz,A.; Akdemir,S.; Anil,H.; Guzel,A.; Arslan,M.Aim: Although the 26F resectoscope is frequently used in transurethral prostatectomy, there are some concerns with high-caliber shafts. We compared 24F and 26F resectoscope used for Holmium Laser Enucleation of the Prostate (HoLEP) in terms of effects on postoperative urethral complications and perioperative outcomes. Methods: Data from patients undergoing HoLEP from 2017 to 2021 was retrospectively analyzed. All surgeries were completed by a single surgeon. The patients were divided into one of two groups according to the resectoscope diameter (24F or 26F). All patients were followed up for urethral complications for 12 months. Perioperative outcomes and urethral complications were compared between the groups. Results: The study included 301 patients. The mean age of patients was 68.5±8.3 and 69.1±8.6 for the 26F group (n=180) and the 24F group (n=121), respectively (p=0.608). A total of seven in the 26F group (3.8%) and 3 patients in the 24F group (2.4%) had postoperative urethral stricture (US) (p=0.503). Besides, 2 patients (26F) and 1 patient (24F) had postoperative bladder neck contracture (BNC) (p=0.807). The operation efficiency was 1.25 g/min in the 26F group and 1.17 g/min in the 24F group (p=0.005). Conclusion: The use of 24F or 26F RS was not shown to cause statistically significant differences in the incidence of US and BNC during the 12-month follow-up. The use of the 24F RS significantly reduces surgical and morcellation efficiency. © 2022 by The Medical Bulletin of Istanbul Haseki Training and Research Hospital The Medical Bulletin of Haseki published by Galenos Yayinevi.Article Citation Count: 4Comparison of the effects of two different marine-derived omega-3 fatty acid sources, krill oil, and fish oil, on the healing of primary colonic anastomoses after colectomy applied Wistar albino rat model(Turkish Assoc Trauma Emergency Surgery, 2019) Ferhatoglu, Murat Ferhat; Kivilcim, Taner; Vural, Gurcan; Kartal, Abdulcabbar; Filiz, Ali Ilker; Kebudi, AbutBACKGROUND: Oils from marine organisms have a different fatty acid composition. Fish oil (FO) has a high content of eicosapentaenoic and docosahexaenoic acids esterified to triacylglycerols; while in krill oil (KO), fatty acids are primarily esterified to phospholipids. This study aimed to compare the efficacy of two different, marine-derived omega-3 fatty acid sources in the wound healing of colon anastomoses rat model. METHODS: For the study, we used 42 male Wistar albino rats. The rats were divided into six groups with seven rats in each group-CO3: left colonic anastomosis (control group), sacrificed on the third day; KO3: left colonic anastomosis + oral KO, sacrificed on the third day; FO3: left colonic anastomosis + oral FO, sacrificed on the third day; CO7: left colonic anastomosis (control group), sacrificed on the seventh day; KO7: left colonic anastomosis + oral KO, sacrificed on the seventh day; FO7: left colonic anastomosis + oral FO, sacrificed on the seventh day. Peritoneal adhesions, anastomotic bursting pressures, hydroxyproline levels, and histological examination of the anastomotic tissue were evaluated. RESULTS: On day 7, bursting pressure and hydroxyproline measurements of the KO group was significantly higher than the FO group (p=0.012; p=0.002, respectively). Also, on day 7, a statistically significant difference was observed between the groups according to inflammatory cell infiltration, fibroblast activity, neoangiogenesis, and collagen deposition in favor of the KO group (p=0.023; p=0.028; p=0.016; p=0.012, respectively). CONCLUSION: Both KO and FO supplementation in patients before colorectal surgery may reduce some risk of anastomotic leakage; and KO might be a better alternative and excellent omega-3 source.Article Citation Count: 0Comparison of the localization of intrauterine adhesions in pregnant and infertile women Gebe ve infertil kad?nlarda intrauterin adezyonlar?n lokalizasyonlar?n?n kar??la?t?r?lmas?(Galenos Publ House, 2022) Bender, Rukiye Ada; Ozcan, Canan; Akar, Bertan; Caliskan, ErayObjective: Intrauterine adhesion (IUA) is the formation of band-shaped fibrotic tissues in the endometrial cavity due to uterine procedures. Most adhesions remain asymptomatic and do not affect fertility or pregnancy conditions. However, they may lead to infertility and pregnancy complications in some women. This study aimed to determine which localization and type of IUA may lead to infertility. Materials and Methods: Seventy-six women with IUA were retrospectively scanned. Thirty-nine women with IUA with uterine factor-related infertility were included in the infertility group. Thirty-seven pregnant women, who had adhesions in the second-trimester ultrasonography and who had a live birth via cesarean section at term, were included in the pregnancy group. The localization of adhesions was determined as the fundus, corpus, isthmus, and cornu. Concerning the type of adhesion, the adhesions were classified as dense-and film-type adhesions. Results: The infertility group was compared with the pregnancy group according to the type and localization of the adhesions. Fundal adhesions were significantly higher in the infertility group compared to the pregnancy group (p<0.05). The isthmic adhesions, however, were more common in the pregnancy group than in the infertility group (p<0.05). Dense-type adhesions were more common in the infertility group than in the pregnancy group (p<0.05). Conclusion: According to the localization and types of adhesions, fundal and dense-type adhesions are among the features of uterine factor-related infertility. However, isthmus-located and film-type adhesions may not cause infertility.Article Citation Count: 0Design and Implementation of an Electric Actuated Valve for Precise Fluid Control(2019) Kıvanç, Ömer Cihan; Üstün, Özgür; Tosun, Gürkan; Oğuz, Ender; Mutlu, YaşarFluid control is one of the essential automation application area in industry. Materials and Methods: This prospective cohort study consisted of 208 healthy premenopausal women. The Female Sexual Function index (FSFI) and the Female Genital Self-image scale (FGSIS) questionnaires were administered. Participants were divided into two groups according to their female sexual dysfunction (FSD) status. External genital measurements and anterior and posterior vaginal length were measured. Results: The external female genital measurements were (cm, mean ± standard deviation): clitoral prepuce length 2.05±0.48; clitoral glans length 0.87±0.21; clitoral glans width 0.60±0.15; clitoris to urethra 2.24±0.55; anterior fornix depth 7.75±0.92; posterior fornix depth 9.25±0.75; labia minora width, right 2.12±0.86, left 2.20±0.96. A weak negative correlation was found between total FGSIS scores and clitoral prepuce length (p=0.01, r=-0.17), whereas a weak positive correlation was seen between total FGSIS scores and anterior-posterior vaginal lengths (p=0.04, r=0.13; p=0.02, r=0.15, respectively). No statistically significant difference was found between the genital measurements of participants with FSD (n=82, 39.4%) and those without FSD (n=126, 60.6%), and the total FSFI scores and orgasm subdomain scores. Conclusion: The female genital measurements were found to be distributed over a wide range. Although the relationship between genital measurements and genital perception varied, no significant relationship was found between genital measurements and sexual functions or orgasm. These findings suggest that a more cautious approach should be taken towards genital surgeries for cosmetic purposes. © 2020 by Turkish Society of Obstetrics and Gynecology.Article Citation Count: 2THE EFFECT OF DYNAMIC PRICING ON HOLIDAY PURCHASE INTENTIONS: MODERATED MEDIATION ROLE OF PERCEIVED RISK(2019) Sözer, Edin Güçlü; Sözer, Edin Güçlü; İşletme / Business AdministrationThis study targets to measure the effect of dynamic pricing offerson the purchase intentions of consumers for a winter holiday bytaking into consideration a moderated mediator role of perceivedrisk on this effect. The study employs an experimental designwith discount level and timing of the offer (offer recency) as themanipulated conditions. The findings confirm that discountoffers have positive direct effect and perceived risk has anegative direct effect on purchase intentions. The levels ofdiscount and perceived risk, independently from each other, determine the level of purchase intentions. On the other hand, the effect of discount offers on purchase intentions is mediatedby the perceived risk level of consumers. Finally, the timing ofthe discount offers moderates the effect of perceived risk onpurchase intentions and eventually generates a moderatedmediation role for perceived risk on the influence of discountoffers on purchase intentions. Based on the findings of this study, some practical implications are provided.Article Citation Count: 0The effect of Kinesio taping on back pain in patients with Lenke Type 1 adolescent idiopathic scoliosis: A randomized controlled trial(2017) Büyükkuşçu, Mehmet Özbey; Aydin, Canan Gonen; Atıcı, Yunus; Balioğlu, Mehmet Bülent; Atıcı, Ayşegül; Arıkan, YavuzPurpose: This study investigated the short-term effects of KT on back pain (BP) in patients with LenkeType 1 adolescent idiopathic scoliosis (AIS).Methods: We chosen Lenke Type 1 scoliosis who have had only back pain (the localization of the pain:the only in the apical convex edge). Forty patients suffering from BP with Lenke Type 1 AIS wererandomly separated into two groups, Group 1 (20 patients) and Group 2 (20 patients). Group 1 was givenKT with tension and home exercises and Group 2 was given KT without tension and home exercises. KTand home exercises was applied to the thoracic area of the patients in both groups for four weeks. Painintensity was measured using a visual analog scale (VAS) and SRS-22 (subtotal SRS-20) before and aftertreatment. Results: Mean age of both groups was 16.1 years. Mean Cobb angle of the thoracic scoliosis was 31.8(range: 17e44) in Group 1 and 32.8(range: 19e43) in Group 2 before the treatment. The decrease inVAS score of Group 1 after taping was higher than that of Group 2. The difference between the pre- andpost-treatment VAS scores of both groups was statistically signiŞcant (p < 0.05). The increase in meanSRS-20 score of Group 1 following taping application was signiŞcantly higher than the increase in thecontrol group (p< 0.05).Conclusion: Results demonstrated that KT application with tension effectively leads to back pain reliefshortly after application. In addition, KT has a positive impact on quality of life. Thus, KT may be asuitable intervention in treating back pain of patients with AIS.Level of Evidence: Level 1, Therapeutic study.
