Review Acute spinal epidural hematoma: A case report and review of the literature(Turkish Assoc Trauma Emergency Surgery, 2020) Akar, Ezgi; Ogrenci, Ahmet; Koban, Orkun; Yilmaz, Mesut; Dalbayrak, SedatSpinal epidural hematoma (SEH) is a rare but a significant cause of spinal cord compression and neurologic deficits. Its etiology is usually unknown and requires emergency intervention. The present study aims to review the clinical significance, treatment strategies and clinical outcomes of traumatic SEH with a rare case presentation. Our patient was a 42-year-old female who presented with back pain and loss of sensation and strength in the legs. The patient did not have any disease and did not use anticoagulant drugs. The patient developed numbness in her legs half an hour after having a traditional back walking massage due to occasional back pain. She was paraplegic and anesthetic when seen in our clinic. Thoracic computed tomography (CT) and magnetic resonance imaging (MRI) revealed posterior epidural hemorrhage at Th3-Th4 levels. In the 12th hour, the hematoma was evacuated by an emergency decompressive hemilaminectomy. At the postoperative 24th hour, the patient had symptomatic improvement, and in the sixth month, the patient was mobilized with support. SEH is a rare condition that should be considered in patients with sudden onset of back pain and extremity weakness. Although the gold standard diagnostic tool is MRI, CT is often sufficient to avoid delayed surgery. Immediate surgical decompression (laminectomy/hemilaminectomy) should be performed in cases diagnosed with SEH with neurological deficits.Article Adherence to guideline-directed medical and device Therapy in outpAtients with heart failure with reduced ejection fraction: The ATA study(Turkish Soc Cardiology, 2020) Kocabas, Umut; Kivrak, Tarik; Oztekin, Gulsum Meral Yilmaz; Tanik, Veysel Ozan; Ozdemir, Ibrahim Halil; Kaya, Ersin; Pehlivanoglu, SeckinObjective: Despite recommendations from heart failure guidelines on the use of pharmacologic and device therapy in patients with heart failure with reduced ejection fraction (HFrEF), important inconsistencies in guideline adherence persist in practice. The aim of this study was to assess adherence to guideline-directed medical and device therapy for the treatment of patients with chronic HFrEF (left ventricular ejection fraction <= 40%). Methods: The Adherence to guideline-directed medical and device Therapy in outpAtients with HFrEF (ATA) study is a prospective, multicenter, observational study conducted in 24 centers from January 2019 to June 2019. Results: The study included 1462 outpatients (male: 70.1%, mean age: 67 +/- 11 years, mean LVEF: 30%+/- 6%) with chronic HFrEF. Renin-angiotensin system (RAS) inhibitors, beta-blockers, mineralocorticoid receptor antagonists (MRAs), and ivabradin were used in 78.2%, 90.2%, 55.4%, and 12.1% of patients, respectively. The proportion of patients receiving target doses of medical treatments was 24.6% for RAS inhibitors, 9.9% for beta-blockers, and 10.5% for MRAs. Among patients who met the criteria for implantable cardioverter-defibrillator (ICD) and cardiac resynchronization therapy (CRT), only 16.9% of patients received an ICD (167 of 983) and 34% (95 of 279) of patients underwent CRT (95 of 279). Conclusion: The ATA study shows that most HFrEF outpatients receive RAS inhibitors and beta-blockers but not MRAs or ivabradin when the medical reasons for nonuse, such as drug intolerance or contraindications, are taken into account. In addition, most eligible patients with HFrEF do not receive target doses of pharmacological treatments or guideline-recommended device therapy. Article Analysis of complications following posterior vertebral column resection for the treatment of severe angular kyphosis greater than 100(2017) Balioğlu, Mehmet Bülent; Albayrak, Akif; Atıcı, Yunus; Kargın, Deniz; Mert, Muhammed; Kaygusuz, Mehmet AkifObjective: The aim of this study was to evaluate the complications, efŞcacy and safety of posteriorvertebral column resection (PVCR) in severe angular kyphosis (SAK) greater than 100.Methods: The medical records of 17 patients (mean age 17.9 (range, 9e27) years) with SAK who underwent PVCR, were reviewed. Collaborative unmanned systems are used in surveillance, tracking, and military operations. The optimal assignment of these tasks requires cooperation among the vehicles in order to generate a strategy that is efficient with respect to overall mission duration and satisfies all problem constraints. The key motivation behind this paper is to design an unmanned vehicle team that mitigates the disadvantages caused by the structures and characteristics of unmanned ground vehicles (UGVs) and unmanned aerial vehicles (UAVs), which are used in these systems. The design steps of the developed system are explained in three sections in a multidisciplinary fashion. In the first section, the optimal energy consumption profile is estimated by simulating the UAV's flight on a Nvidia Jetson TX2 embedded system, located on the UGV. In the second section, a UAV charger design and implementation is done via development of a TMS320F28335 DSP controlled 65 W inductive wireless power transfer circuit, and in the third section an algorithm that calculates the optimal range and route for the user-selected way points using the state of charge of the UAV's LiPo batteries and a preflight simulation using the simulated annealing method are developed. The experiments are performed in real time with an example scenario and the proposed strategy's suitability and effectiveness are verified for cooperative unmanned vehicle teams. Review Bouveret's Syndrome: A Case-Based Review, Clinical Presentation, Diagnostics and Treatment Approaches(Kare Publ, 2020) Ferhatoglu, Murat Ferhat; Kartal, AbdulcabbarGastric outlet obstruction that arises from gallstones impacted in the distal stomach or proximal duodenum after passing through a cholecystoduodenal, cholecystogastric or rarely choledocoduodenal fistula is called as Bouveret's syndrome and it accounts for approximately 1-3% of all the patients with gallstone ileus. Although treatment modalities, including stone removal or fragmentation with classical endoscopic devices, such as snares, and forceps or fragmentation of gallstones with new devices, such as electrohydraulic lithotripsy, laser, extracorporeal shockwave lithotripsy have been described. However, only 29% of the patients benefit from nonsurgical methods. Removal of the stone through a gastrotomy or enterotomy and performing cholecystectomy and fistula repair with a second operation is an approach recommended for older patients with comorbid diseases. In this paper, a case of Bouveret's syndrome was presented. The authors also aimed to review the diagnosis, management and treatment of this rare disease and to update the previous reviews.Article Castleman's disease presenting with mechanical intestinal obstruction: A rare case(Turkish Assoc Trauma Emergency Surgery, 2020) Kartal, Abdulcabbar; Atli, Eray; Vural, Gurcan; Ferhatoglu, Murat Ferhat; Filiz, Ali IlkerCastleman's disease (CD) is a lymphoproliferative disorder and the occurrence of CD in the small bowel is rare. In this study, we present one case of CD causing mechanical intestinal obstruction due to involvement of terminal ileum. A 50-year-old man was admitted to the hospital with signs and symptoms of mechanical intestinal obstruction without history previous surgery. After examination and obtaining abdominal computed tomography, diagnosis of mechanical intestinal obstruction was reached and emergency surgery was performed with a median incision. On abdominal exploration a tumor like mass that also held distal small intestine mesentery, and ileocecal valve causing complet intestinal obstruction was observed. Ileocecal resection and ileocolonic anastomosis were performed. CD is a rare entity and should be kept in mind during the differential diagnosis of mechanical intestinal obstruction provided that wall thickening in terminal ileum mimicking mass, and accompanying enlargement mesenteric lymph nodes observed during preoperative investigations or intraoperative exploration.Letter Causality Association Between Covid-19 Infection and Aortic Dissection(Aves, 2022) Tuncer, Mehmet Altug; Sadeghian, Hakimeh; Sheikhvatan, Mehrdad; Toulany, MahtabArticle Centile charts of cervical length in singleton and twin pregnancies between 16 and 24 weeks of gestation(Galenos Publ House, 2023) Akar, Bertan; Ceylan, Yasin; Kahraman, Alper; Kole, Emre; Caliskan, ErayObjective: The aim of this study was to determine the standard mid-trimester cervical lengths of singleton and twin pregnancies.Material and Methods: This study was conducted by retrospective analysis of mid-trimester transvaginal cervical measurements of women with singleton and twin pregnancies that were examined by a single perinatologist in a single center. Results: A total of 4621 consecutive asymptomatic pregnant women admitting for advanced obstetric ultrasound screening were evaluated. Two patients are alive with no evidence of disease for 7.5 and 10 years, one of whom was diagnosed with papillary thyroid carcinoma 9.5 years after treatment. One of the patients who died had lived for 19 years after diagnosis. Conclusions. Pineoblastomas are rare but very aggressive tumors; more effective treatment strategies are needed. Survivors should be followed up for late effects such as second malignancies and endocrine deficiencies.Article A Comparative Analysis of Once-daily and Twice-daily Formulation of Tacrolimus in De Novo Kidney Transplant Recipients(Kare Publ, 2021) Ferhatoglu, Murat Ferhat; Kartal, Abdulcabbar; Kivilcim, Taner; Filiz, Ali Ilker; Yildiz, Gursel; Gurkan, AlpObjectives: We aimed to compare the once-daily and twice-daily formulation of tacrolimus concerning the efficiency and effects on graft function in de novo kidney transplant patients. Methods: Twenty once-daily (TAC-OD) and twenty twice-daily (TAC-BID) tacrolimus administrated de novo kidney recipients who had received initial immunosuppressive therapy according to protocols at our institution (0.2 mg/kg of tacrolimus combined with 1000 milligrams of steroid taper plus 720 mg of mycophenolate and with 2.5mg/kg anti-thymocyte globulin) assessed concerning demographics, drug doses and blood concentration, and graft function. Results: The mean tacrolimus blood concentration measurements were higher in the TAC-OD group in the first sixty days after transplantation, and the TAC- OD group showed more blood concentration overshoots/fluctuations in the first 30 days of the treatment. The initial drug dose was significantly higher in the TAC-OD group than the TAC-BID group (p=0.04). There was no meaningful difference among groups according to graft function (creatinine measurements) (p>0.05). Conclusion: Between de novo kidney recipients, the new TAC-OD formulation presents a similar short-term efficacy profile as TACBID. However, a higher daily dosage of TAC-OD is needed to achieve similar blood concentrations in the early postoperative period.Article Comparative surgical resection of the ligamentum teres hepatis in a cadaveric model and a patient with ovarian cancer(Galenos Yayincilik, 2019) Selcuk, Ilker; Basarir, Zehra Ozturk; Ohri, Nurian; Akar, Bertan; Caliskan, Eray; Gungor, TayfunResection of all tumor implants with the aim of maximal cytoreduction is the main predictor of overall survival in ovarian carcinoma. However, there are high risk sites of tumor recurrence, and the perihepatic region, especially the point where the ligamentum teres hepatis enters the liver parenchyma under the hepatic bridge (pont hepatique), is one of them. This video demonstrates the resection of the ligamentum teres hepatis both in a cadaveric model and in a patient with ovarian cancer.Article Comparison of the effects of two different marine-derived omega-3 fatty acid sources, krill oil, and fish oil, on the healing of primary colonic anastomoses after colectomy applied Wistar albino rat model(Turkish Assoc Trauma Emergency Surgery, 2019) Ferhatoglu, Murat Ferhat; Kivilcim, Taner; Vural, Gurcan; Kartal, Abdulcabbar; Filiz, Ali Ilker; Kebudi, AbutBACKGROUND: Oils from marine organisms have a different fatty acid composition. Fish oil (FO) has a high content of eicosapentaenoic and docosahexaenoic acids esterified to triacylglycerols; while in krill oil (KO), fatty acids are primarily esterified to phospholipids. This study aimed to compare the efficacy of two different, marine-derived omega-3 fatty acid sources in the wound healing of colon anastomoses rat model. METHODS: For the study, we used 42 male Wistar albino rats. The rats were divided into six groups with seven rats in each group-CO3: left colonic anastomosis (control group), sacrificed on the third day; KO3: left colonic anastomosis + oral KO, sacrificed on the third day; FO3: left colonic anastomosis + oral FO, sacrificed on the third day; CO7: left colonic anastomosis (control group), sacrificed on the seventh day; KO7: left colonic anastomosis + oral KO, sacrificed on the seventh day; FO7: left colonic anastomosis + oral FO, sacrificed on the seventh day. Peritoneal adhesions, anastomotic bursting pressures, hydroxyproline levels, and histological examination of the anastomotic tissue were evaluated. RESULTS: On day 7, bursting pressure and hydroxyproline measurements of the KO group was significantly higher than the FO group (p=0.012; p=0.002, respectively). Also, on day 7, a statistically significant difference was observed between the groups according to inflammatory cell infiltration, fibroblast activity, neoangiogenesis, and collagen deposition in favor of the KO group (p=0.023; p=0.028; p=0.016; p=0.012, respectively). CONCLUSION: Both KO and FO supplementation in patients before colorectal surgery may reduce some risk of anastomotic leakage; and KO might be a better alternative and excellent omega-3 source.Article Comparison of the localization of intrauterine adhesions in pregnant and infertile women Gebe ve infertil kad?nlarda intrauterin adezyonlar?n lokalizasyonlar?n?n kar??la?t?r?lmas?(Galenos Publ House, 2022) Bender, Rukiye Ada; Ozcan, Canan; Akar, Bertan; Caliskan, ErayObjective: Intrauterine adhesion (IUA) is the formation of band-shaped fibrotic tissues in the endometrial cavity due to uterine procedures. Most adhesions remain asymptomatic and do not affect fertility or pregnancy conditions. However, they may lead to infertility and pregnancy complications in some women. This study aimed to determine which localization and type of IUA may lead to infertility. Materials and Methods: Seventy-six women with IUA were retrospectively scanned. Thirty-nine women with IUA with uterine factor-related infertility were included in the infertility group. Thirty-seven pregnant women, who had adhesions in the second-trimester ultrasonography and who had a live birth via cesarean section at term, were included in the pregnancy group. The localization of adhesions was determined as the fundus, corpus, isthmus, and cornu. Concerning the type of adhesion, the adhesions were classified as dense-and film-type adhesions. Results: The infertility group was compared with the pregnancy group according to the type and localization of the adhesions. Fundal adhesions were significantly higher in the infertility group compared to the pregnancy group (p<0.05). The isthmic adhesions, however, were more common in the pregnancy group than in the infertility group (p<0.05). Dense-type adhesions were more common in the infertility group than in the pregnancy group (p<0.05). Conclusion: According to the localization and types of adhesions, fundal and dense-type adhesions are among the features of uterine factor-related infertility. However, isthmus-located and film-type adhesions may not cause infertility.Article Design and Implementation of an Electric Actuated Valve for Precise Fluid Control(Gazi Univ, 2019) Kivanc, Omer Cihan; Ustun, Ozgur; Tosun, Gurkan; Oguz, Ender; Mutlu, YasarFluid control is one of the essential automation application area in industry. The study group consists of 855 individuals between the ages of 18-65 from different genders, and have experienced the pandemic process. For the construct validity of the scale results, exploratory factor analysis was conducted and a one-dimensional structure consisting of 22 items was revealed. It was determined that the variance explained by the scale showing a one-dimensional structure was 41.5%. As a result of the confirmatory factor analysis performed through a separate study group, it was revealed that all items have significant t values, and the model established according to model fit indexes has meaningful and acceptable fit values. Buss-Perry Aggression Scale was applied with HPS for the criterion validity. As a result of the criterion validity analysis, a significant relationship was found between the scale scores. The Cronbach Alpha was calculated to analyses internal consistency of the scale and a reliability level of 0.93 was obtained. The test-retest reliability results were found as 0.89. In addition, item statistics revealed that all of the scale items can discriminate well among the respondents. Results of the analysis revealed that, the Hostility Scale in Pandemic Process provides valid and reliable results.Article Do External Female Genital Measurements Affect Genital Perception and Sexual Function and Orgasm(Galenos Yayıncılık, 2020) Kaya, Aski Ellibes; Dogan, Ozan; Yassa, Murat; Basbug, Alper; Ozcan, Canan; Caliskan, ErayObjective: To provide baseline data for the anatomy of the external female genitalia and to investigate the correlation between those measurements and sexual function and genital perception. Materials and Methods: This prospective cohort study consisted of 208 healthy premenopausal women. The Female Sexual Function index (FSFI) and the Female Genital Self-image scale (FGSIS) questionnaires were administered. Participants were divided into two groups according to their female sexual dysfunction (FSD) status. External genital measurements and anterior and posterior vaginal length were measured. Results: The external female genital measurements were (cm, mean +/- standard deviation): clitoral prepuce length 2.05 +/- 0.48; clitoral glans length 0.87 +/- 0.21; clitoral glans width 0.60 +/- 0.15; clitoris to urethra 2.24 +/- 0.55; anterior fornix depth 7.75 +/- 0.92; posterior fornix depth 9.25 +/- 0.75; labia minora width, right 2.12 +/- 0.86, left 2.20 +/- 0.96. A weak negative correlation was found between total FGSIS scores and clitoral prepuce length (p=0.01, r=-0.17), whereas a weak positive correlation was seen between total FGSIS scores and anterior-posterior vaginal lengths (p=0.04, r=0.13; p=0.02, r=0.15, respectively). No statistically significant difference was found between the genital measurements of participants with FSD (n=82, 39.4%) and those without FSD (n=126, 60.6%), and the total FSFI scores and orgasm subdomain scores. Conclusion: The female genital measurements were found to be distributed over a wide range. Although the relationship between genital measurements and genital perception varied, no significant relationship was found between genital measurements and sexual functions or orgasm. These findings suggest that a more cautious approach should be taken towards genital surgeries for cosmetic purposes.Article The Effect of Intermittent and Continuous Feeding on Growth and Discharge Time in Very Low Birth Weight Preterm Infants(Kare Publ, 2021) Selalmaz, Melek; Uysal, Gulzade; Zubarioglu, Umut; Bulbul, AliObjectives: The aim of this study was to determine the effect of intermittent bolus feeding and continuous feeding models on early growth and discharge time in very low birth weight infants. Methods: The study was designed as a prospective, randomized, and controlled study. Infants born in our hospital with birth weight below 1500 g within a 1 year period were included in the study. The number of samples was determined by power analysis. Babies were randomized according to birth weight and fed with intermittent bolus feeding and continuous feeding models. Demographic characteristics, clinical findings, diagnosis, nutritional status, and length of hospital stay were compared. Results: The study was conducted with 80 preterm infants, which consisted of continuous feeding (n=41) and intermittent bolus feeding (n=39). There was no significant difference in gender, gestational week, birth weight, height, and head circumference distribution of the babies between groups. The difference between the reach time to birth weight and maximum weight loss rates, parenteral feeding time, transition time to full enteral feeding, transition time to oral feeding, development of feeding intolerance, mechanical ventilation time, and hospitalization time in intensive care unit were not statistically significant. Necrotizing enterocolitis (NEC) Stage I and II developed in 34.1% of babies fed with continuous feeding model and 28.2% of babies fed intermittently; NEC was detected to start in 4.5 +/- 2.8 days in the continuous feeding group and in 2.8 +/- 5.2 days in the intermittent group. These differences were found to be insignificant between the two groups (p=0.634 and p=0.266, respectively). Conclusion: There was no difference between growth parameters and discharge time of preterm babies who were applied continuous and intermittent bolus feeding model. Although there was no statistically significant difference on the development of NEC, it was determined that NEC developed earlier in the intermittent bolus feeding model.Article The effect of Kinesio taping on back pain in patients with Lenke Type 1 adolescent idiopathic scoliosis: A randomized controlled trial(2017) Büyükkuşçu, Mehmet Özbey; Aydin, Canan Gonen; Atıcı, Yunus; Balioğlu, Mehmet Bülent; Atıcı, Ayşegül; Arıkan, YavuzPurpose: This study investigated the short-term effects of KT on back pain (BP) in patients with LenkeType 1 adolescent idiopathic scoliosis (AIS).Methods: We chosen Lenke Type 1 scoliosis who have had only back pain (the localization of the pain:the only in the apical convex edge). Forty patients suffering from BP with Lenke Type 1 AIS wererandomly separated into two groups, Group 1 (20 patients) and Group 2 (20 patients). Group 1 was givenKT with tension and home exercises and Group 2 was given KT without tension and home exercises. KTand home exercises was applied to the thoracic area of the patients in both groups for four weeks. Painintensity was measured using a visual analog scale (VAS) and SRS-22 (subtotal SRS-20) before and aftertreatment. Results: Mean age of both groups was 16.1 years. Mean Cobb angle of the thoracic scoliosis was 31.8(range: 17e44) in Group 1 and 32.8(range: 19e43) in Group 2 before the treatment. The decrease inVAS score of Group 1 after taping was higher than that of Group 2. The difference between the pre- andpost-treatment VAS scores of both groups was statistically signiŞcant (p < 0.05). The increase in meanSRS-20 score of Group 1 following taping application was signiŞcantly higher than the increase in thecontrol group (p< 0.05).Conclusion: Results demonstrated that KT application with tension effectively leads to back pain reliefshortly after application. In addition, KT has a positive impact on quality of life. Thus, KT may be asuitable intervention in treating back pain of patients with AIS.Level of Evidence: Level 1, Therapeutic study.
