Article Accuracy of Procalcitonin in the Diagnosis of Bacteremia and Discrimination from Contamination(Istanbul University Press, 2025) Hayirlioglu, N.; Kaya, A.D.; Selale, D.S.; Kurç, M.A.; Varol, G.Objective: In this study, we aimed to evaluate the concordance of blood culture with procalcitonin (PCT) alone and together with C-reactive protein (CRP) in detecting bacteremia and the diagnostic performance of these biomarkers to differentiate contamination from true bacteremia. Materials and Methods: The medical records of 310 patients were analysed retrospectively. Advia Centaur XP immunoassay system and Au analysers were used to determine PCT and CRP levels, respectively. BacT/ Alert 3D60 hemoculture system was used to incubate blood specimens, and VITEK 2 compact was used to identify isolated strains. Results: The accuracy of PCT and CRP in detecting bacteremia were found to be 68.1% and 36.4%, respectively, and combining PCT and CRP had no added value. In analysis of receiver operating characteristic (ROC), the area under the ROC curve (AUROC) values of PCT and CRP were found to be 0.889 and 0.779 in discriminating the culture-negative group from the culture-positive group, and 0.645 and 0.502 in discriminating bacteremia from contamination, respectively. Conclusion: PCT is a reliable marker that can be used to detect bacteremia. However, its discriminative power was low in differentiating true bacteremia from contamination. Therefore, PCT levels alone should not be used to rule out blood culture contamination. © 2025 Elsevier B.V., All rights reserved. Article An analysis of general surgery theses set up between years 1998-2018 in Turkey: Evidence levels and publication rates of 1996 theses(Turkish Surgical Assoc, 2020) Ferhatoglu, Murat Ferhat; Kivilcim, Taner; Kartal, Abdulcabbar; Filiz, Ali Llker; Kebudi, AbutObjective: Setting up and advocating a thesis is mandatory at the end of the residency training program to become a specialist in general surgery according to the regulations on medical specialization in Turkey. Writing a thesis helps the resident to learn to ask structured questions, assembling the most accurate study design, managing the study process, collecting the results and building a conclusion with medical implications. In this descriptive study, we aimed to investigate the publication rates of the theses written in the field of general surgery and to assess the properties of the published theses. Material and Methods: We performed an online search on September 1, 2018, about the theses of general surgery residents on the website of National Thesis data center of Academic Educational Board in Turkey including theses of medical residents in university-affiliated hospitals and analyzed theses accomplished between 1998-2018. The publication status of the theses was assessed by the entry of author name, the title of the theses and keywords of the theses by using the search engines of PubMed, Google Scholar and Turkish Academic Network and Information Center Turkish Database (ULAKBIM). Data were presented in a descriptive form as absolute numbers and percentages. Results: Between 1998-2018, 1996 theses were completed. 393 (20.5%) of these were published in a journal, and 288 (14.4%) were published in a journal indexed in SCI/SCIE. According to research methodologies, 79.2% of the experimental studies were published in SCI/SCIE indexed journals. Conclusion: Publication rates of the theses in the field of general surgery are low as they are in other specialties of medicine. This descriptive study might give an idea about the low scientific publication rates of general surgery theses. Further studies are needed to understand the underlying factors, which are responsible for this scant scientific performance.Article Analysis of the Factors Affecting the Latency Period and Type of Birth in Patients With Preterm Prematur Rupture of Membranes(Dokuz Eylul Univ inst Health Sciences, 2022) İlhan, Yunus; Kuzu, Ozan; Gozel, Sercan; Yumru, Ayşe EnderPurpose: To determine the factors associated with the latent period in patients with preterm premature rupture of membranes (PPROM). Material and Methods: 53 patients who applied to our clinic within 1 year and were diagnosed with PPROM and met the inclusion criteria participated in this prospective cross-sectional study. Factors thought to affect the latent period in these patients were examined in terms of delivery time and mode. Results: Latent period; was negatively correlated with dilatation and positively correlated with collum length. The mean latent time of patients with contraction was statistically significantly lower than those without. In another model, gestational week, collum length and birth weight were found to be the most significant factors associated with the latent period. In the latent period groups; Statistically, a significant difference was found in the mean dilatation and collum length. The contraction rate was higher in those with a latency period of 2 days or less. Conclusion: Latent duration is inversely proportional to dilatation and directly proportional to collum length. Uterine contraction is higher in patients with a latency period of fewer than 2 days. Other statistically significant factors related to the latent period are the gestational week and the fetal weight. Article Antioxidant Activity, Chemical Composition, and Therapeutic Activity of Essential Oils From Certain Lamiaceae Species in Türkiye(Marmara Univ., Fac. Pharmacy, 2025) Karatoprak, Gökçe Şeker; İlgün, Selen; Dumlupınar, Berrak; Bağdat, Reyhan Bahtiyarca; Sürücü, Canan Göksu; Gürbüz, BurçakThe study examined the antioxidant, antimicrobial, and potential anticancer properties of lavandin (Lavandula x intermedia 'Grosso'), oregano (Origanum vulgare 'Uluğ Bey'), and sage (Salvia officinalis 'Beyhekim') essential oils (EOs). Chemical analysis revealed that oregano is rich in carvacrol (81.23%), cymene (5.51%), and 18- cineole (2.13%). Sage contains notable quantities of α-thujone (17.47%), camphor (13.59%), and borneol (12.14%). Lavandin is predominantly composed of linalool (41.10%), linalyl acetate (16.66%), and camphor (10.01%). The antimicrobial tests demonstrated that lavandin EOs exhibited a high inhibition rate of 90.77% at 1000 μg/mL, which subsequently decreased with lower concentrations. The oregano essential oils demonstrated robust antimicrobial activity across all concentrations, with a 10.97% inhibition rate observed even at the lowest concentration. The sage EOs exhibited a rapid decline in antimicrobial activity, reaching 4.97% at the lowest concentration. Both the DPPH and ABTS assays demonstrated that the antioxidant capacity of the essential oils was concentration-dependent. The cytotoxicity values of oregano essential oil were 35.58 at 50 μg/mL, 32.39 at 25 μg/mL, and 30.92 at 12.5 μg/mL. Similar cytotoxicity results were observed for sage and lavandin essential oils. These findings indicate that plant essential oils exert a dose-dependent cytotoxic effect on cell growth, significantly inhibiting cells at high concentrations. The research indicates that essential oils derived from Lamiaceae plants exhibit robust antimicrobial effects against a range of pathogens, suggesting potential applications as alternative therapies for specific microbial infections, comparable to ciprofloxacin at certain concentrations.Article Assessment of IL-7RA T244I Polymorphism as a Risk Factor of MultipleSclerosis in Turkish Population(2020) Nihal IŞIK; Miraç Aysen ÜNSAL; Nüket Yıldız MANUKYAN; Güher SARUHAN DİRESKENELİIntroduction: The T244I variant of the IL-7RA gene, rs6897932, is one of the first polymorphisms found to be associated with the development of multiple sclerosis (MS). Although several studies provided evidence for the association of MS and this variant, other studies could not confirm this result. These inconsistent results suggest that the role of this polymorphism in the development of disease is associated with ethnicity. Methods: We investigated rs6897932 polymorphisms in a large cohort of patients with MS and healthy controls in a turkish population. Results: In our study, there were no significant differences in genotype frequencies in the IL-7RA rs6897932 polymorphism and no significant difference between C and T alleles in patients with MS and controls. Conclusion: This study is the first to evaluate the risk of the rs6897932 polymorphism in turkish patients with MS.Article Assessment of the Relationship Between Nutrition and Obesity in Adolescence(2021) Aslıhan ŞAHİNER; Burcu YEŞİLKAYAObjective: Adolescent obesity is a widely important situation around the world. In results, the percentiles of the Body Mass Index (BMI) by age and weight for age, females have more normal results (p<0.05). The majority of participants had normal BMI values. The BMI values of the students who have an obese person in their family were higher (p<0.05). 42.6% of the students skipped meals while at home. The results of food frequency survey showed that protein, cholesterol, thiamine, riboflavin, sodium, calcium, magnesium, phosphorus and zinc were higher in males (p<0.05). Conclusion: Adolescents need nutritional education on healthy eating, meal timing and snacking. It is recommended to provide nutrition education to students and families in institutions and to provide nutritionists for the monitoring of nutrition. It will affect adulthood and the next generations positively.Article The Association Between Monocyte/HDL-C Ratio and Heart Failure(Marmara Univ., Fac. Pharmacy, 2025) Gungor, Baris; Aksoy, Nilay; Berk, Barkin; Çattık, Büşra NurThe monocyte/HDL-cholesterol ratio (MHR) was shown to be a marker of inflammation. This study investigated the utilization of this ratio as a measure of severity for heart failure which is a condition associated with inflammation. The MHR was calculated for 323 ambulatory patients with chronic heart failure and compared to other variables associated with the severity of the condition. Additionally, the impact of MHR on the Seattle Heart Failure Model (SHFM) score was investigated. MHR correlated positively with C-reactive protein (r: 0.312, p<0.001) and neutrophil-to-lymphocyte ratio (r: 0.242, p<0.001), but not with platelet-to-lymphocyte ratio. In addition, a correlation was found between the SHFM score and MHR (r:-0.267, p<0.001). The SHFM score exhibited a significant result for proB- type natriuretic peptide (pro-BNP) (p<0.001), neutrophil (p<0.001), hematocrit (p=0.001), and serum creatinine (p=0.001) in the ordinal logistic regression analysis, but not for MHR. MHR showed a negative correlation with left ventricular ejection fraction (r: -0.151, p: 0.007), exhibited a positive association with pro-BNP (r: 0.184, p<0.001), and no correlation with New York Heart Association classes. There is a significant correlation between the MHR value and the factors associated with the severity of heart failure. The prognosis and management of this condition may be assessed by utilizing the MHR value in conjunction with existing biomarkers.Article Association between Omega-6 and Omega-3 Polyunsaturated Fatty Acids Intake and IL-6 G(-174)C Polymorphism with Hs-CRP Level in Healthy Subjects(Galenos Publ House, 2022) Girgin, Erhan; Ongun Yilmaz, HandeObjective: Studies have shown that omega-3 and omega-6 polyunsaturated fatty acids play a major role in the prevention of inflammation, and interleukin-6 (IL-6) levels can be associated with inflammation. The aim of this study was to investigate the relationship between G(-174)C polymorphism of IL-6 gene with dietary omega-3 and omega-6 intake and high sensitive C-reactive protein (CRP) (hs-CRP] level. Methods:This retrospective studywas carried out with a total of 302 healthy individuals, 152 women and 150 men, with a mean age of 50.1 years. Food consumption records, biochemical measurements, and genetic analyztes of individuals were recorded from patient files. Daily energy, fat, cholesterol, omega-6 and omega-3 intakes were evaluated from food consumption records. Results: The CC variation was observed in 7%, GC variation in 31%, and GG variation in 62% of the subjects. Dietary cholesterol intake of individuals with CC variation and omega-6/omega-3 intake of individuals with GG variation were found to be positively and significantly correlated with hs-CRP (r=0.155; p=0.035). Conclusion: Inflammation is the complete response of the organism to the stimuli. In order to prevent the formation of inflammation, it is recommended that individuals with GG variation increase their omega-6/omega-3 consumption rates in their diets towards omega-3.Article Asthmatic Patients: Is Homocysteine an Issue(Galenos Publ House, 2024) Keskinel, İlkay; Eryilmaz, Müzeyyen; Güntürk, ArzuObjective: Understanding the causes of asthma is crucial for developing new therapeutic modalities. Homocysteine (HCY) is an intermediate in methionine metabolism. Elevated HCY levels may indicate folate and vitamin B12 deficiencies, which are cofactors for the methylation of HCY to methionine. The relationship between hyperHCY mia and atherosclerosis is well-documented, and it is considered a cause of cardiovascular, neurodegenerative, and ocular diseases. HyperHCY mia may also cause atopy and, consequently, asthma. We aimed to evaluate the levels of HCY, vitamin B12, and folic acid in asthmatic patients and healthy adults, as well as to determine whether correlations exist between these levels and lung function, eosinophil counts, total immunoglobulin E (IgE), and eosinophilic cationic protein (ECP) levels in asthmatic subjects. Methods: A total of 142 asthmatic patients and 36 healthy controls were enrolled in the study. Folic acid, vitamin B12, total IgE, ECP, eosinophil percentage, eosinophil counts, and HCY levels were evaluated in both groups. Results: HCY, vitamin B12, and folic acid levels did not significantly differ between patients with asthma and controls. There was a statistically significant positive correlation (at the 0.95 confidence level) between HCY values and forced vital capacity, peak expiratory flow (PEF), and eosinophil counts in patients with asthma. Folic acid levels correlated positively only with PEF%, whereas vitamin B12 levels did not correlate with any functional parameters or atopic markers like IgE and ECP. Conclusion: Should large-scale randomized controlled trials conclusively establish HCY as a causative factor of asthma, metabolic interventions to lower HCY levels using methyl donors could be considered alongside conventional asthma treatments.Article Awareness of child abuse and neglect among students(Kare Publ, 2022) Uysal, Gulzade; Bozkurt, Gulcin; Duzkaya, Duygu SonmezObjectives: This descriptive study was conducted to determine the level of awareness of child abuse and neglect (CAN) among students at a private university in Istanbul, Turkey. Methods: The study sample comprised 1533 students from all faculties and colleges of the university selected using the stratified sampling method. A demographic information form and the Child Abuse and Neglect Awareness Scale (CANAS) were used to collect the study data. Results:The findings revealed that 30.5% of the students had received some instruction about CAN and 72.9% indicated that they wanted lectures on CAN to be added to the curriculum. The mean total score of the CANAS and the mean scores of the physical abuse, emotional abuse, and neglect subscales were statistically significantly higher among female students (p<0.05). The physical abuse and neglect subscale scores of students who had formal education on abuse were higher than those of students who had not (p<0.05). Conclusion:The female students and those who had received instruction about abuse had greater awareness of abuse and neglect. Broader coverage in the curriculum may increase the awareness of all students and help to enhance public understanding. A Clinical Observational Prospective Study(Turkiye Klinikleri, 2020) Demircan Karadag, Sinem; Karadag, Cihan; Senturk, Mehmet Baki; Turgut, AbdulkadirObjective: Endometrial biopsy is a pain-related gynecological procedure. The aim of this study is to compare the efficacy of rectal indomethacin and intrauterine lidocaine on pain control during endometrial sampling. Material and Methods: This clinical observational prospective trial was conducted in 120 patients with the complaint of abnormal uterine bleeding who presented at Istanbul Medeniyet University Goztepe Training and Research Hospital, Obstetrics and Gynecology Department, between January 2017 and June 2017. Patients were divided into two groups. The rectal group had 64 patients, to whom indomethacin was given as a local anesthetic. The intrauterine group had 56 patients, for whom lidocaine was the local anesthetic used. Endometrial biopsy was performed by using a no. 4 Karman cannula. The primary goal of measurement was pain intensity assessed by a visual analog scale before, during, and 10 minutes after the procedure. Results: There were no statistically significant differences between the two groups in mean age, body mass index, weight and median levels of gravidity, parity and abortion. The median pain scores of the two groups during and 10 minutes after the procedure were similar (p>0.05). Conclusion: In this study, rectal indomethacin was shown to be equal to intrauterine anesthesia for reducing pain during the endometrial biopsy. Rectal indomethacin may be an alternative drug to relieve pain during gynecological procedures. Further researches with larger numbers of patients are needed to confirm our findings.Article Cancer Rate in Pulmonary Embolism Diagnosed Cases: A Single-Center, Retrospective Study(Turkish Geriatrics Soc., 2025) Doğan, Demet; Gökçe, KağanIntroduction: The aim of this study was to evaluate the incidence of pulmonary embolism in relation to age, cancer, and other comorbidities; to assess clinical, laboratory, and radiological findings; and to examine the associated mortality rate. Materials and Method: Patients diagnosed with pulmonary embolism were included in this single-center retrospective study. The patients were divided into two age groups: < 60 and > 60 years. The incidence of pulmonary embolism; presence of deep vein thrombosis, cancer, surgical history; other comorbidities; mortality rate; prevalence of embolism; and C-reactive protein and D-dimer levels were evaluated according to age group and sex. Results: Of the 1,281 patients who underwent pulmonary computed tomography angiography for suspected pulmonary embolism, 235 were diagnosed with pulmonary embolism. Of these, 114 were female and 121 were male. The mean age was 62.3 ± 16.8 years. In the ≥60 age group, the proportion of females was significantly higher than that of males (p < 0.05). Cancer prevalence and C-reactive protein and D-dimer levels were also significantly higher in this group (p < 0.05). No statistically significant differences were observed between the age groups for the other evaluated parameters. Conclusion: The incidence of pulmonary embolism is higher in the elderly population, and the presence of additional conditions, particularly cancer, further increases the risk of pulmonary embolism. Careful monitoring of elderly patients for comorbidities, risk factors, and complications—such as pulmonary embolism—is crucial. Early diagnosis and timely treatment play vital roles in reducing mortality and significantly improving quality of life.Review Childhood Traumatic Experiences and Dissociative Symptoms in Patients with Parasomnia(Galenos Publ House, 2021) Dogan, Fatma; Icten, Sacit; Taycan, Serap ErdoganObjective: Parasomnias are sleep disorders that are characterised by abnormal or unwanted movements, behaviours, emotions, perceptions, dreams and autonomic nervous system dysfunction. No significant differences were found in terms of DES scores (p=0.456) and anxiety symptoms (p=0.934). A significant difference was noted in the scores of the sexual abuse (p=0.038) and emotional abuse (p=0.001) subscales of childhood trauma questionnaire. No significant difference was found in other subscales. Conclusion: The finding that childhood trauma is higher in patients in the parasomnia group than in those in the control group emphasises the importance of cooperation with psychiatrists in order to diagnose and treat these patients.Article The classical adiabatic constancy of PVγ for an ideal gas, can be shown to be a quantum mechanical occurrence, which yields the particular value of the constant, in question(2012) Yarman,T.; Kholmetskii,A.L.; Korfali,O.In this paper we find a full connection between the long lasting macroscopic classical laws of gases and the quantum mechanical description of non-interacting particles confined in a box, thus constituting an ideal gas. In such a gas, the motion of each individual molecule can be considered to be independent of all other molecules, and the macroscopic parameters of an ideal gas, mainly, pressure P and temperature T, can be defined as simple average quantities based on individual motions of all molecules in consideration. It is shown that for an ideal gas enclosed in a macroscopic cubic box of volume V, an alphanumeric expression for the Constant appearing in the classical law of adiabatic expansion law, i.e. PV5/3 = Constant, can be derived based on quantum mechanics. Note that this constant has otherwise remained for centuries, as just an abstract quantity in the form of P1V 15/3=P2V25/3 = P 3V35/3 written for different thermodynamic states, delineated through an adiabatic transformation. No one even seems to have thought that it may eventually have a particular expression. Physical implications of the result we disclose are discussed. © 2012 TIBTD Printed in Turkey.
