    Hemiplejik hastalarda torakal manuel terapinin solunum kapasitesi üzerine akut etkisinin incelenmesi

    Bu çalışmanın amacı, hemiplejik hastalara uygulanan torakal manuel tedavinin solunum kapasitesi üzerine akut etkilerini araştırmaktır. Çalışma Bayrampaşa Belediyesi Fizik Tedavi ve Engelliler Merkezi' inde, Brunnstrom evrelemesine göre alt ekstremite düzeyi Evre 4 ve Evre 6 arası olan, 31' i tedavi, 16'sı kontrol grubu 47 hasta katılımıyla gerçekleştirilmiştir. Hastaların klinik ve demografik bilgileri kaydedilmiştir. Hastalara göğüs çevre ölçümü yapılmıştır. Solunum fonksiyonları portatif spirometre ile ölçülmüştür. Solunum kas kuvveti ölçümünde elektronik basınç dönüştürücü kullanılmıştır. Kalp hızı, solunum sıklığı ve oksijen saturasyonu ölçümü pulse oksimetre ile yapılmıştır. Tedavi grubu akut etkilerini ölçmek için tek sefer manuel tedaviye alındı. Bütün değerlendirmeler tedavi öncesi ve manuel tedavinin hemen sonrası kör bir fizyoterapist tarafından yapılmıştır. Tedavi grubunda aksillar ve subkostal göğüs çevre ölçümlerinde, zorlu ekspiratuar kapasitenin birici saniyesinde, zorlu vital kapasitenin birici saniyesinin zorlu vital kapasiteye oranında ve tepe akım hızı değerleri MT seansı öncesine göre anlamlı düzeyde artmıştır. (p<0,05). Maximal ekspiratuar basınç değerleri MT seansı öncesine kıyasla anlamlı şeklde artmıştır. (p<0,05). Kas enduransı, kalp hızı, solunum sayısı ve saturasyon değerlerinde gruplar arasında anlamlı bir fark oluşmadı. (p>0,05) Hemiplejik hastalarda tek seans uygulanan torakal manuel tedavi solunum fonksiyonlarını ve inspiratuar kas kuvvetini akut olarak geliştirmiştir. Hemiplejik hastalara uygulanan tedavilere ek olarak torakal manuel tedavi eklenmesi hastalarda olumlu gelişmelere neden olabilir.

    Karaciğer transplantasyonu sonrası uygulanan fizyoterapi programının hareket düzeyi, kinezyofobi ve fiziksel uygunluk üzerine etkisi

    Bu çalışmanın amacı, karaciğer transplantasyonu sonrası solunum egzersizlerine ek olarak uygulanan fizyoterapi programının hareket düzeyi, kinezyofobi ve fiziksel uygunluk üzerine olan etkilerini araştırmaktır. Çalışmaya karaciğer nakli olan 40 birey dahil edildi. Bireyler basit randomizasyon yöntemi ile iki gruba ayrıldı. Çalışma grubuna pulmoner rehabilitasyon ve egzersiz uygulanırken, kontrol grubuna sadece pulmoner rehabilitasyon uygulandı (4 hafta boyunca haftada 5 gün). Bireylerin fiziksel ve demografik özellikleri kaydedildi. Hareket düzeyi, kinezyofobi ve fiziksel uygunluk sırasıyla 'Hasta Hareketlilik Ölçeği ve Gözlemci Hareketlilik Ölçeği', 'Tampa Kinezyofobi Ölçeği (TKÖ)', 'Senior Fitness Testi (SFT)' kullanılarak değerlendirildi. Çalışmanın sonucunda 'Hasta Hareketlilik Ölçeği' sonuçlarında ağrı toplam skoru, zorluk yaşama derecesi toplam skoru ve genel toplam skorlarının tamamında kontrol grubuna oranla çalışma grubundaki gelişim anlamlı farklılık göstermiştir (p<0.05). Gözlemci Hareketlilik Ölçeği toplam skorlarında tedavi öncesi ve sonrası her iki grupta da ileri derecede istatiksel anlamlı gelişme görülmüştür (p<0,001). Tampa Kinezyofobi Ölçeği skorları tedavi öncesi ve sonrası olarak kıyaslandığında her iki grupta da grup içi anlamlı bir azalma görülürken (p<0,001), gruplar arasında anlamlı bir farklılık bulunmamıştır (p>0,05). Senior Fitness Test sonuçları gruplar arası değerlendirmede, aerobik endurans, alt ekstremite kas enduransı, dinamik denge ve çeviklik değerlendirmelerinde çalışma grubundaki gelişme kontrol grubuna kıyasla anlamlı şekilde yüksekti (p:0,026; p:0,001). Sonuç olarak, karaciğer transplantasyonu sonrası solunum egzersizlerine ek olarak uygulanan fizyoterapi programı hareket düzeyi, kinezyofobi ve fiziksel uygunluk üzerine anlamlı etkiye sahiptir.

    Kronik boyun ağrılı bireylerde nöral mobilizasyonun solunum parametreleri, ağrı ve boyun farkındalığı üzerine etkisi

    Çalışmanın amacı nöral mobilizasyonun kronik boyun ağrısı olan bireylerde solunum parametreleri, ağrı ve boyun farkındalığı üzerindeki etkisini incelemekti. Çalışmaya katılan kronik boyun ağrılı 26 birey deney ve kontrol grubu olarak ikiye ayrıldı. Kontrol grubuna (n=13) konvansiyonel (5gün/haftax3) tedavi uygulandı. Deney grubuna (n=13) konvansiyonel tedaviye ek olarak 10 seans nöral mobilizasyon uygulandı. Servikal bölge eklem hareket açıklığı değerlendirilmesinde gonyometre, ağrı şiddeti değerlendirilmesinde visual analog skalası (VAS), kas kuvveti değerlendirmesinde manuel kas testi, boyun farkındalıkları değerlendirmesinde Fremantle Boyun Farkındalık Anketi (FreBFA) kullanıldı. Spirometre ile solunum fonksiyonları değerlendirildi. Tedavi sonrası her iki grupta da ağrı ve boyun eklem hareketi açıklığında anlamlı fark görüldü (p<0.05). Solunum fonksiyon testlerinde ve boyun farkındalığında gruplar arasında anlamlı farklılık görüldü (p<0.05). Çalışmanın sonucunda kronik boyun ağrılı hastalarda, konvansiyonel fizyoterapi ile birlikte uygulanan nöral mobilizasyonun; eklem hareket açıklığı, ağrı, boyun farkındalığı ve solunum fonksiyonları üzerinde yararlı etkileri olabileceği düşünüldü.

    Effectiveness of Robot-Assisted Gait Training on Functional Skills in Children with Cerebral Palsy

    This study was aimed to investigate the effects of robot- assisted gait training (RAGT) on motor functions, spasticity status, balance, and functionality in children with cerebral palsy (CP). A total of 26 patients who were diagnosed with CP ( diplegic, with gross motor function classification system [GMFCS] levels of 2-5) and who regularly participated in a rehabilitation program were recruited in the study after obtaining approval from their parents. The patients were randomly assigned to two groups. Group 1 (n = 13) received conventional physical therapy (65 minutes, 2 days/week x 8) and group 2 (n = 13) received 25minutes of RAGT (RoboGait) in addition to conventional therapy (CT; 40 minutes, 2 days/week x 8). GMFCS was used to evaluate motor functions and the Modified Ashworth Scale was used to evaluate spasticity. The pediatric Berg balance scale, pediatric functional independence measure, and timed up and go tests were employed to assess balance and functional status. The evaluations were performed at baseline and after 8 weeks of therapy. Both rehabilitation methods led to a statistically significant decrease in spasticity (p< 0.05); however, there was no difference in this improvement of spasticity between the groups (p> 0.05). Both groups exhibited significant improvements in functional independence, balance, and performance at the end of therapy (p< 0.05), and there was no significant difference between the groups (p> 0.05). The results of this study show that addition of RAGT to CT for 8 weeks is not superior to CT alone in children with CP.

    Are the Lumbar Regional Muscles’ Endurance, Flexibility and Abdominal Adiposis Related to Disability in Fibromyalgia Syndrome?

    Objective: The patients who have widespread pain around the thoracic and cervical region came to the physical therapy clinic with the diagnosis of fibromyalgia syndrome complain about symptoms reappear after a period of time. This situation revealed the idea of evaluating the lumbar region as a more central segment and applying treatment to the lumbar region. The main purpose is to examine the endurance, flexibility and abdominal fat levels of lumbar muscles in women with fibromyalgia and investigate the relationship of these parameters with disability caused by fibromyalgia. Material and Methods: The study included 40 female patients with fibromyalgia syndrome, aged 24-45, who met the American College of Rheumotology 2010 diagnostic criteria. functional disability of patients was evaluated with Revised Fibromyalgia Impact Questionnaire (FIQR). The endurance of the lumbar region muscles was assessed by the Biering-Sorensen Test, flexibility by the sit and reach test, spinal mobility by the Modified Schober Test. With the skinfold caliper (Accu-MeasureR) abdominal region fat mass measurement, with the tape measure waist and hip circumference measurement were recorded. Results: There was a positive correlation between FIQR score and waist circumference (p<0.001, r=0.568), hip circumference (p<0.001, r=0.567), waist/hip ratio (p=0.018, r=0.370), skinfold measurements (p<0.001, r=0.869). On the other hand, a negative correlation between FIQR score and endurance (p<0.001, r=-0.969) and flexibility (p<0.001, r=-0.950) were viewed. Conclusion: Decrease in endurance and flexibility of the lumbar extensor muscle group and the increase in abdominal adipose tissue in fibromyalgia syndrome increase the severity and disability of disease.

    EFFECTS OF KINESIO TAPING ON POSTURAL STABILITY IN YOUNG SOCCER PLAYERS

    Background: This study aims to investigate the effects of Kinesio Taping on postural stability in young soccer players. Methods: 62 volunteer soccer players from Cayyolu Sports Club were included in our study. Players were randomly allocated into two groups; a Kinesio taping (KT) (n=31), and a control group (n = 31). KT application including gastrocnemius and quadriceps femoris muscle facilitation techniques were applied to the first group. The second group was set as the control group and no application was made. Sociodemographic characteristics, postural stability and thermographic assessment were evaluated prior to and after the application of the Kinesio Taping. All the statistical analyses were set a priori at an alpha level of p<0.05. The tests for homogeneity (Levene's test) and normality (Shapiro-Wilk) were used to determine the appropriate statistical methods to apply for comparison between the groups. According to the test results, the parametric paired sample T-test was used to compare between baseline and post-treatment within a group, while independent sample T-test for equality of means was used for comparison between the groups. Results: No statistically significant differences were observed between the groups at baseline regarding age, body mass index, skin temperature and postural stability (p > 0.05). Statistically significant differences were found in all the assessed parameters in both Kinesio Taping and control groups (all p<0.05) except thermal imaging of dominant gastrocnemius muscle results (p=0.668) after the Kinesio application. Statistically significant differences were found in all parameters across groups (all p<0.05). Conclusions: Kinesio Taping may increase postural stability and decrease the risk of injury. According to thermal imaging assessments, both quadriceps and gastrocnemius muscles may be active in maintaining postural stability. As the decreasing risk of injury is very important for young soccer players, our results may show a preventive measure for sports physiotherapists working on soccer fields.

    The acute effects of different intensity whole-body vibration exposure on muscle tone and strength of the lower legs, and hamstring flexibility: A pilot study

    Context: The research on the change in properties of the lower leg muscles by different intensity sinusoidal vertical whole-body vibration (SV-WBV) exposures has not yet been investigated. Objective: The purpose of this study was to determine effect of a 20-minute different intensity SV-WBV application to the ankle plantar flexor and dorsiflexor muscles properties and hamstring flexibility. Design: Prospective preintervention-postintervention design. Setting: Physiotherapy department. Participants: A total of 50 recreationally active college-aged individuals with no history of a lower leg injury volunteered. Interventions: The SV-WBV was applied throughout the session with an amplitude of 2 to 4 mm and a frequency of 25 Hz in moderate-intensity vibration group and 40 Hz in a vigorous-intensity vibration group. Main Outcome Measures: The gastrocnemius and tibialis anterior muscle tone was assessed with MyotonPRO, and the strength evaluation was made on the same lower leg muscles using hand-held dynamometer. The sit and reach test was used for the lower leg flexibility evaluation. Results: The gastrocnemius muscle tone decreased on the right side (d=0.643, P=.01) and increased on the left (d = 0.593, P=.04) when vigorous-intensity vibration was applied. Bilateral gastrocnemius muscle strength did not change in both groups (P > .05). Without differences between groups, bilateral tibialis anterior muscle strength increased in both groups (P < .01). Bilateral gastrocnemius and tibialis anterior muscle tone did not change in the moderate-intensity vibration group (P > .05). Flexibility increased in both groups (P < .01); however, there was no statistically significant difference between the groups (d = 0.169, P = .55). Conclusions: According to study results, if SV-WBV is to be used in hamstring flexibility or ankle dorsiflexor muscle strengthening, both vibration exposures should be preferred. Different vibration programs could be proposed to increase ankle plantar flexor muscle strength in the acute results.     EFFECTS OF SUBSCAPULARIS MUSCLE SOFT TISSUE MOBILIZATION ON PAIN AND FUNCTIONALITY IN SHOULDER DYSFUNCTION

    Purpose: Tightness of the subscapularis muscle causes glenohumeral external rotation limitation and difficulties in over-head activities. This study aimed to determine the effects of soft tissue mobilization applied to the subscapularis muscle on pain and functionality in shoulder dysfunctions. Methods: The 48 patients with shoulder pain and limitation were included in the study. They were randomly divided into conventional physiotherapy program (CPP) group (n=25) and soft tissue mobilization (STM) group (n=23). The first group received CPP and the second group received STM. All patients had treatment at a physiotherapy clinic for 15 sessions. The pain was evaluated using Visual Analogue Scale (VAS), shoulder joint movements were measured using a goniometer and overhead reach test, and functionality was evaluated using the Shoulder Pain and Disability Index (SPADI). The patients were assessed before, immediately after, and three weeks after the treatment. Results: Statistically significant improvements were found in VAS, shoulder flexion and extemal rotation range of motion, and overhead reach test in both groups immediately after treatment and at the end of the 3rd week (p<0.05). When groups were compared after three weeks, statistically significant improvements were found in VAS, external rotation range of motion, and overhead reach test in the STM group (p<0.05). There was a statistically significant improvement in SPADI in both groups after the treatment (p<0.05). There was no significant difference in SPADI score between the groups (p>0.05). Conclusion: The STM was more effective on pain, range of motion, and functionality than CPP. STM of subscapularis muscle might be an alternative treatment of the shoulder dysfunction.

    THE EFFECTS OF INSTRUMENT-ASSISTED SOFT TISSUE MOBILIZATION AND KINESIO TAPING ON PAIN, FUNCTIONAL DISABILITY AND DEPRESSION IN PATIENTS WITH CHRONIC LOW BACK PAIN: A RANDOMIZED TRIAL

    Purpose: Low back pain is a common condition that can become chronic, which reduces the life quality of the patient by causing functional disability and depression. This study aimed to investigate the effects of instrument-assisted soft tissue mobilization (IASTM) and kinesio taping (KT) along with conservative treatment in patients with chronic nonspecific low back pain (CNLBP). Methods: A total of 30 patients with chronic low back pain aged between 30-50 years included in the study were randomized to IASTM (n=15) and KT (n=15) groups. Both the groups underwent conservative treatment that comprised of a hot pack, ultrasound, transcutaneous electrical nerve stimulation (TENS) and home exercises. In this study, assessments were made using the Visual Analogue Scale (VAS), Roland Morris Disability Questionnaire (RMDQ), and Beck Depression Inventory (BDI). The measurements were carried out at the beginning and end of the treatment. Results: As a result of the study, IASTM method improved in parameters such as pain (p<0.05), functionality (p=0.001) and depression (p<0.05). As a result of the study, the KT group improved in parameters such as pain, functionality and depression compared to pre-treatment (p<0.05). However, the two treatments were not superior to each other in pain (p=0.241), functionality (p=0.687) and depression (p=0.699) parameters. Conclusion: It has been observed that both treatments have positive effects on many parameters such as pain, disability, depression and mental state in patients with CNLBP. This study demonstrates that IASTM and KT treatments can be used to support therapeutic effects in patients with CNLBP.
