Investigation of the attitudes of physicians and nurses working in a university hospital towards change
Abstract
Bu tez çalışmasında, Kocaeli'ndeki bir araştırma ve uygulama hastanesinde çalışmakta olan hekim ve hemşirelerin değişime yönelik tutumlarını, bu tutumlarda değişikliğe neden olan kişisel ve mesleki faktörleri belirlemek amaçlanmıştır. Bu çalışmada, ilgili hastanede çalışan 596 hemşireye ve 399 hekime ulaşmak amaçlanmış ancak 92 hekim ve 158 hemşireden sağlanan verilerle çalışma tamamlanmıştır. Veriler, tanımlayıcı bilgi formu ve Değişime Karşı Tutum Ölçeği kullanılarak toplanmıştır. Verilerin analizinde tanımlayıcı istatistikler (yüzde, ortalama, standart sapma), Kolmogorov Simirnov testi, Mann Whitney U testi, Kruskal Wallis tek yönlü varyans analizi ve Dunn's Post Hoc testi kullanılmıştır. Ölçeğin iç tutarlılık analizi Cronbach Alfa katsayısı ile hesaplanmıştır. Ankete katılan hekimlerin Değişime Karşı Tutum Ölçeği toplam puanı ortalamasının 54,29±11,54 olduğu, hemşirelerin toplam puan ortalamasının 59,98±12,31 olduğu saptanmıştır. Değişime karşı hemşirelerin, hekimlerden daha olumlu oldukları görülmüştür. Gelir durumu, yaş grubu, kurum, birim ve ücret memnuniyeti, çalışma koşulları, çalışma şekilleri, çalışma pozisyonları, meslekte çalışma süresi, kurumda çalışma süresi gibi değişkenlere göre yapılan karşılaştırmalarda ölçek skorlarında istatistiksel olarak anlamlı farklılıklar saptanmıştır (p<0,05). Kurumda çalışan hekim ve hemşirelerin değişime yönelik tutumlarının iyileştirilmesi, değişime yönelik tutumlarında değişikliğe neden olan faktörler göz önünde bulundurularak değişimin planlanması önerilir.
In this thesis, it is aimed to determine the attitudes of physicians and nurses who work in a research and application hospital in Kocaeli, towards change and the personal and professional factors causing the changes of attitudes. In this study, it was aimed to reach 596 nurses and 399 physicians working in the hospital, but the study was completed with the data provided from 92 physicians and 158 nurses. Data were collected using descriptive information form and Attitude to Change Scale. Descriptive statistics (percentage, mean, standard deviation) were used for the analysis of the data and the distribution of the variables was measured by Kolmogorov Simirnov test. Pearson, Yates, Monte Carlo Chi-square test and tests were used. Internal consistency analysis of the scale was calculated with Cronbach's alpha coefficient. The mean score of physicians form the Attitude towards Change Scale was found to be 54.29 ± 11.54. The mean score of the nurses was 59.98 ± 12.31. Nurses were more positive than physicians towards change. Statistically significant differences were found between the scale scores in the comparisons made according to income status, age group, institution, unit and wage satisfaction, working conditions, working styles, working positions, working time in the profession, working time in the institution (p <0.05). It is recommended to improve the attitudes of physicians and nurses working at the institution towards change, and to plan the change considering the factors affecting their attitudes towards change.
